Impact of Eye Health Education on Eye Care Practices among University Students in Nigeria
Abstract
Organ transplantation is an important component of modern healthcare and can provide life-saving treatment for individuals with end-stage organ failure and other serious medical conditions. The availability of organs for transplantation depends substantially on public awareness, willingness to donate, effective donor registration systems, and appropriate legal and ethical frameworks. University students represent an important population for organ donation education because they are generally within an age group capable of developing informed attitudes toward health-related decisions and may influence awareness within their families and communities. Organ donation awareness provides students with information about the meaning and importance of organ donation, organs and tissues that may be donated, eligibility considerations, the donation and transplantation process, ethical and legal principles, confidentiality, consent, religious and cultural considerations, and procedures for donor registration. Willingness to register as an organ donor refers to an individual's intention or readiness to formally indicate their consent to donate eligible organs or tissues after death, where applicable, through an approved donor registration process. In Nigeria, limited public awareness, misconceptions about organ donation and transplantation, concerns about premature declaration of death, religious and cultural beliefs, fear of organ misuse, mistrust of healthcare institutions, and inadequate information about donor registration may influence willingness to become an organ donor. Effective organ donation awareness may improve knowledge, correct misconceptions, increase confidence in the transplantation system, and encourage informed willingness to register as an organ donor. Against this background, this study investigates the effect of organ donation awareness on willingness to register as organ donors among university students in Nigeria. The study is anchored on the Health Belief Model, Theory of Planned Behaviour, and Knowledge-Attitude-Practice framework. The Health Belief Model explains health-related decision-making through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding students' willingness to consider organ donation. The Theory of Planned Behaviour explains behavioural intention through attitudes, subjective norms, and perceived behavioural control, providing a framework for understanding students' willingness to register as organ donors. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge resulting from organ donation awareness may influence attitudes and willingness to engage in donor registration. Collectively, these theoretical perspectives provide a suitable framework for explaining how organ donation awareness may influence willingness to register as an organ donor among university students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from undergraduate students in selected universities in Nigeria using a structured anonymous self-administered questionnaire. A multistage sampling technique will be employed to select states, universities, faculties or departments, academic levels, and eligible students. Where feasible, selected student groups will be assigned to intervention and comparison groups. Students in the intervention group will receive structured organ donation awareness education covering the importance of organ donation, the transplantation process, organs and tissues that may be donated, donor eligibility, consent, ethical and legal considerations, confidentiality, cultural and religious considerations, common misconceptions, family communication, and appropriate procedures for donor registration. Willingness to register as an organ donor will be assessed using indicators such as intention to register, readiness to complete donor registration, willingness to discuss donation decisions with family members, confidence in the donation system, and willingness to provide consent through an approved donor registration process. Where an approved registration mechanism is available within the study setting, actual registration may also be assessed with participants' consent. Descriptive statistics will be used to summarize students' sociodemographic characteristics, organ donation knowledge, exposure to awareness education, attitudes, and willingness to register. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the effect of organ donation awareness on willingness to register as an organ donor. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that organ donation awareness has a significant positive effect on willingness to register as an organ donor among university students in Nigeria. Students exposed to structured organ donation awareness education are expected to demonstrate improved knowledge and greater willingness to register as organ donors. Education may correct misconceptions, improve understanding of organ transplantation, increase awareness of consent and registration procedures, reduce unfounded fears, and encourage students to discuss their donation decisions with family members. However, awareness alone may not guarantee willingness to register because students may continue to face barriers such as cultural and religious concerns, mistrust of healthcare institutions, fear of organ misuse, uncertainty about the determination of death, concerns about family reactions, and limited awareness of formal donor registration procedures. Consequently, organ donation awareness is expected to be most effective when supported by transparent donor registration systems, clear ethical and legal guidelines, trusted healthcare institutions, appropriate public communication, family engagement, and accessible information about organ transplantation. The study is expected to contribute to the literature on organ donation, transplantation, health education, health promotion, university student health, health communication, and public health in Nigeria by providing empirical evidence on the effect of organ donation awareness on willingness to register as an organ donor among university students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Health Act regulatory authorities, relevant transplant and organ donation programmes, National Universities Commission, university administrations, healthcare institutions, transplant professionals, public health practitioners, faith and community leaders, development partners, and policymakers regarding strategies for improving public understanding and acceptance of organ donation. The study will also provide evidence-based recommendations for strengthening organ donation education, improving transparency and public trust, clarifying donor registration procedures, addressing cultural and religious concerns, encouraging informed family discussions, and promoting ethical and voluntary organ donor registration among university students across Nigeria.
