Influence of Community-Based Eye Screening on Visual Impairment Detection among Adults in Nigeria

Abstract

Visual impairment is an important public health concern in Nigeria and may result from preventable or treatable conditions such as uncorrected refractive errors, cataract, glaucoma, diabetic eye disease, and other ocular disorders. Undetected visual impairment can negatively affect mobility, productivity, independence, educational and occupational participation, and quality of life. Early identification of visual problems provides an opportunity for timely referral, diagnosis, treatment, corrective measures, and rehabilitation. Community-based eye screening provides an important approach for taking basic eye health assessment closer to populations that may have limited access to conventional eye-care facilities. Such services may be delivered through community outreach programmes, primary healthcare facilities, mobile clinics, workplaces, markets, community centres, and other accessible locations. However, limited awareness, geographical barriers, cost of eye-care services, inadequate screening equipment, shortages of eye-care professionals, and weak referral systems may contribute to delayed detection of visual impairment among adults in Nigeria. Visual impairment detection refers to the identification of individuals with reduced visual function through appropriate screening and subsequent clinical assessment where required. Against this background, this study investigates the influence of community-based eye screening on visual impairment detection among adults in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Health Systems Framework. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action and provides a framework for understanding adults' participation in eye screening. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how accessibility of community-based eye screening may influence detection of visual impairment. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical technologies, financing, and accessibility as important components of effective eye-care delivery. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based eye screening may influence visual impairment detection among adults in Nigeria. The study will adopt a quantitative cross-sectional or community-based analytical research design. The study population will comprise adults aged 18 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, and eligible participants. Community-based eye screening will be assessed using indicators such as availability and frequency of screening activities, accessibility of screening locations, availability of trained eye-care personnel, availability of visual acuity testing equipment, screening coverage, community mobilization, health education, screening procedures, referral services, and follow-up arrangements. Visual impairment detection will be assessed using indicators such as completion of visual acuity screening, identification of reduced visual acuity, identification of suspected ocular conditions, referral for comprehensive eye examination, and confirmed diagnosis of visual impairment where appropriate. Data will be collected using structured questionnaires, visual acuity screening records, community health-worker records, referral registers, primary healthcare records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, and visual impairment detection patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the influence of community-based eye screening on visual impairment detection. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based eye screening has a significant positive influence on visual impairment detection among adults in Nigeria. Adults who have access to regular and accessible community-based eye screening are expected to have a greater likelihood of having previously undetected visual impairment identified than adults who do not participate in screening activities. Bringing eye screening closer to communities may reduce geographical, financial, and time-related barriers to eye-care utilization. Community mobilization and health education may also increase awareness of common eye conditions and encourage adults to seek assessment even when visual problems are mild or asymptomatic. Early identification of visual impairment may facilitate timely referral for corrective lenses, medical treatment, surgical intervention, or other appropriate eye-care services. Conversely, inadequate screening equipment, shortages of trained personnel, low community participation, limited awareness, high referral costs, and weak follow-up systems may reduce the effectiveness of community-based eye screening. The study may also demonstrate that effective detection requires strong linkages between community screening activities and accessible comprehensive eye-care and diagnostic services. The study is expected to contribute to the literature on community-based eye screening, visual impairment detection, eye health, ophthalmology, community health, noncommunicable disease-related eye conditions, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, ophthalmologists, optometrists, ophthalmic nurses, primary healthcare providers, community health workers, eye-care programme managers, development partners, and policymakers regarding strategies for improving early detection of visual impairment. The study will also provide evidence-based recommendations for expanding community-based eye screening programmes, improving availability of screening equipment, strengthening training of community and primary healthcare workers, increasing public awareness of eye health, strengthening referral and follow-up systems, reducing financial and geographical barriers to comprehensive eye care, and integrating basic eye screening into routine community and primary healthcare services across Nigeria.
