Effect of Rehabilitation Services on Functional Recovery among Stroke Survivors in Nigeria

Abstract

Stroke is a major public health concern in Nigeria and is a leading cause of long-term disability among adults. Stroke survivors may experience persistent impairments affecting mobility, muscle strength, balance, speech, swallowing, cognition, and the ability to perform activities of daily living. These limitations can reduce independence, increase caregiver burden, and negatively affect quality of life. Rehabilitation services provide an important component of post-stroke care by addressing physical, functional, communication, and psychosocial impairments through interventions such as physiotherapy, occupational therapy, speech and language therapy, functional training, psychological support, and patient and caregiver education. However, utilization and availability of rehabilitation services in Nigeria may be constrained by limited rehabilitation facilities, shortages of trained professionals, treatment costs, transportation difficulties, inadequate referral systems, and limited awareness among stroke survivors and caregivers. Against this background, this study investigates the effect of rehabilitation services on functional recovery among stroke survivors in Nigeria. The study will be anchored on the International Classification of Functioning, Disability and Health (ICF) Framework, the Biopsychosocial Model, and the Donabedian Model of Healthcare Quality. The ICF Framework explains functioning and disability through the interaction between health conditions, body functions and structures, activities, participation, and environmental factors, providing a suitable framework for assessing functional recovery after stroke. The Biopsychosocial Model emphasizes the interaction of biological, psychological, and social factors in determining recovery and independence among stroke survivors. The Donabedian Model explains healthcare quality through structure, process, and outcome, with rehabilitation services representing an important care process and functional recovery serving as a measurable outcome. Collectively, these theoretical perspectives provide a suitable framework for explaining how rehabilitation services may influence functional recovery among stroke survivors in Nigeria. The study will adopt a quantitative prospective cohort or quasi-experimental research design. The study population will comprise adult stroke survivors receiving care in selected teaching hospitals, specialist hospitals, general hospitals, rehabilitation centres, physiotherapy clinics, and community-based rehabilitation programmes in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, rehabilitation centres, and eligible stroke survivors. Rehabilitation services will be assessed using indicators such as availability and frequency of physiotherapy, occupational therapy, speech and language therapy, mobility training, balance training, strength training, activities-of-daily-living training, functional rehabilitation, cognitive rehabilitation where appropriate, psychological support, caregiver education, home exercise programmes, assistive-device training, individualized rehabilitation plans, treatment duration, follow-up services, and continuity of rehabilitation care. Functional recovery will be assessed using appropriate standardized measures of mobility, activities of daily living, balance, muscle strength, gait, communication, and functional independence, including validated stroke functional assessment tools where appropriate. Data will be collected using structured questionnaires, patient medical records, rehabilitation registers, physiotherapy and occupational therapy records, speech therapy records, functional assessment tools, discharge records, and follow-up documentation. Descriptive statistics will be used to summarize participants' demographic and clinical characteristics, patterns of rehabilitation service utilization, and functional recovery outcomes. Inferential statistical techniques, including paired-sample tests, independent-sample tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the effect of rehabilitation services on functional recovery. Where a quasi-experimental design is adopted, functional outcomes before and after rehabilitation intervention may be compared, or stroke survivors receiving structured rehabilitation may be compared with similar survivors receiving usual post-stroke care. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that rehabilitation services have a significant positive effect on functional recovery among stroke survivors in Nigeria. Stroke survivors who receive structured and sustained rehabilitation are expected to demonstrate greater improvements in mobility, balance, muscle strength, activities of daily living, functional independence, and other relevant areas of recovery than those receiving limited or no rehabilitation services. Early initiation and continuity of rehabilitation may facilitate recovery by addressing physical impairments, improving functional skills, and helping survivors adapt to residual disabilities. Physiotherapy may improve mobility and strength, occupational therapy may promote independence in daily activities, while speech and language therapy may address communication and swallowing difficulties where required. Caregiver education and home-based rehabilitation may further support continued recovery after discharge. However, inadequate rehabilitation facilities, shortages of qualified rehabilitation professionals, high treatment costs, transportation barriers, delayed referral, poor adherence, caregiver limitations, and weak integration of rehabilitation into routine stroke care may reduce effectiveness. The study therefore expects accessible, timely, multidisciplinary, individualized, and adequately supervised rehabilitation services to contribute significantly to improved functional recovery among stroke survivors in Nigeria. The study is expected to contribute to the literature on stroke rehabilitation, functional recovery, rehabilitation services, physiotherapy, occupational therapy, speech and language therapy, disability management, functional independence, stroke survivorship, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, teaching hospitals, specialist hospitals, rehabilitation centres, physiotherapists, occupational therapists, speech and language therapists, neurologists, rehabilitation nurses, caregivers, development partners, and policymakers regarding strategies for improving post-stroke rehabilitation. The study will also provide evidence-based recommendations for expanding access to rehabilitation services, strengthening multidisciplinary stroke rehabilitation teams, improving early referral to rehabilitation, integrating rehabilitation into primary and secondary healthcare, increasing the availability of trained rehabilitation professionals, improving rehabilitation infrastructure, promoting community- and home-based rehabilitation, reducing financial and transportation barriers, and strengthening follow-up services to support functional recovery among stroke survivors across Nigeria.
