Impact of Psychological First-Aid Training on Emergency Psychological Support Skills among Paramedics in Nigeria

Abstract

Emergency situations such as road traffic crashes, disasters, mass-casualty incidents, violence, sudden deaths, and other traumatic events may expose affected individuals to acute psychological distress. Paramedics are often among the first healthcare professionals to interact with people experiencing such distress and therefore have an important role in providing immediate psychological support alongside emergency medical care. However, limited training in psychological first aid, inadequate knowledge of appropriate supportive communication, uncertainty about managing acute emotional reactions, and limited awareness of referral pathways may affect paramedics' ability to provide effective psychological support. Psychological first-aid training provides an opportunity to equip paramedics with practical skills for recognizing acute psychological distress, communicating calmly and empathetically, providing immediate emotional support, promoting safety, and connecting affected individuals with appropriate mental health and social support services. Against this background, this study investigates the impact of psychological first-aid training on emergency psychological support skills among paramedics in Nigeria. The study will be anchored on Social Cognitive Theory, the Transactional Model of Stress and Coping, and the Health Belief Model. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, behavioural skills, observational learning, and supportive professional environments in developing effective psychological support skills. The Transactional Model of Stress and Coping explains how individuals respond to traumatic events and how appropriate coping resources and supportive interventions may assist in managing acute psychological distress. The Health Belief Model explains how paramedics' perceptions of psychological distress, perceived benefits of psychological first-aid training, perceived barriers, and cues to action may influence their willingness and ability to provide appropriate psychological support. Collectively, these theoretical perspectives provide a suitable framework for explaining how psychological first-aid training may influence emergency psychological support skills among paramedics in Nigeria. The study will adopt a quantitative quasi-experimental research design. The study population will comprise paramedics working with selected emergency medical services, hospitals, ambulance services, and emergency response organizations across Nigeria. A multistage sampling technique will be used to select states, emergency medical service organizations, healthcare facilities, ambulance units, and eligible paramedics. Psychological first-aid training will be measured using indicators such as exposure to structured training sessions, training frequency and duration, knowledge of psychological first aid principles, recognition of acute psychological distress, active listening, empathetic communication, emotional stabilization, promotion of safety, appropriate reassurance, practical assistance, identification of immediate needs, confidentiality, professional boundaries, referral procedures, and follow-up awareness. Emergency psychological support skills will be assessed using indicators such as recognition of psychological distress, active listening, empathetic communication, emotional validation, calm communication during emergencies, provision of immediate emotional support, promotion of safety, appropriate information provision, management of acute emotional reactions, maintenance of professional boundaries, referral to mental health services, and appropriate documentation. Data will be collected using structured questionnaires, case-based scenarios, skills assessments, observation checklists, and validated psychological first-aid competency instruments administered before and after the training intervention. Descriptive statistics will be used to summarize paramedics' demographic and professional characteristics, previous psychological support training, and emergency psychological support skill levels. Inferential statistical techniques, including paired sample tests, independent sample tests, chi-square tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the impact of psychological first-aid training on emergency psychological support skills. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that psychological first-aid training has a significant positive impact on emergency psychological support skills among paramedics in Nigeria. Paramedics exposed to structured psychological first-aid training are expected to demonstrate improved recognition of acute psychological distress, active listening, empathetic communication, emotional support, safety promotion, appropriate information provision, and referral skills compared with their baseline skills or paramedics without similar training. Practical training approaches involving simulated emergency scenarios, role-play, case discussions, communication exercises, and supervised skills demonstrations may strengthen paramedics' confidence and ability to provide appropriate psychological support while delivering emergency medical care. Improved psychological support skills may facilitate early assistance for individuals experiencing acute distress and strengthen referral to qualified mental health professionals when specialized care is required. However, emergency workload, time constraints, repeated exposure to traumatic incidents, staff shortages, limited access to mental health professionals, inadequate training resources, and organizational pressures may affect the application and sustainability of these skills. The study therefore expects accessible, evidence-based, contextually appropriate, and regularly reinforced psychological first-aid training to contribute significantly to improved emergency psychological support skills among paramedics in Nigeria. The study is expected to contribute to the literature on psychological first-aid training, emergency psychological support skills, paramedics, emergency mental health, psychological first aid, trauma support, emergency medical services, crisis intervention, health workforce development, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Emergency Medical Services and Ambulance System, emergency medical service providers, hospitals, ambulance services, emergency response organizations, paramedic training institutions, psychologists, psychiatrists, counsellors, public health practitioners, development partners, and policymakers regarding strategies for strengthening psychological support within emergency medical services. The study will also provide evidence-based recommendations for integrating psychological first aid into paramedic education and continuing professional development, establishing standardized psychological support protocols for emergency services, strengthening referral pathways, improving access to mental health professionals, providing regular simulation-based training, and developing sustainable psychological support systems for emergency responders and trauma-affected populations across Nigeria.
