Impact of Workplace Cardiometabolic Screening on Metabolic Syndrome Detection among Adults in Nigeria
Abstract
Metabolic syndrome is an important public health concern among adults in Nigeria and is characterized by a clustering of cardiometabolic risk factors, including abdominal obesity, elevated blood pressure, abnormal blood glucose levels, elevated triglycerides, and reduced high-density lipoprotein cholesterol. The presence of multiple risk factors increases the likelihood of developing cardiovascular disease, type 2 diabetes, and other chronic health conditions. Many adults with metabolic syndrome may remain unaware of their risk status because the individual components may be asymptomatic during their early stages. Workplace conditions and lifestyle patterns, including prolonged sitting, physical inactivity, occupational stress, unhealthy dietary practices, irregular working hours, and sedentary occupations, may further increase cardiometabolic risks among workers. Workplace cardiometabolic screening provides an opportunity to identify employees with metabolic risk factors through routine occupational health assessments, employee wellness programmes, health checks, and workplace screening initiatives. Early detection may facilitate lifestyle modification, medical evaluation, risk-factor management, and appropriate referral and follow-up. However, inadequate workplace health programmes, limited screening equipment, shortage of trained healthcare personnel, low employee participation, financial constraints, and weak referral and follow-up systems may limit the effectiveness of workplace cardiometabolic screening in Nigeria. Against this background, this study investigates the impact of workplace cardiometabolic screening on metabolic syndrome detection among adults in Nigeria. The study will be anchored on Occupational Health Theory, the Health Belief Model, and the Health Systems Framework. Occupational Health Theory emphasizes the prevention, identification, monitoring, and management of workplace-related health risks and the promotion of workers' health and well-being. The Health Belief Model explains how adults' perceptions of susceptibility to metabolic syndrome, perceived severity, perceived benefits of cardiometabolic screening, perceived barriers, and cues to action may influence participation in workplace screening. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective workplace cardiometabolic screening and prevention programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace cardiometabolic screening may influence metabolic syndrome detection among adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adult employees aged 18 years and above working in selected public and private organizations across Nigeria. A multistage sampling technique will be used to select states, organizations, institutions, workplaces, departments, and eligible employees. Workplace cardiometabolic screening will be measured using indicators such as availability of screening programmes, screening coverage, frequency of health assessments, measurement of blood pressure, assessment of waist circumference or other measures of central adiposity, blood glucose testing, lipid profile assessment, availability of trained healthcare personnel, screening equipment, cardiometabolic health education, lifestyle-risk assessment, and referral arrangements. Metabolic syndrome detection will be assessed using indicators such as the presence of multiple cardiometabolic risk factors according to an appropriate recognized diagnostic criterion, abdominal obesity, elevated blood pressure, elevated fasting blood glucose or other appropriate glucose measures, elevated triglycerides, reduced high-density lipoprotein cholesterol, newly identified metabolic syndrome, severity or number of risk components, referrals for further clinical assessment, and linkage to appropriate healthcare and lifestyle-management services. Data will be collected using structured questionnaires, workplace screening registers, anthropometric assessment forms, blood pressure records, laboratory reports, occupational health records, referral registers, and relevant organizational documents. Descriptive statistics will be used to summarize participants' characteristics, occupational factors, screening coverage, cardiometabolic findings, and patterns of metabolic syndrome detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of workplace cardiometabolic screening on metabolic syndrome detection. Where appropriate, metabolic syndrome detection rates before and after implementation of workplace cardiometabolic screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that workplace cardiometabolic screening has a significant positive impact on metabolic syndrome detection among adults in Nigeria. Adults who participate in workplace cardiometabolic screening are expected to have a higher likelihood of having previously undetected clusters of cardiometabolic risk factors identified than employees without access to routine screening. Regular assessment of blood pressure, central adiposity, glucose levels, and lipid-related risk factors may facilitate early identification of individuals at increased risk of cardiovascular disease and type 2 diabetes. Early detection may provide opportunities for lifestyle counselling, dietary modification, increased physical activity, weight management, medical evaluation, appropriate treatment, and continued monitoring. Conducting cardiometabolic screening within workplaces may reduce time-related, financial, and accessibility barriers and may reach employees who do not routinely seek preventive healthcare. Workplace health education may also improve awareness of healthy eating, physical activity, weight management, blood pressure control, and other strategies for reducing cardiometabolic risk. Conversely, inadequate screening coverage, limited laboratory capacity, shortage of trained healthcare personnel, low employee participation, occupational time constraints, financial barriers, and weak referral and follow-up systems may reduce the effectiveness of workplace cardiometabolic screening. The study therefore expects accessible and well-organized workplace cardiometabolic screening to contribute significantly to improved detection and early management of metabolic syndrome among adults in Nigeria. The study is expected to contribute to the literature on workplace cardiometabolic screening, metabolic syndrome detection, occupational health, cardiovascular disease prevention, diabetes prevention, obesity, workplace health promotion, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, employers, occupational health practitioners, healthcare providers, cardiologists, endocrinologists, nutritionists, workplace safety officers, human resource managers, development partners, and policymakers regarding strategies for improving cardiometabolic health among workers. The study will also provide evidence-based recommendations for expanding workplace cardiometabolic screening programmes, improving access to appropriate laboratory and anthropometric assessments, strengthening employee health education, promoting healthy dietary practices and physical activity, improving workplace wellness programmes, strengthening referral and follow-up systems, increasing access to trained healthcare personnel, and integrating cardiometabolic risk assessment into comprehensive occupational health programmes across Nigeria.
