Influence of Public Health Surveillance Education on Disease Reporting Practices among Community Health Workers in Nigeria
Abstract
Public health surveillance is a fundamental component of disease prevention and control because it enables the systematic collection, analysis, interpretation, and reporting of health information for timely public health action. Community health workers play an important role in surveillance systems because of their close contact with households and communities, which places them in a strategic position to identify suspected disease cases, recognize unusual health events, collect relevant information, and report health events to appropriate authorities. Public health surveillance education provides community health workers with knowledge and practical training on disease recognition, standard case definitions, identification of suspected cases, surveillance procedures, data collection, documentation, notification requirements, reporting channels, reporting timelines, confidentiality, infection prevention, referral procedures, and communication with surveillance officers. Disease reporting practices refer to the behaviours and procedures through which community health workers identify, document, notify, and report suspected cases and other relevant health events accurately and promptly through established surveillance systems. In Nigeria, weaknesses in disease reporting may be associated with inadequate surveillance training, limited knowledge of reporting procedures, poor access to reporting tools, weak communication networks, insufficient supervision, workload, and limited feedback from higher levels of the health system. Effective public health surveillance education may improve community health workers' knowledge and competence and encourage accurate and timely disease reporting. Against this background, this study investigates the influence of public health surveillance education on disease reporting practices among community health workers in Nigeria. The study is anchored on Social Cognitive Theory, Experiential Learning Theory, and the Knowledge-Attitude-Practice framework. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, observational learning, professional behaviour, social influences, and workplace environments in shaping disease reporting practices. Experiential Learning Theory provides a framework for understanding how practical demonstrations, case scenarios, simulations, field exercises, and repeated reporting practice facilitate the acquisition and retention of surveillance competencies. The Knowledge-Attitude-Practice framework provides a basis for examining how public health surveillance education may improve knowledge and attitudes and translate these improvements into appropriate disease reporting practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how public health surveillance education may influence disease reporting practices among community health workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from community health workers working in selected primary healthcare facilities and communities in Nigeria using structured questionnaires, public health surveillance knowledge assessments, case-based reporting exercises, and standardized disease reporting practice checklists. A multistage sampling technique will be employed to select states, local government areas, primary healthcare facilities, communities, and eligible community health workers. Where feasible, selected community health workers or healthcare facilities will be assigned to intervention and comparison groups. Participants in the intervention group will receive structured public health surveillance education through lectures, demonstrations, case-based learning, simulation exercises, field practice, and supervised reporting activities. The education will cover disease recognition, standard case definitions, identification of suspected cases, event-based surveillance, data collection, documentation, disease notification, reporting timelines, appropriate reporting channels, use of paper-based and electronic reporting tools, confidentiality, infection prevention, referral procedures, and communication with surveillance authorities. Disease reporting practices will be assessed using indicators such as accurate identification of reportable health events, completeness of reporting forms, accuracy of recorded information, timeliness of notification, use of appropriate reporting channels, adherence to reporting procedures, referral of suspected cases, follow-up of reported cases, and communication with relevant surveillance personnel. Where feasible, facility registers, surveillance records, reporting forms, electronic reporting records, or supervisory assessments will complement self-reported practices to improve the objectivity of the findings. Descriptive statistics will be used to summarize participants' sociodemographic and professional characteristics, previous surveillance training, surveillance knowledge, and disease reporting practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the influence of public health surveillance education on disease reporting practices. Where appropriate, reporting practices before and after the intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, inter-rater reliability of practical assessments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that public health surveillance education has a significant positive influence on disease reporting practices among community health workers in Nigeria. Community health workers exposed to structured surveillance education are expected to demonstrate improved understanding of reportable diseases, standard case definitions, reporting requirements, notification procedures, reporting timelines, and appropriate reporting channels. Education is expected to improve the accuracy, completeness, and timeliness of disease reports and strengthen communication between community health workers and formal surveillance authorities. Practical and simulation-based education is expected to be particularly effective because it enables community health workers to practise identifying and reporting suspected cases under realistic conditions and receive immediate feedback. However, education alone may not guarantee sustained improvement in reporting practices because community health workers may continue to experience barriers such as inadequate reporting forms and equipment, limited access to electronic reporting platforms, unreliable communication networks, transportation challenges, heavy workloads, inadequate supervision, and delayed feedback from surveillance authorities. Consequently, public health surveillance education may be most effective when supported by functional reporting systems, adequate surveillance tools, regular supportive supervision, reliable communication networks, prompt feedback, and effective integration of community health workers into national and subnational disease surveillance systems. The study is expected to contribute to the literature on public health surveillance education, disease reporting practices, community health workers, disease surveillance, epidemiology, primary healthcare, health education, disease prevention, and public health in Nigeria by providing empirical evidence on the influence of surveillance education on disease reporting practices. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, National Primary Health Care Development Agency, state ministries of health, state primary healthcare development agencies, local government health authorities, primary healthcare facilities, community health programmes, epidemiologists, public health practitioners, health educators, development partners, and policymakers regarding strategies for improving disease reporting at the community level. The study will also provide evidence-based recommendations for strengthening public health surveillance education in community health worker training programmes, introducing regular refresher courses and practical reporting exercises, improving access to reporting tools and digital surveillance platforms, strengthening supportive supervision and feedback mechanisms, and enhancing the capacity of community health workers to provide accurate and timely disease reports in Nigeria.
