Impact of Blood Donation Awareness Campaigns on Voluntary Blood Donation Practices among Young Adults in Nigeria
Abstract
Voluntary blood donation is an essential component of a safe and sustainable blood supply and plays an important role in the management of patients with severe anaemia, traumatic injuries, complications of childbirth, surgical conditions, and other medical emergencies. Despite the need for adequate blood supplies, Nigeria continues to face challenges related to insufficient voluntary blood donation, dependence on replacement and family-directed donations, limited public awareness, misconceptions about blood donation, fear of needles, concerns about weakness or adverse effects, and inadequate knowledge of donor eligibility and donation procedures. Young adults represent an important population for voluntary blood donation because they are generally healthy and may contribute to establishing sustainable blood donation practices. Blood donation awareness campaigns provide an opportunity to improve knowledge, address misconceptions, and encourage eligible young adults to participate in voluntary blood donation. Against this background, this study investigates the impact of blood donation awareness campaigns on voluntary blood donation practices among young adults in Nigeria. The study will be anchored on the Health Belief Model, Theory of Planned Behavior, and Social Cognitive Theory. The Health Belief Model explains how young adults' perceptions of susceptibility to blood shortages, perceived benefits of blood donation, perceived barriers, self-efficacy, and cues to action may influence their decision to donate blood. The Theory of Planned Behavior emphasizes attitudes toward blood donation, subjective norms, perceived behavioural control, and behavioural intentions as determinants of voluntary donation practices. Social Cognitive Theory emphasizes observational learning, peer influence, self-efficacy, reinforcement, social support, and environmental factors in shaping blood donation behaviour. Collectively, these theoretical perspectives provide a suitable framework for explaining how blood donation awareness campaigns may influence voluntary blood donation practices among young adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise young adults aged 18–35 years residing in selected urban and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, educational institutions, workplaces, youth organizations, and eligible participants. Blood donation awareness campaigns will be assessed using indicators such as exposure to campaign messages, frequency and duration of campaigns, information on donor eligibility, blood donation procedures, health requirements, donation intervals, benefits of voluntary donation, common concerns and misconceptions, safety of blood donation, management of minor donation-related reactions, confidentiality, blood screening procedures, locations of blood donation centres, mobile blood drives, and appropriate sources of blood-donation information. Voluntary blood donation practices will be assessed using indicators such as history of voluntary blood donation, frequency of donation, intention to donate, participation in organized blood drives, willingness to donate without receiving financial or replacement incentives, adherence to recommended donation intervals, registration as a voluntary donor where applicable, response to blood donation appeals, and continued participation in voluntary blood donation programmes. Data will be collected using structured questionnaires, standardized blood-donation awareness and practice assessment tools, blood-donation campaign records, donor registers where ethically accessible, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize participants' demographic and socioeconomic characteristics, previous donation experience, sources of blood-donation information, campaign exposure, awareness levels, and voluntary blood donation practices. Inferential statistical techniques, including chi-square tests, paired and independent t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of blood donation awareness campaigns on voluntary blood donation practices. Where a quasi-experimental design is adopted, voluntary blood donation practices before and after the awareness campaign may be compared with those of a comparison group or community to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that blood donation awareness campaigns have a significant positive impact on voluntary blood donation practices among young adults in Nigeria. Young adults exposed to structured, evidence-based, culturally appropriate, and sustained blood donation campaigns are expected to demonstrate greater willingness and participation in voluntary blood donation than young adults without comparable exposure. Campaigns may improve understanding of donor eligibility, donation procedures, safety, donation intervals, and the importance of maintaining an adequate blood supply. Education may also address misconceptions that blood donation causes permanent weakness, excessive blood loss, infertility, or serious illness and may improve confidence in the safety of regulated blood donation. Community-based campaigns, peer advocacy, mobile blood drives, and social-media communication may further increase opportunities for young adults to participate in voluntary donation. However, fear of needles, misconceptions, lack of awareness of donation centres, inconvenient donation locations, concerns about adverse effects, distrust of healthcare institutions, time constraints, and limited availability of organized blood drives may reduce the effectiveness of awareness campaigns alone. The study therefore expects accessible, credible, practical, and sustained blood donation awareness campaigns, supported by convenient donation opportunities and efficient donor services, to contribute significantly to improved voluntary blood donation practices among young adults in Nigeria. The study is expected to contribute to the literature on voluntary blood donation, blood donation awareness, donor recruitment, blood safety, young adult health, health education, community health promotion, transfusion services, healthcare preparedness, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Commission, blood transfusion centres, hospitals, universities, youth organizations, community health workers, healthcare professionals, public health practitioners, non-governmental organizations, development partners, and policymakers regarding strategies for strengthening voluntary blood donation. The study will also provide evidence-based recommendations for expanding blood donation awareness campaigns, improving donor education, strengthening mobile blood donation programmes, increasing access to convenient donation centres, addressing misconceptions and fears surrounding blood donation, improving donor communication and retention, and developing sustainable community-based strategies that promote regular voluntary blood donation among young adults across Nigeria.
