Influence of Workplace Cardiovascular Screening on Cardiovascular Disease Detection among Bank Employees in Nigeria

 

Abstract

Cardiovascular disease is an important public health concern in Nigeria and a major contributor to illness, disability, and premature mortality among adults. Bank employees may be exposed to occupational and lifestyle factors that increase cardiovascular risk, including prolonged sedentary work, occupational stress, extended working hours, physical inactivity, unhealthy dietary practices, sleep disturbances, and other work-related pressures. Cardiovascular risk factors may remain undetected because many affected employees do not undergo regular preventive health assessments, particularly when early disease is asymptomatic. Undetected cardiovascular risk and disease may subsequently contribute to hypertension, coronary heart disease, stroke, heart failure, and other cardiovascular complications. Workplace cardiovascular screening provides an opportunity to identify cardiovascular risk factors and possible disease among bank employees through routine occupational health assessments, employee wellness programmes, periodic medical examinations, blood pressure measurement, anthropometric assessment, blood glucose and lipid testing, cardiovascular risk assessment, and appropriate referral for further clinical evaluation. Early detection may facilitate timely lifestyle modification, medical management, risk-factor control, and follow-up. However, inadequate workplace health programmes, limited screening coverage, shortage of trained healthcare personnel, insufficient diagnostic resources, low employee participation, and weak referral and follow-up systems may limit the effectiveness of workplace cardiovascular screening in Nigeria. Against this background, this study investigates the influence of workplace cardiovascular screening on cardiovascular disease detection among bank employees in Nigeria. The study will be anchored on Occupational Health Theory, the Health Belief Model, and the Health Systems Framework. Occupational Health Theory emphasizes the prevention, identification, monitoring, and management of workplace-related health risks and the promotion of workers' health and well-being. The Health Belief Model explains how bank employees' perceptions of susceptibility to cardiovascular disease, perceived severity, perceived benefits of cardiovascular screening, perceived barriers, and cues to action may influence participation in workplace screening programmes. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective workplace cardiovascular screening and prevention programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace cardiovascular screening may influence cardiovascular disease detection among bank employees in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise bank employees aged 18 years and above working in selected commercial banks and other licensed banking institutions across Nigeria. A multistage sampling technique will be used to select states, banking institutions, branches, departments, and eligible employees. Workplace cardiovascular screening will be measured using indicators such as availability of cardiovascular screening programmes, screening coverage, frequency of screening activities, blood pressure measurement, body mass index and waist circumference assessment, blood glucose testing, lipid profile assessment, cardiovascular risk assessment, assessment of lifestyle and occupational risk factors, availability of trained healthcare personnel, screening equipment, cardiovascular health education, and referral arrangements. Cardiovascular disease detection will be assessed using indicators such as elevated blood pressure, abnormal glucose or lipid findings, obesity and central adiposity, identified cardiovascular risk profiles, newly suspected cardiovascular conditions, previously undetected cardiovascular disease, abnormal clinical findings requiring further assessment, referrals for comprehensive cardiovascular evaluation, confirmed cardiovascular disease where appropriate, and linkage to treatment and follow-up services. Data will be collected using structured questionnaires, workplace screening registers, medical examination forms, anthropometric records, laboratory reports, occupational health records, referral registers, and relevant organizational documents. Descriptive statistics will be used to summarize employees' characteristics, occupational factors, screening coverage, cardiovascular findings, and patterns of cardiovascular disease detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of workplace cardiovascular screening on cardiovascular disease detection. Where appropriate, cardiovascular disease detection rates before and after implementation of workplace cardiovascular screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that workplace cardiovascular screening has a significant positive influence on cardiovascular disease detection among bank employees in Nigeria. Bank employees who participate in workplace cardiovascular screening are expected to have a higher likelihood of having previously undetected cardiovascular risk factors or cardiovascular abnormalities identified than employees without access to routine screening. Regular assessment of blood pressure, body composition, glucose levels, lipid levels, and other cardiovascular risk indicators may facilitate early identification of employees requiring further clinical evaluation. Early detection may provide opportunities for lifestyle counselling, dietary modification, increased physical activity, stress management, weight management, appropriate medical treatment, and continued monitoring. Conducting cardiovascular screening within the workplace may reduce time-related, financial, and accessibility barriers and reach employees who may not routinely seek preventive healthcare. Workplace cardiovascular health education may also improve employees' awareness of cardiovascular risk factors and encourage healthier lifestyle practices. Conversely, inadequate screening coverage, limited laboratory and diagnostic resources, shortage of trained healthcare personnel, low employee participation, occupational time constraints, and weak referral and follow-up systems may reduce the effectiveness of workplace cardiovascular screening. The study therefore expects accessible and well-organized workplace cardiovascular screening to contribute significantly to improved detection and early management of cardiovascular disease and its risk factors among bank employees in Nigeria. The study is expected to contribute to the literature on workplace cardiovascular screening, cardiovascular disease detection, bank employees' health, occupational health, cardiovascular risk assessment, non-communicable disease prevention, workplace health promotion, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, Central Bank of Nigeria, commercial banks, occupational health practitioners, cardiologists, healthcare providers, workplace safety officers, human resource managers, development partners, and policymakers regarding strategies for improving cardiovascular health among bank employees. The study will also provide evidence-based recommendations for expanding workplace cardiovascular screening programmes, improving access to appropriate cardiovascular assessments, strengthening employee health education, promoting physical activity and healthy dietary practices, addressing occupational stress and sedentary behaviour, improving access to trained healthcare personnel and diagnostic resources, strengthening referral and follow-up systems, and integrating cardiovascular screening into comprehensive workplace health and wellness programmes across Nigeria.
