Impact of Community-Based Elderly Health Assessment on Geriatric Health Problems Detection among Older Adults in Nigeria
Abstract
The ageing population in Nigeria is experiencing increasing health needs, making the early identification of geriatric health problems an important public health priority. Older adults may experience multiple chronic and age-related health conditions, including hypertension, diabetes, visual and hearing impairment, cognitive decline, mobility difficulties, nutritional problems, falls, functional limitations, and other conditions that may remain undetected because of limited access to healthcare, financial constraints, geographical barriers, social isolation, inadequate geriatric services, and low awareness of age-related health risks. Community-based elderly health assessment provides an opportunity to identify health problems among older adults through community outreach programmes, primary healthcare centres, mobile health services, senior citizen programmes, and other accessible community platforms. Early detection may facilitate appropriate clinical evaluation, treatment, rehabilitation, nutritional support, assistive services, counselling, and referral to specialized care where necessary. Against this background, this study investigates the impact of community-based elderly health assessment on geriatric health problems detection among older adults in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Comprehensive Geriatric Assessment Framework. The Health Belief Model explains how older adults' perceptions of susceptibility to age-related health problems, perceived severity, perceived benefits of health assessment, perceived barriers, and cues to action may influence participation in community-based elderly health assessment. The Social Ecological Model emphasizes the influence of individual, family, community, socioeconomic, cultural, environmental, and healthcare factors on older adults' health, healthcare-seeking behaviour, and access to preventive services. The Comprehensive Geriatric Assessment Framework emphasizes multidimensional evaluation of older adults' physical health, functional capacity, psychological well-being, cognitive status, nutritional status, medication use, and social circumstances to identify health problems and care needs. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based elderly health assessment may influence geriatric health problem detection among older adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adults aged 60 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, community organizations, and eligible older adults. Community-based elderly health assessment will be measured using indicators such as availability of elderly health assessment services, assessment coverage, frequency of assessment activities, blood pressure measurement, blood glucose assessment, vision assessment, hearing assessment, cognitive assessment, nutritional assessment, mobility and functional assessment, fall-risk assessment, medication review, mental health assessment, availability of trained healthcare personnel, health education, and referral arrangements. Geriatric health problem detection will be assessed using indicators such as newly identified chronic diseases, visual impairment, hearing impairment, cognitive impairment, nutritional problems, mobility difficulties, fall risk, functional limitations, medication-related problems, mental health concerns, newly detected age-related health conditions, referrals for clinical evaluation, and linkage to appropriate healthcare and support services. Data will be collected using structured questionnaires, validated geriatric assessment tools, community health assessment forms, healthcare records where available, screening registers, medication records, and referral registers. Descriptive statistics will be used to summarize participants' demographic and health characteristics, assessment coverage, identified health conditions, functional status, and patterns of geriatric health problem detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of community-based elderly health assessment on geriatric health problem detection. Where appropriate, detection rates before and after implementation of community-based elderly health assessment activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based elderly health assessment has a significant positive impact on geriatric health problem detection among older adults in Nigeria. Older adults who participate in community-based elderly health assessment are expected to have a higher likelihood of previously unrecognized health problems being identified than those without access to such assessment. Comprehensive assessment may facilitate early identification of hypertension, diabetes, visual and hearing impairment, cognitive difficulties, nutritional problems, fall risks, functional limitations, and other age-related health conditions. Early detection may provide opportunities for timely clinical evaluation, treatment, lifestyle modification, rehabilitation, nutritional intervention, medication review, assistive services, and referral to appropriate healthcare providers. Bringing assessment services closer to older adults may reduce geographical, financial, mobility, and transportation barriers and may reach individuals who rarely access formal healthcare services. Community-based health education may also improve awareness of age-related health risks and encourage older adults and their families to seek appropriate care. Conversely, inadequate assessment coverage, shortage of trained healthcare personnel, limited geriatric services, financial constraints, mobility difficulties, social isolation, and weak referral and follow-up systems may reduce the effectiveness of community-based elderly health assessment. The study therefore expects accessible and comprehensive community-based elderly health assessment to contribute significantly to improved detection and early management of geriatric health problems among older adults in Nigeria. The study is expected to contribute to the literature on community-based elderly health assessment, geriatric health problem detection, ageing and health, comprehensive geriatric care, chronic disease prevention, functional health, healthy ageing, community health services, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, geriatricians, nurses, community health workers, social workers, rehabilitation professionals, older adults' organizations, development partners, and policymakers regarding strategies for strengthening health services for older adults. The study will also provide evidence-based recommendations for expanding community-based elderly health assessment programmes, strengthening geriatric health services at primary healthcare level, increasing availability of trained healthcare personnel, improving access to basic screening and assessment equipment, strengthening referral and follow-up systems, promoting healthy ageing education, supporting older adults with functional limitations, and integrating comprehensive elderly health assessment into appropriate community-based primary healthcare programmes across Nigeria.
