Impact of Community-Based Malaria Screening on Asymptomatic Malaria Detection among Adults in Nigeria

 

Abstract

Malaria remains a major public health concern in Nigeria, with transmission occurring in many communities and affecting populations across different age groups. Although malaria commonly presents with symptoms such as fever, chills, headache, and body weakness, some infected individuals may carry malaria parasites without experiencing obvious symptoms. Asymptomatic malaria infections may remain undetected when individuals do not seek healthcare because they feel healthy, thereby creating challenges for malaria surveillance, case management, and transmission control. Adults living in malaria-endemic communities may continue to harbour malaria parasites without recognizing the need for testing, particularly where routine malaria screening is limited. Community-based malaria screening provides an opportunity to identify malaria infections among individuals who may not present at health facilities by bringing diagnostic services closer to households, communities, workplaces, markets, community gatherings, and other accessible settings. Early detection of asymptomatic infections may support appropriate clinical assessment and management and contribute to malaria surveillance and control efforts. However, inadequate screening coverage, limited availability of malaria diagnostic supplies, shortage of trained healthcare workers, low community awareness, financial and geographical barriers, and weak referral and follow-up systems may limit the effectiveness of community-based malaria screening in Nigeria. Against this background, this study investigates the impact of community-based malaria screening on asymptomatic malaria detection among adults in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and the Health Systems Framework. The Health Belief Model explains how adults' perceptions of susceptibility to malaria, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community-based malaria screening. The Social Ecological Model emphasizes the influence of individual, household, community, environmental, and social factors on malaria prevention behaviours, healthcare utilization, and access to diagnostic services. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective community-based malaria screening and disease-control programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based malaria screening may influence the detection of asymptomatic malaria infections among adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adults aged 18 years and above residing in selected malaria-endemic communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible adults. Community-based malaria screening will be measured using indicators such as availability of community screening services, screening coverage, frequency of screening activities, number of adults screened, malaria diagnostic testing, availability of rapid diagnostic tests or other appropriate diagnostic methods, trained healthcare personnel, accessibility of screening locations, malaria health education, and referral arrangements. Asymptomatic malaria detection will be assessed using indicators such as positive malaria test results among individuals without malaria-related symptoms at the time of screening, newly identified asymptomatic infections, parasite detection, malaria parasite density where appropriate, repeat or confirmatory testing where clinically indicated, referrals for further assessment, and linkage to appropriate malaria management services. Data will be collected using structured questionnaires, community malaria screening registers, diagnostic testing records, healthcare facility records, referral registers, and relevant malaria control programme documents. Descriptive statistics will be used to summarize participants' characteristics, malaria screening coverage, diagnostic findings, and patterns of asymptomatic malaria detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of community-based malaria screening on asymptomatic malaria detection. Where appropriate, asymptomatic malaria detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based malaria screening has a significant positive impact on asymptomatic malaria detection among adults in Nigeria. Adults who participate in community-based malaria screening are expected to have a higher likelihood of having previously undetected malaria infections identified than adults without access to such services. Community-based screening may identify malaria infections among individuals who do not seek treatment because they do not experience noticeable symptoms, thereby improving knowledge of the distribution of malaria infections within communities. Bringing diagnostic services closer to adults may reduce geographical, financial, and time-related barriers and increase participation in malaria testing. Early identification may facilitate appropriate clinical assessment and management and provide useful information for malaria surveillance and community-level control activities. Community-based screening may also increase awareness of malaria transmission, prevention, and the importance of appropriate diagnostic testing. Conversely, inadequate screening coverage, limited diagnostic supplies, shortage of trained personnel, low community participation, financial constraints, seasonal variation in malaria transmission, and weak referral and follow-up systems may reduce the effectiveness of community-based malaria screening. The study therefore expects accessible and well-organized community-based malaria screening to contribute significantly to improved detection of asymptomatic malaria infections among adults in Nigeria. The study is expected to contribute to the literature on community-based malaria screening, asymptomatic malaria detection, malaria surveillance, malaria prevention and control, adult health, community health services, infectious disease control, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Malaria Elimination Programme, National Primary Health Care Development Agency, state malaria control programmes, primary healthcare centres, community health workers, malaria researchers, development partners, and policymakers regarding strategies for improving malaria detection and control. The study will also provide evidence-based recommendations for expanding community-based malaria screening programmes, improving access to reliable malaria diagnostic testing, strengthening community health education, increasing availability of trained healthcare workers and diagnostic supplies, improving referral and follow-up systems, integrating asymptomatic malaria detection into appropriate malaria surveillance activities, and strengthening community-level malaria prevention and control strategies across Nigeria.
