Effect of Newborn Hearing Screening Services on Hearing Loss Detection among Newborns in Nigeria
Abstract
Newborn hearing loss is an important public health and developmental concern because undetected hearing impairment may affect speech and language development, cognitive development, educational attainment, social interaction, and overall quality of life. Early identification of hearing loss provides an opportunity for timely diagnostic evaluation, referral, intervention, and family counselling, thereby improving developmental outcomes. Newborn hearing screening services provide a systematic approach to identifying infants who may have hearing impairment before noticeable developmental delays occur. Screening may involve technologies such as otoacoustic emissions and automated auditory brainstem response, depending on the resources and capacity of healthcare facilities. Despite the importance of early detection, access to newborn hearing screening remains limited in many healthcare settings in Nigeria due to inadequate screening equipment, shortages of trained audiology and neonatal healthcare personnel, limited awareness among parents and healthcare providers, financial constraints, and weak referral and follow-up systems. Hearing loss detection refers to the identification of newborns with suspected or confirmed hearing impairment through appropriate screening and subsequent diagnostic assessment. Against this background, this study investigates the effect of newborn hearing screening services on hearing loss detection among newborns in Nigeria. The study will be anchored on the Health Systems Framework, the Donabedian Model of Healthcare Quality, and Andersen's Behavioral Model of Health Services Use. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical technologies, essential supplies, financing, and accessibility as important components of effective healthcare delivery. The Donabedian Model evaluates healthcare quality through structure, process, and outcome and provides a framework for examining how the availability and quality of newborn hearing screening services influence hearing loss detection. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding access to newborn screening services. Collectively, these theoretical perspectives provide a suitable framework for explaining how newborn hearing screening services may influence hearing loss detection in Nigeria. The study will adopt a quantitative cross-sectional or retrospective hospital-based research design. The study population will comprise newborns delivered or receiving neonatal care in selected healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, hospitals, maternity units, neonatal units, and eligible newborns or medical records. Newborn hearing screening services will be assessed using indicators such as availability of functional screening equipment, availability of trained audiologists or healthcare personnel, screening protocols, screening coverage, timing of screening, screening frequency, quality of screening procedures, parental counselling, documentation of results, referral arrangements, and follow-up services. Hearing loss detection will be assessed using indicators such as completion of newborn hearing screening, identification of newborns who fail initial screening, completion of repeat screening or diagnostic evaluation, confirmed hearing loss, and referral for appropriate audiological or specialist assessment. Data will be collected from structured questionnaires, neonatal records, screening registers, audiology records, laboratory or diagnostic records, referral registers, and relevant hospital programme documents. Descriptive statistics will be used to summarize participants' characteristics, availability and utilization of hearing screening services, and hearing loss detection patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic regression, will be used to determine the effect of newborn hearing screening services on hearing loss detection. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that accessible and adequately implemented newborn hearing screening services have a significant positive effect on hearing loss detection among newborns in Nigeria. Newborns delivered in facilities with functional screening equipment, trained personnel, standardized screening procedures, and effective referral systems are expected to have a greater likelihood of hearing impairment being identified at an early stage. Early screening may reduce the likelihood that hearing loss remains undetected until infancy or early childhood, when developmental delays may already have become apparent. Effective parental counselling and follow-up systems may further improve completion of repeat screening and diagnostic assessment among newborns who do not pass initial screening. Conversely, inadequate screening equipment, shortages of trained personnel, high screening costs, limited awareness, poor documentation, weak referral systems, and loss to follow-up may reduce the effectiveness of newborn hearing screening programmes. The study may also demonstrate that screening services alone cannot ensure confirmed detection unless appropriate diagnostic and specialist services are available for newborns requiring further assessment. The study is expected to contribute to the literature on newborn hearing screening, hearing loss detection, neonatal health, child development, audiology, maternal and child health, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospital administrators, audiologists, paediatricians, neonatologists, nurses, maternal and child health programme managers, development partners, and policymakers regarding strategies for strengthening early identification of childhood hearing impairment. The study will also provide evidence-based recommendations for expanding newborn hearing screening services, increasing availability of functional screening equipment, strengthening training of healthcare personnel, integrating hearing screening into routine newborn care, improving parental education and counselling, establishing effective referral and follow-up systems, reducing financial barriers to screening and diagnostic services, and strengthening early intervention pathways for newborns identified with hearing loss across Nigeria.
