Influence of Dental Health Education on Dental Service Utilization among University Students in Nigeria

 

Abstract

Oral health is an important component of overall health and well-being, yet dental diseases such as dental caries, periodontal disease, and other oral conditions remain common public health concerns. University students may be at risk of poor oral health because of inadequate oral hygiene, unhealthy dietary practices, stress, tobacco and alcohol use, limited awareness of preventive dental care, and infrequent utilization of dental services. Dental health education provides students with information about oral diseases, risk factors, preventive measures, appropriate oral hygiene practices, the importance of routine dental check-ups, and the need for early treatment of dental problems. Dental service utilization refers to the use of preventive, diagnostic, and treatment services provided by qualified dental professionals, including routine dental examinations, professional cleaning, treatment of dental conditions, and other appropriate oral healthcare services. In Nigeria, limited knowledge, misconceptions about dental care, fear of dental procedures, cost of services, perceived lack of need for routine dental visits, and limited access to dental facilities may discourage university students from utilizing dental services. Effective dental health education may improve oral health knowledge, perceived need for professional care, and utilization of appropriate dental services. Against this background, this study investigates the influence of dental health education on dental service utilization among university students in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains healthcare utilization through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding students' decisions to seek dental care. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from dental health education may influence dental service utilization. Collectively, these theoretical perspectives provide a suitable framework for explaining how dental health education may influence dental service utilization among university students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from undergraduate students in selected universities in Nigeria using a structured self-administered questionnaire and, where feasible, verification of dental service attendance through available institutional records. A multistage sampling technique will be employed to select states, universities, faculties or departments, academic levels, and eligible students. Where feasible, selected student groups will be assigned to intervention and comparison groups. Students in the intervention group will receive structured dental health education covering common oral diseases, risk factors, oral hygiene practices, dietary factors affecting oral health, tobacco-related oral health risks, preventive dental care, importance of routine dental examinations, professional cleaning, early treatment, dental anxiety, and available dental services. Dental service utilization will be assessed using indicators such as attendance for routine dental examinations, preventive dental visits, professional dental cleaning, treatment-seeking for oral health problems, frequency of dental visits, and utilization of available university or community dental services. Descriptive statistics will be used to summarize students' sociodemographic characteristics, oral health knowledge, exposure to education, and dental service utilization. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of dental health education on dental service utilization. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that dental health education has a significant positive influence on dental service utilization among university students in Nigeria. Students exposed to structured dental health education are expected to demonstrate improved knowledge of oral health and greater awareness of the importance of preventive dental care. Education may encourage students to attend routine dental examinations, seek professional care earlier when oral health problems occur, utilize preventive services, and overcome misconceptions and fear associated with dental treatment. However, education alone may not guarantee increased utilization because students may continue to face barriers such as cost, limited availability of dental facilities, distance to dental clinics, fear of dental procedures, academic schedules, and the perception that dental care is necessary only when pain or serious symptoms occur. Consequently, dental health education is expected to be most effective when supported by accessible, affordable, student-friendly dental services and convenient appointment systems. The study is expected to contribute to the literature on oral health, dental health education, healthcare utilization, university student health, preventive dentistry, and public health in Nigeria by providing empirical evidence on the influence of dental health education on dental service utilization among university students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Universities Commission, university administrations, university health services, dental schools, dental clinics, public health dentists, healthcare providers, development partners, and policymakers regarding strategies for improving oral healthcare utilization among young adults. The study will also provide evidence-based recommendations for strengthening university-based dental health education, integrating routine oral health promotion into student health programmes, improving access to affordable dental services, reducing dental anxiety and misconceptions, and promoting regular preventive dental care among university students across Nigeria.
