Impact of Lassa Fever Education on Lassa Fever Prevention Practices among Residents of Endemic Communities in Nigeria

Abstract

Lassa fever is an important viral haemorrhagic disease and public health concern in Nigeria, particularly in communities where environmental and socioeconomic conditions may increase exposure to the virus and its rodent reservoirs. Prevention and control depend substantially on community awareness and the adoption of appropriate behaviours that reduce contact with infected rodents, contaminated materials, and other potential sources of transmission. Lassa fever education provides residents with information about the causes and transmission of Lassa fever, common signs and symptoms, rodent control, proper food storage, environmental sanitation, safe household practices, avoidance of contact with rodent urine and faeces, appropriate handling of suspected cases, early healthcare-seeking, and the importance of reporting suspected cases to appropriate health authorities. Lassa fever prevention practices refer to the actions undertaken by community residents to reduce their risk of exposure to and transmission of Lassa fever. In Nigeria, inadequate prevention practices may be associated with limited knowledge, misconceptions about the disease, poor environmental sanitation, inadequate food storage, rodent infestation, unsafe household practices, delayed healthcare-seeking, and limited access to reliable health information. Effective Lassa fever education may improve residents' knowledge, risk perception, confidence, and adherence to recommended preventive practices. Against this background, this study investigates the impact of Lassa fever education on Lassa fever prevention practices among residents of endemic communities in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding residents' Lassa fever prevention decisions through perceived susceptibility, perceived severity, perceived benefits of preventive measures, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, and environmental conditions in shaping preventive health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how Lassa fever education may improve residents' knowledge and attitudes and influence prevention practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how Lassa fever education may influence prevention practices among residents of endemic communities in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from residents of selected Lassa fever-endemic communities in Nigeria using a structured interviewer-administered questionnaire, Lassa fever knowledge assessment, and prevention-practice checklist. A multistage sampling technique will be employed to select states, local government areas, endemic communities, households, and eligible adult residents. Where feasible, selected communities or participant groups will be assigned to intervention and comparison groups. Residents in the intervention group will receive structured Lassa fever education covering transmission, signs and symptoms, rodent control, environmental sanitation, proper food storage, safe household practices, prevention of contact with rodent urine and faeces, appropriate handling of suspected cases, early healthcare-seeking, and reporting of suspected cases. Lassa fever prevention practices will be assessed using indicators such as safe food storage, household sanitation, rodent-control measures, safe disposal of household waste, prevention of rodent access to food and living spaces, avoidance of direct contact with potentially contaminated materials, appropriate healthcare-seeking when symptoms occur, and awareness of appropriate procedures for suspected cases. Where feasible, self-reported practices will be complemented by structured household observation and environmental assessment. Descriptive statistics will be used to summarize participants' sociodemographic characteristics, previous exposure to Lassa fever information, knowledge, environmental conditions, and prevention practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the impact of Lassa fever education on prevention practices. Where appropriate, prevention practices before and after the intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that Lassa fever education has a significant positive impact on Lassa fever prevention practices among residents of endemic communities in Nigeria. Residents exposed to structured Lassa fever education are expected to demonstrate improved knowledge of transmission routes, risk factors, symptoms, rodent control, environmental sanitation, food protection, and appropriate healthcare-seeking. Education may encourage improved household sanitation, safer food storage, better rodent-control practices, reduced contact with potentially contaminated materials, and earlier utilization of appropriate healthcare services when symptoms develop. Community-based education and practical demonstrations are expected to improve residents' ability to translate knowledge into routine preventive practices. However, education alone may not eliminate Lassa fever risk because residents may continue to face challenges such as poor environmental sanitation, inadequate housing, rodent infestation, limited access to safe waste disposal, poverty, inadequate healthcare services, and other environmental and socioeconomic conditions. Consequently, Lassa fever education is expected to be most effective when supported by environmental sanitation programmes, rodent-control measures, accessible healthcare services, effective disease surveillance, community engagement, and timely public health responses. The study is expected to contribute to the literature on Lassa fever prevention, viral haemorrhagic diseases, community health education, zoonotic disease prevention, environmental health, infectious disease control, epidemiology, and public health in Nigeria by providing empirical evidence on the impact of Lassa fever education on prevention practices among residents of endemic communities. The findings will provide useful information to the Nigeria Centre for Disease Control and Prevention, Federal Ministry of Health and Social Welfare, state ministries of health, local government health authorities, primary healthcare centres, community health workers, public health departments, development partners, and policymakers regarding strategies for strengthening community-level Lassa fever prevention. The study will also provide evidence-based recommendations for strengthening community-based Lassa fever education, improving environmental sanitation, promoting safe food storage, enhancing rodent-control programmes, strengthening early healthcare-seeking and referral systems, improving community surveillance and reporting, and promoting coordinated public health interventions in endemic communities across Nigeria.
