Impact of Electronic Patient Registration on Registration Errors in Nigerian Hospitals

 

Abstract

Accurate patient registration is an essential component of effective healthcare delivery because registration information forms the foundation for patient identification, medical records, billing, clinical documentation, communication, and continuity of care. Errors during registration may include incorrect patient names, inaccurate demographic information, duplicate patient records, incorrect contact details, wrong identification numbers, incomplete registration fields, and mismatches between patients and their records. Such errors may contribute to delays in care, duplication of medical records, incorrect billing, difficulties retrieving patient information, and potential patient-safety incidents. In many Nigerian hospitals, reliance on manual or partially computerized registration processes may increase the risk of documentation errors because of illegible handwriting, repetitive data entry, overcrowded registration points, inadequate verification procedures, and limited access to reliable information technology. Electronic patient registration provides a digital mechanism for capturing, verifying, storing, and retrieving patient information and may reduce registration errors by improving data standardization and automated validation. Against this background, this study investigates the impact of electronic patient registration on registration errors in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and the Technology Acceptance Model. The Donabedian Model explains healthcare quality through structure, process, and outcome, with electronic patient registration representing an important technological and process component and registration errors representing a measurable outcome. Systems Theory views patient registration as part of an interconnected hospital information system involving patients, registration personnel, medical records departments, healthcare providers, information technology infrastructure, and administrative processes. The Technology Acceptance Model explains how perceived usefulness and perceived ease of use may influence healthcare workers' acceptance and effective utilization of electronic registration systems. Collectively, these theoretical perspectives provide a suitable framework for explaining how electronic patient registration may influence registration errors in Nigerian hospitals. The study will adopt a quantitative quasi-experimental or analytical comparative research design. The study population will comprise selected public and private hospitals in Nigeria, registration personnel, health information management professionals, other relevant healthcare workers, and patient registration records. A multistage sampling technique will be used to select states, local government areas, hospitals, registration units, healthcare workers, and patient records. Electronic patient registration will be assessed using indicators such as availability of electronic registration systems, digital patient-data capture, unique patient identification numbers, mandatory data fields, automated validation, duplicate-record detection, electronic demographic verification, integration with hospital information systems, staff training, system accessibility, data-security measures, system reliability, and technical support. Registration errors will be assessed using indicators such as incorrect patient names, inaccurate demographic information, incorrect contact details, duplicate patient records, incorrect patient identification numbers, incomplete registration information, incorrect insurance or billing information, mismatched records, data-entry errors, and difficulties retrieving patient records. Data will be collected using structured questionnaires, patient registration records, electronic registration databases, medical records, health information department reports, incident reports, and system-generated error logs where available. Descriptive statistics will be used to summarize hospital characteristics, registration procedures, electronic-system utilization, and patterns of registration errors. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of electronic patient registration on registration errors. Where a quasi-experimental design is adopted, registration-error rates before and after implementation of electronic patient registration may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective electronic patient registration systems have a significant negative impact on registration errors in Nigerian hospitals, indicating that increased use of electronic registration reduces errors in patient information. Hospitals with functional electronic registration systems are expected to demonstrate fewer duplicate records, demographic errors, incomplete registration fields, incorrect patient identification numbers, and data-entry errors compared with hospitals relying primarily on manual registration. Automated validation, mandatory fields, standardized data formats, and duplicate-record alerts may improve the accuracy and completeness of patient information. Electronic registration may also facilitate faster retrieval of patient records and improve information sharing across hospital departments. However, system downtime, unreliable electricity and internet connectivity, inadequate staff training, poor system design, duplicate data entry, resistance to technology, and insufficient technical support may limit the effectiveness of electronic registration systems. The study therefore expects reliable, user-friendly, integrated, and adequately supported electronic patient registration systems to contribute significantly to reducing registration errors in Nigerian hospitals. The study is expected to contribute to the literature on electronic patient registration, registration errors, health information systems, digital health, medical records management, patient identification, data quality, healthcare efficiency, and patient safety in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare administrators, health information managers, information technology professionals, registration personnel, healthcare providers, development partners, and policymakers regarding strategies for improving the accuracy of patient information. The study will also provide evidence-based recommendations for expanding electronic patient registration systems, strengthening unique patient identification, improving automated data validation and duplicate-record detection, training registration personnel, integrating registration systems with electronic health records, strengthening technical support, improving power and connectivity infrastructure, and establishing routine data-quality audits across Nigerian hospitals.
