Impact of Ambulance Dispatch Systems on Emergency Response Time in Nigerian Hospitals

 

Abstract

Timely emergency response is essential for reducing preventable deaths, complications, and disability among patients experiencing medical emergencies, road traffic injuries, trauma, obstetric emergencies, and other life-threatening conditions. Ambulance dispatch systems play an important role in coordinating emergency calls, locating available ambulances, prioritizing emergencies, assigning response units, communicating with ambulance crews, and monitoring emergency journeys. In Nigeria, emergency response may be affected by delays in receiving and processing emergency calls, limited ambulance availability, poor communication infrastructure, traffic congestion, inadequate dispatch coordination, inaccurate location information, and limited integration between emergency dispatch centres and healthcare facilities. Inefficient dispatch processes may increase the time between an emergency request and ambulance arrival, potentially delaying access to appropriate emergency care. Against this background, this study investigates the impact of ambulance dispatch systems on emergency response time in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and Queuing Theory. The Donabedian Model explains healthcare quality through structure, process, and outcome, with ambulance dispatch infrastructure representing an important structural component and emergency response time serving as a measurable outcome. Systems Theory emphasizes the interaction among emergency call centres, dispatch personnel, ambulance crews, communication networks, traffic conditions, healthcare facilities, and other components of the emergency medical system in determining response efficiency. Queuing Theory provides a framework for understanding emergency calls and ambulance requests as service demands requiring efficient allocation of limited emergency resources. Collectively, these theoretical perspectives provide a suitable framework for explaining how ambulance dispatch systems may influence emergency response time in Nigerian hospitals. The study will adopt a quantitative analytical comparative or quasi-experimental research design. The study population will comprise selected hospitals providing ambulance and emergency services, ambulance dispatch centres, emergency medical personnel, dispatch officers, ambulance records, and emergency response cases across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, hospitals, dispatch centres, ambulance units, emergency personnel, and eligible emergency response records. Ambulance dispatch systems will be assessed using indicators such as availability of centralized dispatch systems, computerized dispatch platforms, GPS or location-tracking capabilities, emergency call processing procedures, call prioritization, ambulance availability monitoring, automated ambulance assignment, communication between dispatchers and ambulance crews, real-time tracking, dispatch documentation, system reliability, and staff training. Emergency response time will be assessed using indicators such as call-answering time, call-processing time, dispatch time, ambulance mobilization time, travel time, total response time from emergency call to ambulance arrival, proportion of responses meeting established response targets, delayed responses, and cancelled or uncompleted emergency requests. Data will be collected using structured questionnaires, dispatch logs, ambulance tracking records, emergency call records, hospital emergency registers, ambulance service records, and relevant electronic dispatch-system data. Descriptive statistics will be used to summarize ambulance service characteristics, dispatch practices, emergency call patterns, and response-time distributions. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of ambulance dispatch systems on emergency response time. Where a quasi-experimental design is adopted, response times before and after implementation of an improved dispatch system, or between facilities using different dispatch approaches, may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective ambulance dispatch systems have a significant positive impact on emergency response efficiency, resulting in shorter emergency response times in Nigerian hospitals. Hospitals and emergency services using coordinated and technology-supported dispatch systems are expected to demonstrate faster call processing, quicker ambulance mobilization, improved allocation of available ambulances, and shorter overall response times compared with systems relying on less coordinated dispatch procedures. GPS-based location tracking, real-time ambulance monitoring, emergency prioritization, and efficient communication may enable dispatch personnel to identify and assign the nearest appropriate ambulance more quickly. However, limited ambulance availability, poor road infrastructure, traffic congestion, inadequate communication networks, unreliable electricity, shortage of trained dispatch personnel, inaccurate location information, and weak coordination between dispatch centres and healthcare facilities may reduce the effectiveness of ambulance dispatch systems. The study therefore expects reliable, integrated, adequately staffed, and technology-supported ambulance dispatch systems to contribute significantly to improved emergency response time in Nigeria. The study is expected to contribute to the literature on ambulance dispatch systems, emergency response time, emergency medical services, ambulance services, emergency healthcare, patient transport, healthcare coordination, health-system efficiency, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospitals, emergency medical service agencies, ambulance service providers, emergency dispatch centres, healthcare administrators, road safety authorities, telecommunications stakeholders, development partners, and policymakers regarding strategies for strengthening emergency medical response. The study will also provide evidence-based recommendations for improving centralized emergency dispatch systems, expanding GPS and real-time ambulance tracking, strengthening emergency communication infrastructure, improving ambulance availability and allocation, training dispatch personnel, developing standardized emergency call-prioritization protocols, integrating dispatch systems with hospitals, and establishing effective monitoring and performance-evaluation mechanisms to reduce emergency response times across Nigeria.
