Impact of Flood Preparedness Education on Disease Prevention Practices among Residents of Flood-Prone Communities in Nigeria

Abstract

Flooding is an important environmental and public health concern in Nigeria, particularly in communities located in flood-prone areas where heavy rainfall, overflowing rivers, inadequate drainage systems, urban development, and other environmental factors may increase the risk of flooding. Flood events can disrupt water and sanitation systems, contaminate drinking-water sources, increase exposure to disease vectors, displace households, and create conditions that facilitate the spread of waterborne, foodborne, vector-borne, and other communicable diseases. Flood preparedness education provides residents with information about flood risks, early warning signs, household preparedness, safe water and food practices, sanitation, hygiene, waste management, disease prevention, evacuation procedures, and appropriate healthcare seeking before, during, and after flooding. Disease prevention practices refer to behaviours adopted by residents to reduce the risk of illness associated with flooding, including safe drinking-water practices, appropriate water treatment, food hygiene, hand hygiene, sanitation, environmental cleanliness, vector-control measures, proper waste disposal, and timely healthcare seeking when symptoms occur. In Nigeria, inadequate preparedness, limited access to reliable information, poor drainage and sanitation infrastructure, contaminated water sources, overcrowding, and limited access to healthcare services may increase the vulnerability of flood-prone communities to disease outbreaks. Effective flood preparedness education may improve residents' knowledge and encourage the adoption of appropriate disease prevention practices. Against this background, this study investigates the impact of flood preparedness education on disease prevention practices among residents of flood-prone communities in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding residents' decisions to adopt disease prevention practices during flood events. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping preparedness and health behaviours. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from flood preparedness education may influence disease prevention practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how flood preparedness education may influence disease prevention practices among residents of flood-prone communities in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from adult residents of selected flood-prone communities in Nigeria using a structured interviewer-administered questionnaire and an observational environmental health checklist. A multistage sampling technique will be employed to select states, local government areas, flood-prone communities, households, and eligible residents. Where feasible, selected communities or participant groups will be assigned to intervention and comparison groups. Residents in the intervention group will receive structured flood preparedness education covering flood risks, early warning systems, household preparedness, safe evacuation, safe water storage and treatment, food hygiene, hand hygiene, sanitation, waste disposal, vector control, environmental cleanliness, prevention of waterborne and vector-borne diseases, and timely healthcare seeking. Disease prevention practices will be assessed using indicators such as household water treatment, safe water storage, food hygiene, hand hygiene, sanitation practices, environmental cleanliness, vector-control practices, appropriate waste disposal, and timely healthcare seeking during and after flood events. Descriptive statistics will be used to summarize residents' sociodemographic characteristics, flood preparedness knowledge, exposure to education, and disease prevention practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the impact of flood preparedness education on disease prevention practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that flood preparedness education has a significant positive impact on disease prevention practices among residents of flood-prone communities in Nigeria. Residents exposed to structured flood preparedness education are expected to demonstrate improved knowledge of flood-related health risks and greater adoption of appropriate disease prevention behaviours. Education may encourage safer drinking-water practices, household water treatment and storage, improved food hygiene, hand hygiene, sanitation, waste disposal, environmental cleanliness, vector-control practices, and timely healthcare seeking. However, education alone may not guarantee sustained preventive practices because residents may continue to face structural barriers such as inadequate drainage, contaminated water sources, limited access to clean water, poor sanitation infrastructure, overcrowding, displacement, inadequate healthcare facilities, and limited access to flood warning information. Consequently, flood preparedness education is expected to be most effective when supported by effective flood early-warning systems, adequate water and sanitation infrastructure, accessible healthcare services, proper drainage, emergency response systems, and sustained community engagement. The study is expected to contribute to the literature on flood preparedness, environmental health, disaster risk reduction, disease prevention, community health education, climate-related health risks, and public health in Nigeria by providing empirical evidence on the impact of flood preparedness education on disease prevention practices among residents of flood-prone communities. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Emergency Management Agency (NEMA), National Primary Health Care Development Agency (NPHCDA), Federal Ministry of Environment, state emergency management agencies, local government authorities, healthcare providers, environmental health officers, community health workers, development partners, and policymakers regarding strategies for reducing flood-related health risks. The study will also provide evidence-based recommendations for strengthening community flood preparedness education, improving flood early-warning communication, promoting safe water and sanitation practices, strengthening disease surveillance, improving emergency healthcare preparedness, and enhancing community resilience to flood-related disease risks across Nigeria.
