Effect of Onchocerciasis Education on Preventive Practices among Residents of Endemic Communities in Nigeria
Abstract
Onchocerciasis, commonly known as river blindness, remains an important neglected tropical disease and public health concern in some endemic communities in Nigeria. The disease is caused by the parasitic worm Onchocerca volvulus and is transmitted through repeated bites of infected blackflies that breed near fast-flowing rivers and streams. Onchocerciasis may cause severe itching, skin changes, visual impairment, and permanent blindness, thereby affecting the health, productivity, education, and quality of life of affected individuals and communities. Onchocerciasis education provides residents with information about disease transmission, risk factors, signs and symptoms, prevention, treatment, community-directed interventions, and the importance of participating in available disease-control programmes. Preventive practices refer to behaviours adopted by residents to reduce the risk of onchocerciasis transmission and complications, including participation in recommended ivermectin distribution programmes, prompt reporting of symptoms, utilization of appropriate healthcare services, adoption of measures that reduce exposure to blackfly bites where feasible, and participation in community disease-control activities. In endemic communities, inadequate knowledge, misconceptions about the disease and its treatment, limited awareness of preventive programmes, and poor utilization of available health services may affect the adoption of appropriate preventive practices. Effective onchocerciasis education may improve residents' knowledge and encourage greater participation in preventive and control measures. Against this background, this study investigates the effect of onchocerciasis education on preventive practices among residents of endemic communities in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding residents' decisions to participate in onchocerciasis prevention and control activities. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping preventive health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from onchocerciasis education may influence preventive practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how onchocerciasis education may influence preventive practices among residents of endemic communities in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from adult residents of selected onchocerciasis-endemic communities in Nigeria using a structured interviewer-administered questionnaire and, where appropriate, a community health programme participation record. A multistage sampling technique will be employed to select states, local government areas, endemic communities, households, and eligible residents. Where feasible, selected communities or participant groups will be assigned to intervention and comparison groups. Residents in the intervention group will receive structured onchocerciasis education covering the causes and transmission of the disease, blackfly breeding environments, signs and symptoms, possible complications, prevention and control measures, the importance of ivermectin treatment, management of common misconceptions, and the importance of participating in community-directed treatment and other available disease-control activities. Preventive practices will be assessed using indicators such as participation in recommended ivermectin distribution, adherence to treatment recommendations, utilization of appropriate healthcare services, prompt reporting of symptoms, participation in community health activities, and appropriate measures for reducing exposure to blackfly bites where feasible. Descriptive statistics will be used to summarize residents' sociodemographic characteristics, onchocerciasis knowledge, exposure to education, and preventive practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the effect of onchocerciasis education on preventive practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that onchocerciasis education has a significant positive effect on preventive practices among residents of endemic communities in Nigeria. Residents exposed to structured onchocerciasis education are expected to demonstrate improved knowledge of disease transmission, symptoms, complications, and available prevention and control measures. Education may increase participation in ivermectin distribution programmes, improve adherence to recommended treatment, encourage early healthcare seeking, reduce misconceptions, and strengthen participation in community-based disease-control activities. However, education alone may not guarantee sustained preventive practices because residents may continue to face barriers such as limited access to healthcare services, inadequate availability of medicines, geographical isolation, misconceptions, fear of adverse effects, and limited community health resources. Consequently, onchocerciasis education is expected to be most effective when supported by reliable medicine distribution, accessible healthcare services, trained community health workers, sustained community engagement, and effective disease-surveillance and control programmes. The study is expected to contribute to the literature on neglected tropical diseases, onchocerciasis prevention, community health education, disease-control practices, health promotion, and public health in Nigeria by providing empirical evidence on the effect of onchocerciasis education on preventive practices among residents of endemic communities. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, local government health authorities, neglected tropical disease programmes, community health workers, healthcare providers, development partners, and policymakers regarding strategies for strengthening onchocerciasis prevention and control. The study will also provide evidence-based recommendations for improving community-based onchocerciasis education, strengthening participation in ivermectin distribution programmes, addressing misconceptions about treatment, improving community mobilization, strengthening health education through local health workers, and promoting sustained preventive practices in endemic communities across Nigeria.
