Influence of School-Based Health Risk Assessment on Early Health Problem Detection among Adolescents in Nigeria

Abstract

Adolescence is an important stage of physical, psychological, and social development during which various health problems and risk factors may emerge or become more apparent. Early identification of health problems among adolescents can support timely intervention, prevent complications, and improve educational participation and overall well-being. However, many adolescents in Nigeria may have undetected health conditions because of limited access to preventive healthcare, inadequate health awareness, financial barriers, stigma, and insufficient routine health assessments. School-based health risk assessment provides an accessible opportunity to identify potential health problems and risk factors among adolescents within the school environment. Such assessment may include evaluation of physical health, nutritional status, mental and psychosocial well-being, vision and hearing, respiratory symptoms, blood pressure, lifestyle behaviours, and other age-appropriate health indicators. Early detection through school-based assessment may facilitate counselling, health education, referral, treatment, and follow-up. However, inadequate school health programmes, limited healthcare personnel, insufficient assessment tools, privacy concerns, parental consent challenges, and weak referral systems may limit the effectiveness of school-based health risk assessment in Nigeria. Against this background, this study investigates the influence of school-based health risk assessment on early health problem detection among adolescents in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and the Health Systems Framework. The Health Belief Model explains how adolescents' perceptions of susceptibility to health problems, perceived severity, perceived benefits of health assessment, perceived barriers, and cues to action may influence participation in school-based health risk assessment. The Social Ecological Model emphasizes the influence of individual, family, peer, school, community, socioeconomic, and environmental factors on adolescent health behaviours, healthcare utilization, and access to preventive services. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective school-based health assessment, referral, and follow-up programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how school-based health risk assessment may influence early health problem detection among adolescents in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adolescents aged 10 to 19 years enrolled in selected public and private secondary schools across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, schools, classes, and eligible adolescents. School-based health risk assessment will be measured using indicators such as availability of school health assessment programmes, assessment coverage, frequency of health assessments, physical health evaluation, nutritional assessment, blood pressure measurement, vision and hearing assessment, mental and psychosocial health assessment, lifestyle-risk assessment, availability of trained healthcare personnel, assessment tools, health education, confidentiality measures, and referral arrangements. Early health problem detection will be assessed using indicators such as newly identified physical health conditions, nutritional problems, elevated blood pressure, visual or hearing abnormalities, respiratory symptoms, psychosocial or mental health concerns, unhealthy lifestyle risk factors, other clinically relevant health findings, referrals for further evaluation, confirmed health conditions where appropriate, and linkage to appropriate healthcare services. Data will be collected using structured questionnaires, school health assessment forms, screening registers, school health records, healthcare facility records, referral registers, and relevant education and health programme documents. Descriptive statistics will be used to summarize adolescents' characteristics, health risk factors, assessment coverage, identified health problems, and patterns of early detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of school-based health risk assessment on early health problem detection. Where appropriate, detection rates before and after implementation of school-based health risk assessment activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school-based health risk assessment has a significant positive influence on early health problem detection among adolescents in Nigeria. Adolescents who participate in school-based health risk assessment are expected to have a higher likelihood of previously unrecognized health conditions or risk factors being identified than adolescents without access to routine school-based assessment. Systematic assessment may facilitate early recognition of nutritional problems, elevated blood pressure, visual and hearing abnormalities, respiratory symptoms, mental and psychosocial concerns, and other health conditions that may otherwise remain undetected. Early identification may provide opportunities for health education, counselling, lifestyle modification, clinical assessment, treatment, and appropriate referral. Conducting assessments within schools may reduce geographical, financial, and time-related barriers to preventive healthcare and reach adolescents who may not routinely visit healthcare facilities. School-based health education may also improve adolescents' knowledge of health risks and encourage appropriate healthcare-seeking behaviour. Conversely, inadequate assessment coverage, shortage of trained healthcare personnel, limited assessment equipment, privacy concerns, low student participation, inadequate parental involvement, and weak referral and follow-up systems may reduce the effectiveness of school-based health risk assessment. The study therefore expects accessible and well-organized school-based health risk assessment to contribute significantly to improved early detection and management of health problems among adolescents in Nigeria. The study is expected to contribute to the literature on school-based health risk assessment, early health problem detection, adolescent health, school health services, preventive healthcare, health promotion, adolescent wellness, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Primary Health Care Development Agency, state ministries of health and education, secondary schools, school health coordinators, nurses, community health workers, healthcare providers, parents, development partners, and policymakers regarding strategies for strengthening adolescent health services. The study will also provide evidence-based recommendations for expanding school-based health risk assessment programmes, improving availability of trained healthcare personnel and appropriate assessment tools, strengthening adolescent health education, protecting privacy and confidentiality during assessments, improving parental and school participation, strengthening referral and follow-up systems, and integrating comprehensive age-appropriate health risk assessment into school health programmes across Nigeria.
