Impact of Workplace Blood Pressure Monitoring on Hypertension Detection among Commercial Drivers in Nigeria
Abstract
Hypertension remains a major public health concern in Nigeria and is an important risk factor for cardiovascular disease, stroke, kidney disease, and premature mortality. Commercial drivers may face occupational and lifestyle conditions that increase their risk of elevated blood pressure, including prolonged sitting, occupational stress, irregular working hours, inadequate physical activity, unhealthy dietary practices, sleep disturbances, and limited opportunities for routine healthcare. Because hypertension may remain asymptomatic for long periods, many commercial drivers may be unaware that they have elevated blood pressure and may therefore remain undiagnosed until complications occur. Workplace blood pressure monitoring provides an opportunity to identify elevated blood pressure among commercial drivers through routine occupational health assessments, periodic blood pressure checks at transport workplaces, driver medical examinations, mobile health services, and workplace wellness programmes. Early detection may facilitate confirmatory assessment, lifestyle modification, medical evaluation, treatment, and appropriate follow-up, thereby reducing the risk of hypertension-related complications. However, limited occupational health programmes, irregular monitoring, inadequate awareness, shortage of trained healthcare personnel, limited access to blood pressure monitoring equipment, and weak referral and follow-up systems may restrict the effectiveness of workplace blood pressure monitoring among commercial drivers in Nigeria. Against this background, this study investigates the impact of workplace blood pressure monitoring on hypertension detection among commercial drivers in Nigeria. The study will be anchored on Occupational Health Theory, the Health Belief Model, and the Health Systems Framework. Occupational Health Theory emphasizes the prevention, identification, monitoring, and management of occupational health risks and the promotion of workers' health and safety. The Health Belief Model explains how commercial drivers' perceptions of susceptibility to hypertension, perceived severity, perceived benefits of blood pressure monitoring, perceived barriers, and cues to action may influence participation in workplace health assessments. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective workplace hypertension screening and management programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace blood pressure monitoring may influence hypertension detection among commercial drivers in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise commercial drivers operating in selected transport companies, motor parks, bus terminals, taxi associations, and other organized transport settings across Nigeria. A multistage sampling technique will be used to select states, transport organizations, motor parks, terminals, driver associations, and eligible commercial drivers. Workplace blood pressure monitoring will be measured using indicators such as availability of blood pressure monitoring programmes, monitoring coverage, frequency of blood pressure assessments, number of monitoring sessions attended, availability of validated blood pressure measuring devices, trained healthcare personnel, occupational health education, risk-factor assessment, and referral arrangements. Hypertension detection will be assessed using indicators such as elevated blood pressure measurements, repeated elevated readings where appropriate, newly identified hypertension, previous awareness of hypertension status, referrals for clinical confirmation, confirmed hypertension following appropriate medical assessment, and linkage to treatment and follow-up services. Data will be collected using structured questionnaires, workplace health records, blood pressure monitoring registers, medical examination forms, referral records, and relevant transport organization documents. Descriptive statistics will be used to summarize drivers' characteristics, occupational factors, monitoring coverage, blood pressure findings, and patterns of hypertension detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of workplace blood pressure monitoring on hypertension detection. Where appropriate, hypertension detection rates before and after implementation of workplace blood pressure monitoring programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that workplace blood pressure monitoring has a significant positive impact on hypertension detection among commercial drivers in Nigeria. Commercial drivers who participate in regular workplace blood pressure monitoring are expected to have a higher likelihood of having previously undetected elevated blood pressure identified than drivers without access to routine monitoring. Regular monitoring may facilitate early recognition of elevated blood pressure and provide opportunities for repeated assessment, clinical confirmation, lifestyle counselling, medical treatment where indicated, and continued follow-up. Providing blood pressure monitoring within transport workplaces may reduce time-related, financial, and geographical barriers that discourage commercial drivers from accessing routine healthcare. Workplace health education may also improve drivers' awareness of hypertension risk factors, healthy dietary practices, physical activity, stress management, medication adherence, and the importance of regular blood pressure assessment. Early detection may reduce the risk of untreated hypertension and associated cardiovascular complications. Conversely, irregular monitoring, inadequate screening coverage, limited equipment, shortage of trained healthcare personnel, low driver participation, occupational time constraints, and weak referral and follow-up systems may reduce the effectiveness of workplace blood pressure monitoring. The study therefore expects accessible and well-organized workplace blood pressure monitoring to contribute significantly to improved detection and early management of hypertension among commercial drivers in Nigeria. The study is expected to contribute to the literature on workplace blood pressure monitoring, hypertension detection, commercial drivers' health, occupational health, cardiovascular disease prevention, workplace health promotion, preventive healthcare, transport-worker health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, transport unions and associations, transport companies, motor park authorities, occupational health practitioners, primary healthcare providers, cardiologists, public health professionals, development partners, and policymakers regarding strategies for improving the health of commercial drivers. The study will also provide evidence-based recommendations for expanding workplace blood pressure monitoring programmes, increasing access to validated blood pressure monitoring equipment, strengthening occupational health services for commercial drivers, improving hypertension awareness, promoting healthy lifestyle practices, strengthening referral and follow-up systems, encouraging regular medical assessments, and integrating cardiovascular risk screening into comprehensive occupational health programmes for commercial drivers across Nigeria.
