Effect of Postpartum Mental Health Education on Postpartum Mental Health Knowledge among New Mothers in Nigeria

Abstract

Postpartum mental health is an important public health concern among new mothers in Nigeria because the period following childbirth involves substantial physical, emotional, social, and psychological changes. New mothers may experience sleep disruption, physical recovery from childbirth, breastfeeding difficulties, childcare responsibilities, financial pressures, relationship challenges, inadequate social support, and concerns about their infants' health and development. These challenges may increase vulnerability to postpartum depression, anxiety, emotional distress, and other mental health difficulties. However, postpartum mental health problems may remain unrecognized because of limited knowledge, stigma, cultural misconceptions, normalization of postpartum emotional difficulties, and inadequate awareness of available mental health services. Postpartum mental health education provides an opportunity to equip new mothers with accurate information about common postpartum mental health conditions, risk factors, warning signs, coping strategies, available treatment options, support systems, and appropriate help-seeking pathways. Against this background, this study investigates the effect of postpartum mental health education on postpartum mental health knowledge among new mothers in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and the Theory of Planned Behavior. The Health Belief Model explains how new mothers' perceptions of susceptibility to postpartum mental health problems, perceived severity, perceived benefits of mental health education, perceived barriers, and cues to action may influence their acquisition and use of mental health knowledge. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, observational learning, social support, and environmental influences in developing mental health knowledge and appropriate health behaviours. The Theory of Planned Behavior explains how attitudes, subjective norms, perceived behavioural control, and behavioural intentions may influence mothers' recognition of mental health difficulties and willingness to seek appropriate assistance. Collectively, these theoretical perspectives provide a suitable framework for explaining how postpartum mental health education may influence postpartum mental health knowledge among new mothers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise women who have recently given birth and are receiving postpartum, postnatal, infant-care, or community maternal health services in selected public and private healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, postnatal clinics, and eligible new mothers. Postpartum mental health education will be measured using indicators such as exposure to structured educational sessions, frequency and duration of education, information about postpartum depression and anxiety, recognition of emotional and behavioural warning signs, identification of risk and protective factors, postpartum stress management, coping strategies, sleep and rest management, social and family support, stigma reduction, maternal self-care, partner involvement, available mental health services, referral pathways, treatment options, and emergency support. Postpartum mental health knowledge will be assessed using indicators such as mothers' ability to identify common postpartum mental health problems, recognize symptoms and warning signs, understand risk factors, distinguish clinically significant symptoms from normal postpartum emotional changes, identify appropriate coping strategies, understand available treatment and support options, recognize when professional assistance may be required, identify reliable sources of mental health information, and understand appropriate referral and emergency support pathways. Data will be collected using structured questionnaires and appropriately validated postpartum mental health knowledge instruments administered before and after the educational intervention where a quasi-experimental design is used. Descriptive statistics will be used to summarize participants' demographic, socioeconomic, obstetric, and postpartum characteristics, exposure to postpartum mental health education, and knowledge levels. Inferential statistical techniques, including paired or independent sample tests, chi-square tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the effect of postpartum mental health education on postpartum mental health knowledge. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that postpartum mental health education has a significant positive effect on postpartum mental health knowledge among new mothers in Nigeria. New mothers exposed to structured postpartum mental health education are expected to demonstrate improved knowledge of postpartum depression, anxiety, emotional distress, risk factors, warning signs, coping strategies, available treatments, and appropriate sources of support compared with their baseline knowledge or mothers without similar educational exposure. Education may improve mothers' ability to distinguish common postpartum emotional changes from symptoms that may require professional attention and may increase awareness of the importance of early recognition and appropriate help-seeking. Individual counselling, group postnatal education, educational materials, interactive discussions, and practical scenarios may strengthen mothers' understanding of postpartum mental health. However, stigma, cultural beliefs, inadequate mental health integration into postnatal services, limited availability of mental health professionals, financial barriers, childcare demands, sleep deprivation, and inadequate family support may limit the effectiveness and sustainability of postpartum mental health education. The study therefore expects accessible, culturally appropriate, practical, and sustained postpartum mental health education to contribute significantly to improved postpartum mental health knowledge among new mothers in Nigeria. The study is expected to contribute to the literature on postpartum mental health education, postpartum mental health knowledge, maternal mental health, postnatal care, maternal wellbeing, postpartum depression, postpartum anxiety, mental health promotion, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare facilities, postnatal clinics, obstetricians, midwives, nurses, psychologists, psychiatrists, counsellors, community health workers, maternal health programmes, development partners, and policymakers regarding strategies for strengthening postpartum mental health services. The study will also provide evidence-based recommendations for integrating postpartum mental health education into routine postnatal care, strengthening maternal mental health screening and referral systems, improving healthcare workers' capacity to provide postpartum mental health information, reducing stigma and misconceptions, increasing family and community support, and expanding access to appropriate mental health services for new mothers across Nigeria.
