Impact of Antenatal Dental Education on Dental Care-Seeking Practices among Pregnant Women in Nigeria
Abstract
Oral health is an important component of maternal health because hormonal and physiological changes during pregnancy may increase susceptibility to gingival inflammation, dental caries, periodontal problems, and other oral health conditions. However, pregnant women in Nigeria may have limited knowledge of appropriate dental care during pregnancy and may avoid dental services because of misconceptions about the safety of dental treatment, fear of harm to the fetus, cost, cultural beliefs, previous negative experiences, or the perception that dental care is unnecessary during pregnancy. Delayed dental care-seeking may allow preventable oral health problems to worsen and may affect maternal wellbeing. Antenatal dental education provides an opportunity to improve pregnant women's knowledge of oral health, preventive dental care, appropriate dental visits, oral hygiene, and the safety of seeking professional dental care during pregnancy. Against this background, this study investigates the impact of antenatal dental education on dental care-seeking practices among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Health Literacy Framework, and Social Ecological Model. The Health Belief Model explains how pregnant women's perceptions of susceptibility to oral health problems, perceived severity, perceived benefits of dental care, perceived barriers, self-efficacy, and cues to action may influence dental care-seeking practices. The Health Literacy Framework emphasizes women's ability to access, understand, evaluate, and apply reliable information about oral health and dental services during pregnancy. The Social Ecological Model emphasizes the influence of individual, family, community, healthcare, cultural, socioeconomic, and environmental factors on access to and utilization of dental services. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal dental education may influence dental care-seeking practices among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise pregnant women aged 18 years and above attending selected public and private antenatal clinics across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, healthcare facilities, antenatal clinics, and eligible pregnant women. Antenatal dental education will be assessed using indicators such as exposure to oral-health education sessions, frequency and duration of education, information on pregnancy-related oral health changes, oral hygiene practices, tooth brushing, use of appropriate oral hygiene products, dental check-ups, management of dental pain, dental caries prevention, gum disease prevention, nutrition and oral health, misconceptions about dental treatment during pregnancy, safety of professional dental care, appropriate dental services, and sources of reliable oral-health information. Dental care-seeking practices will be assessed using indicators such as attendance at dental clinics during pregnancy, routine dental check-ups, seeking professional care for dental pain or bleeding gums, timely presentation for oral health problems, compliance with dental recommendations, preventive dental visits, use of qualified dental professionals, and follow-up attendance where required. Data will be collected using structured questionnaires, standardized oral-health knowledge and dental care-seeking assessment tools, antenatal records where ethically accessible, dental attendance records where available, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize participants' demographic and obstetric characteristics, gestational age, previous dental-care experience, oral health conditions, sources of oral-health information, exposure to antenatal dental education, and dental care-seeking patterns. Inferential statistical techniques, including chi-square tests, paired and independent t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of antenatal dental education on dental care-seeking practices. Where a quasi-experimental design is adopted, dental care-seeking outcomes before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that antenatal dental education has a significant positive impact on dental care-seeking practices among pregnant women in Nigeria. Pregnant women exposed to structured, evidence-based, culturally appropriate, and sustained antenatal dental education are expected to demonstrate greater utilization of appropriate dental services than women without comparable education. Education may improve women's understanding of pregnancy-related oral health changes, the importance of preventive dental care, appropriate oral hygiene, and the need to seek professional attention for dental pain, gum bleeding, swelling, or other oral health concerns. It may also correct misconceptions that dental treatment should be avoided completely during pregnancy and encourage women to discuss oral health concerns with qualified healthcare and dental professionals. Improved education may promote timely dental visits and reduce delays in seeking care. However, financial barriers, limited availability of dental services, geographical barriers, fear of dental procedures, cultural beliefs, concerns about fetal safety, low awareness, and weak integration of oral health into antenatal care may reduce the effectiveness of education alone. The study therefore expects accessible, practical, culturally sensitive, and sustained antenatal dental education, supported by integrated maternal and dental health services, to contribute significantly to improved dental care-seeking practices among pregnant women in Nigeria. The study is expected to contribute to the literature on antenatal dental education, maternal oral health, dental care-seeking, pregnancy-related oral health, preventive dental care, antenatal care, health education, women's health, oral health promotion, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare centres, antenatal clinics, dental clinics, dentists, midwives, nurses, obstetricians, public health practitioners, maternal-health organizations, development partners, and policymakers regarding strategies for strengthening maternal oral health. The study will also provide evidence-based recommendations for integrating dental education and referral into antenatal care, improving access to pregnancy-appropriate dental services, strengthening collaboration between dental and maternal-health professionals, addressing misconceptions about dental treatment during pregnancy, increasing awareness of preventive oral care, and developing sustainable interventions that promote appropriate dental care-seeking among pregnant women across Nigeria.
