Impact of Healthcare Waste Management Training on Waste Segregation Practices among Hospital Workers in Nigeria
Abstract
Healthcare waste management is an important public health and environmental concern in Nigeria because healthcare facilities generate different categories of waste that may pose risks to healthcare workers, patients, waste handlers, surrounding communities, and the environment when improperly managed. Inadequate waste segregation may result in the mixing of infectious, sharps, pharmaceutical, chemical, and general waste, increasing the risk of accidental injuries, infection transmission, environmental contamination, and inappropriate disposal. Hospital workers play a critical role in waste segregation because proper separation of healthcare waste begins at the point of generation. Healthcare waste management training provides an opportunity to improve workers' knowledge and practical skills concerning waste classification, colour-coded containers, sharps disposal, handling procedures, personal protective equipment, and appropriate waste-management practices. Against this background, this study investigates the impact of healthcare waste management training on waste segregation practices among hospital workers in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and the Theory of Planned Behavior. The Health Belief Model explains how hospital workers' perceptions of the risks and consequences of improper healthcare waste handling, perceived benefits of appropriate segregation, perceived barriers, and cues to action may influence waste-management practices. Social Cognitive Theory emphasizes knowledge, self-efficacy, observational learning, behavioural skills, and workplace environmental influences in the development of appropriate waste-segregation practices. The Theory of Planned Behavior explains how workers' attitudes, subjective norms, perceived behavioural control, and behavioural intentions may influence their compliance with recommended healthcare waste-management procedures. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare waste management training may influence waste segregation practices among hospital workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. The study population will comprise hospital workers, including doctors, nurses, laboratory personnel, pharmacists, cleaners, waste handlers, environmental health personnel, and other workers involved in healthcare waste generation or handling in selected public and private hospitals across Nigeria. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, units, and eligible workers. Healthcare waste management training will be measured using indicators such as exposure to structured training sessions, training frequency and duration, waste classification education, colour-coded segregation, sharps management, infectious waste handling, pharmaceutical waste management, use of personal protective equipment, waste storage procedures, transportation procedures, spill management, occupational safety, and follow-up training. Waste segregation practices will be assessed using indicators such as correct separation of general and infectious waste, appropriate segregation of sharps, correct use of colour-coded waste containers, segregation at the point of generation, proper disposal of pharmaceutical and chemical waste, correct handling of contaminated materials, use of appropriate waste bags and containers, compliance with facility waste-management guidelines, and observed segregation accuracy. Data will be collected using structured questionnaires, observational checklists, waste-management records, training attendance records, and hospital environmental health documentation. Waste segregation practices will be assessed before and after participation in the training intervention. Descriptive statistics will be used to summarize workers' demographic and occupational characteristics, training exposure, knowledge levels, and waste segregation practices. Inferential statistical techniques, including paired sample tests, independent sample tests, chi-square tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of healthcare waste management training on waste segregation practices. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that healthcare waste management training has a significant positive impact on waste segregation practices among hospital workers in Nigeria. Hospital workers exposed to structured and practical waste-management training are expected to demonstrate improved compliance with recommended segregation procedures, including appropriate separation of infectious waste, sharps, pharmaceutical waste, and general waste. Training on colour-coded containers, waste classification, point-of-generation segregation, personal protective equipment, and occupational safety may improve workers' knowledge, confidence, and ability to implement appropriate waste-management practices. Practical demonstrations and periodic refresher training may further strengthen compliance and reduce inappropriate mixing of different categories of healthcare waste. However, inadequate waste-management infrastructure, insufficient colour-coded containers, irregular supply of waste bags, poor supervision, heavy workloads, inadequate staffing, weak enforcement of facility guidelines, and limited management support may reduce the effectiveness of training. The study therefore expects regular, practical, evidence-based, and adequately supported healthcare waste management training to contribute significantly to improved waste segregation practices among hospital workers in Nigeria. The study is expected to contribute to the literature on healthcare waste management training, waste segregation practices, hospital workers, environmental health, occupational safety, infection prevention and control, healthcare waste management, hospital hygiene, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare facilities, environmental health authorities, infection prevention and control committees, healthcare workers, waste-management personnel, development partners, and policymakers regarding strategies for strengthening healthcare waste management. The study will also provide evidence-based recommendations for integrating healthcare waste management training into hospital staff development programmes, conducting regular refresher training, improving the availability of colour-coded waste containers and appropriate disposal equipment, strengthening supervision and monitoring, enforcing healthcare waste-management policies, and developing sustainable waste segregation systems across Nigerian hospitals.
