Impact of Healthcare Waste Management Education on Waste Disposal Practices among Healthcare Workers in Nigeria
Abstract
Healthcare waste management is an important component of infection prevention, occupational safety, environmental protection, and quality healthcare delivery. Healthcare facilities generate different categories of waste, including infectious waste, sharps, pathological waste, pharmaceutical waste, chemical waste, and general non-hazardous waste. Improper handling, segregation, transportation, treatment, and disposal of healthcare waste may expose healthcare workers, patients, waste handlers, surrounding communities, and the environment to biological, chemical, and physical hazards. Healthcare waste management education provides healthcare workers with relevant information and practical skills relating to waste classification, segregation at source, appropriate use of colour-coded waste containers, sharps management, safe handling and transportation, temporary storage, treatment, final disposal, personal protective equipment, spill management, and occupational exposure prevention. Waste disposal practices refer to the procedures followed by healthcare workers when segregating, handling, transporting, storing, and disposing of healthcare waste according to established safety and environmental standards. In Nigeria, inappropriate healthcare waste disposal practices may be associated with inadequate training, limited knowledge of waste classification, insufficient waste-management facilities, shortages of appropriate containers and personal protective equipment, poor supervision, and inconsistent implementation of healthcare waste management policies. Effective healthcare waste management education may improve healthcare workers' knowledge, skills, and adherence to appropriate waste disposal procedures. Against this background, this study investigates the impact of healthcare waste management education on waste disposal practices among healthcare workers in Nigeria. The study is anchored on Social Cognitive Theory, Experiential Learning Theory, and the Knowledge-Attitude-Practice framework. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, environmental conditions, social influences, and behaviour in shaping healthcare workers' waste management practices. Experiential Learning Theory emphasizes learning through practical demonstrations, experience, reflection, and application, making it relevant to healthcare waste management training. The Knowledge-Attitude-Practice framework provides a basis for examining how healthcare waste management education may improve workers' knowledge and attitudes and influence appropriate waste disposal practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare waste management education may influence waste disposal practices among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from healthcare workers in selected public and private hospitals, primary healthcare centres, clinics, and other healthcare facilities in Nigeria using a structured self-administered questionnaire, healthcare waste management knowledge assessment, and waste disposal practice checklist. A multistage sampling technique will be employed to select states, local government areas, healthcare facilities, departments, and eligible healthcare workers. Where feasible, selected healthcare facilities or healthcare worker groups will be assigned to intervention and comparison groups. Healthcare workers in the intervention group will receive structured healthcare waste management education covering waste classification, segregation at source, appropriate use of colour-coded waste containers, sharps management, safe handling and transportation, temporary storage, treatment and disposal procedures, use of personal protective equipment, spill management, occupational exposure prevention, and environmental protection. Waste disposal practices will be assessed using indicators such as waste segregation at source, correct disposal of sharps, use of appropriate waste containers, proper labelling, safe handling and transportation, appropriate use of personal protective equipment, temporary storage practices, and adherence to established healthcare waste management procedures. Where feasible, self-reported practices will be complemented by direct observation using a standardized practice checklist and facility assessment. Descriptive statistics will be used to summarize healthcare workers' sociodemographic and professional characteristics, previous waste-management training, knowledge, and disposal practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the impact of healthcare waste management education on waste disposal practices. Where appropriate, waste disposal practices before and after the intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that healthcare waste management education has a significant positive impact on waste disposal practices among healthcare workers in Nigeria. Healthcare workers exposed to structured waste management education are expected to demonstrate improved knowledge of waste classification, segregation, sharps disposal, handling procedures, personal protective equipment, and safe treatment and disposal methods. Education may increase compliance with waste segregation at source, correct use of designated waste containers, safe sharps disposal, appropriate transportation and storage, and other established waste management procedures. Practical demonstrations and repeated training are expected to improve workers' ability to apply waste management principles in routine clinical settings. However, education alone may not guarantee appropriate waste disposal practices because healthcare workers may continue to face challenges such as inadequate waste containers, irregular collection, insufficient personal protective equipment, poor waste-treatment facilities, inadequate staffing, limited supervision, and weak implementation of institutional waste management policies. Consequently, healthcare waste management education is expected to be most effective when supported by adequate infrastructure, reliable supplies and equipment, effective supervision, clear institutional policies, regular monitoring, and functional healthcare waste treatment and disposal systems. The study is expected to contribute to the literature on healthcare waste management, occupational safety, infection prevention, environmental health, healthcare management, health education, and public health in Nigeria by providing empirical evidence on the impact of healthcare waste management education on waste disposal practices among healthcare workers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospitals, primary healthcare agencies, healthcare facility administrators, infection prevention and control committees, environmental health authorities, nurses, doctors, laboratory personnel, waste handlers, development partners, and policymakers regarding strategies for improving healthcare waste management. The study will also provide evidence-based recommendations for integrating healthcare waste management education into staff induction and continuing professional development, strengthening practical waste segregation training, improving availability of appropriate waste containers and personal protective equipment, establishing effective monitoring and supervision systems, strengthening healthcare waste management policies, and improving treatment and final disposal systems in Nigerian healthcare facilities.
