Impact of Community-Based Hypertension Screening on Hypertension Detection among Market Traders in Nigeria
Abstract
Hypertension is a major public health challenge in Nigeria and is an important risk factor for cardiovascular disease, stroke, kidney disease, and premature mortality. Despite its substantial health burden, many adults with hypertension remain undiagnosed because the condition may not produce noticeable symptoms in its early stages. Market traders represent an important occupational population that may face barriers to accessing routine healthcare services because of long working hours, financial constraints, limited time, and the need to remain at their places of business. Community-based hypertension screening provides an opportunity to take blood pressure assessment and related health services closer to market traders and other underserved populations, thereby improving opportunities for early identification of elevated blood pressure and referral for appropriate clinical assessment. Community-based screening may be conducted through market outreach programmes, primary healthcare facilities, mobile health services, health campaigns, and trained community health workers. Hypertension detection refers to the identification of individuals with elevated blood pressure through appropriate screening followed by repeated assessment or clinical evaluation where required. Against this background, this study investigates the impact of community-based hypertension screening on hypertension detection among market traders in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and the Health Systems Framework. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action and provides a framework for understanding market traders' participation in hypertension screening. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a basis for understanding how accessible screening services may influence hypertension detection. The Health Systems Framework emphasizes service delivery, health workforce, health information, health technologies, financing, and accessibility as important components of effective healthcare delivery. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based hypertension screening may influence hypertension detection among market traders in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from market traders aged 18 years and above in selected markets across Nigeria. A multistage sampling technique will be used to select states, local government areas, markets, and eligible participants. Community-based hypertension screening will be assessed using indicators such as availability of screening activities, frequency of screening, accessibility of screening locations, availability of trained screening personnel, availability and functionality of blood pressure measuring equipment, screening coverage, community mobilization, health education, privacy, referral services, follow-up arrangements, and continuity of screening activities. Hypertension detection will be assessed using indicators such as completion of blood pressure screening, identification of elevated blood pressure, repeated blood pressure assessment, referral for clinical evaluation, and newly identified cases of hypertension following appropriate diagnostic assessment. Data will be collected using structured questionnaires, blood pressure measurement records, screening registers, referral records, primary healthcare records, and relevant programme documents where available. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, and hypertension detection patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of community-based hypertension screening on hypertension detection. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that community-based hypertension screening has a significant positive impact on hypertension detection among market traders in Nigeria. Market traders who have access to regular and accessible community-based screening are expected to have a greater likelihood of having previously undiagnosed hypertension identified than traders who do not participate in screening activities. Bringing screening services directly to markets may reduce time, transportation, and financial barriers associated with visiting conventional healthcare facilities. Community mobilization and health education may also improve traders' awareness of hypertension and encourage participation in screening. Early identification of elevated blood pressure may facilitate referral for repeated measurement, clinical evaluation, and appropriate management. Conversely, inadequate screening coverage, shortages of trained personnel, malfunctioning blood pressure equipment, poor referral systems, limited follow-up, low awareness, and low participation may reduce the effectiveness of community-based screening. However, community-based screening alone may not fully determine hypertension detection because individual health awareness, perceived risk, socioeconomic circumstances, healthcare-seeking behaviour, family history, and access to confirmatory clinical services may also influence detection. The study is expected to contribute to the literature on community-based hypertension screening, hypertension detection, noncommunicable disease prevention, cardiovascular health, occupational health, community health, primary healthcare, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, noncommunicable disease programme managers, primary healthcare facilities, market associations, community health workers, healthcare professionals, development partners, and policymakers regarding strategies for improving early detection of hypertension among occupational populations. The study will also provide evidence-based recommendations for expanding market-based hypertension screening, improving availability of functional blood pressure equipment, training community health workers, strengthening health education and mobilization, establishing effective referral and follow-up systems, integrating hypertension screening into routine community outreach services, and improving access to appropriate hypertension diagnosis and care among market traders across Nigeria.
