Impact of Workplace Mental Health Education on Mental Health Help-Seeking among Healthcare Workers in Nigeria

Abstract

Mental health is an important component of healthcare worker well-being and may influence occupational functioning, quality of life, interpersonal relationships, and the delivery of safe and effective healthcare services. Healthcare workers may experience substantial psychological pressures arising from heavy workloads, long working hours, staff shortages, exposure to illness and death, occupational stress, workplace conflicts, and other challenging working conditions. Despite these pressures, healthcare workers may be reluctant to seek professional mental health support because of stigma, concerns about confidentiality, fear of professional consequences, limited knowledge of available services, misconceptions about mental health treatment, and difficulties accessing appropriate care. Workplace mental health education provides healthcare workers with information about common mental health conditions, psychological distress, warning signs, stress and burnout, available mental health services, confidentiality, treatment options, referral pathways, and the importance of seeking timely professional assistance. Mental health help-seeking refers to the willingness and actions of healthcare workers to seek appropriate professional or formal support when experiencing psychological distress, mental health symptoms, or other significant emotional difficulties. In Nigeria, limited workplace mental health education and inadequate awareness of available support services may contribute to delays or reluctance in seeking appropriate mental healthcare among healthcare workers. Effective workplace mental health education may improve knowledge, reduce misconceptions and stigma, increase awareness of available services, and encourage appropriate help-seeking. Against this background, this study investigates the impact of workplace mental health education on mental health help-seeking among healthcare workers in Nigeria. The study is anchored on the Health Belief Model, Theory of Planned Behaviour, and Knowledge-Attitude-Practice framework. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding healthcare workers' decisions to seek mental health support. The Theory of Planned Behaviour explains behavioural intention through attitudes, subjective norms, and perceived behavioural control, providing a framework for understanding healthcare workers' willingness to seek professional mental health assistance. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from workplace mental health education may influence mental health help-seeking. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace mental health education may influence mental health help-seeking among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from healthcare workers in selected hospitals and healthcare facilities in Nigeria using a structured anonymous self-administered questionnaire and an appropriate validated mental health help-seeking assessment instrument. A multistage sampling technique will be employed to select states, healthcare facilities, departments or units, and eligible healthcare workers. Where feasible, selected healthcare worker groups or facilities will be assigned to intervention and comparison groups. Healthcare workers in the intervention group will receive structured workplace mental health education covering common mental health conditions, psychological distress, stress and burnout, early warning signs, misconceptions about mental illness, stigma, available mental health services, confidentiality, referral pathways, treatment options, and the importance of timely professional help-seeking. Mental health help-seeking will be assessed using indicators such as willingness to seek professional assistance when experiencing psychological distress, previous utilization of formal mental health services where applicable, intention to seek professional support in the future, knowledge of appropriate referral pathways, and willingness to recommend professional assistance to colleagues experiencing significant psychological difficulties. Descriptive statistics will be used to summarize healthcare workers' sociodemographic and occupational characteristics, mental health knowledge, exposure to education, and help-seeking behaviour. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the impact of workplace mental health education on mental health help-seeking. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that workplace mental health education has a significant positive impact on mental health help-seeking among healthcare workers in Nigeria. Healthcare workers exposed to structured workplace mental health education are expected to demonstrate improved understanding of mental health conditions and greater willingness to seek appropriate professional support. Education may increase recognition of psychological distress, reduce misconceptions and stigma, improve awareness of available mental health services, clarify confidentiality and referral procedures, and strengthen confidence in seeking professional assistance. It may also encourage healthcare workers to support colleagues in accessing appropriate mental health services. However, education alone may not eliminate barriers to help-seeking because healthcare workers may continue to experience concerns about stigma, confidentiality, professional reputation, time constraints, service availability, cost, and fear of negative workplace consequences. Consequently, workplace mental health education is expected to be most effective when supported by confidential and accessible mental health services, supportive workplace policies, trained professionals, employee assistance programmes, appropriate referral systems, and organizational efforts to reduce mental health stigma. The study is expected to contribute to the literature on workplace mental health, healthcare worker well-being, mental health help-seeking, occupational health, health education, healthcare workforce development, and public health in Nigeria by providing empirical evidence on the impact of workplace mental health education on mental health help-seeking among healthcare workers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare institutions, professional regulatory bodies, healthcare managers, occupational health professionals, mental health practitioners, public health practitioners, development partners, and policymakers regarding strategies for improving mental health support within the healthcare workforce. The study will also provide evidence-based recommendations for integrating workplace mental health education into healthcare worker training and continuing professional development, strengthening confidential mental health support systems, improving referral pathways, reducing workplace stigma, expanding employee assistance programmes, and promoting timely and appropriate mental health help-seeking among healthcare workers across Nigeria.
