Effect of Malaria Test-and-Treat Services on Appropriate Antimalarial Treatment among Rural Residents in Nigeria

Abstract

Malaria remains a major public health challenge in Nigeria, particularly in rural communities where access to timely diagnostic and treatment services may be limited. Inappropriate treatment of suspected malaria, including treatment without diagnostic confirmation, incorrect drug selection, inappropriate dosage, and failure to complete recommended treatment, may contribute to poor treatment outcomes, unnecessary medicine use, increased healthcare costs, and the development of antimalarial drug resistance. Malaria test-and-treat services provide an approach to improving malaria case management by ensuring that individuals with suspected malaria are tested using appropriate diagnostic methods before antimalarial treatment is provided and that confirmed cases receive recommended treatment. These services may include malaria rapid diagnostic testing or microscopy, clinical assessment, appropriate antimalarial prescription, treatment counselling, referral, and follow-up. Appropriate antimalarial treatment refers to the use of the recommended antimalarial medicine, correct dosage, appropriate treatment duration, and treatment based on an appropriate diagnostic assessment. In rural Nigeria, barriers such as limited availability of diagnostic equipment, shortages of trained healthcare workers, inadequate medicine supplies, geographical barriers, financial constraints, and limited knowledge of malaria diagnosis and treatment may affect the quality of malaria case management. Strengthening malaria test-and-treat services may therefore improve appropriate treatment practices among rural residents. Against this background, this study investigates the effect of malaria test-and-treat services on appropriate antimalarial treatment among rural residents in Nigeria. The study will be anchored on the Health Systems Framework, Andersen's Behavioral Model of Health Services Use, and the Health Belief Model. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical technologies, essential medicines, financing, and access to healthcare as important components of effective malaria control. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how access to malaria diagnostic and treatment services may influence treatment practices. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action and provides a basis for understanding patients' acceptance of malaria testing and recommended treatment. Collectively, these theoretical perspectives provide a suitable framework for explaining how malaria test-and-treat services may influence appropriate antimalarial treatment among rural residents in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from rural residents who seek care for suspected malaria at selected primary healthcare facilities and community-based malaria service points across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, rural communities, healthcare facilities, and eligible participants. Malaria test-and-treat services will be assessed using indicators such as availability of malaria diagnostic testing, availability of trained healthcare workers, availability of rapid diagnostic test kits or microscopy services, adherence to testing procedures, availability of recommended antimalarial medicines, treatment counselling, prescription practices, referral services, follow-up procedures, and continuity of service. Appropriate antimalarial treatment will be assessed using indicators such as confirmation of malaria before treatment where clinically indicated, use of recommended antimalarial medicines, correct dosage, appropriate treatment duration, adherence to prescribed treatment, and compliance with relevant malaria treatment guidelines. Structured questionnaires, facility assessment checklists, prescription records, laboratory registers, malaria testing records, and treatment registers will be used for data collection where available. Descriptive statistics will be used to summarize participants' characteristics, availability of test-and-treat services, and antimalarial treatment patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of malaria test-and-treat services on appropriate antimalarial treatment. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that effective malaria test-and-treat services have a significant positive effect on appropriate antimalarial treatment among rural residents in Nigeria. Residents who have access to reliable malaria diagnostic testing and trained healthcare providers are expected to be more likely to receive treatment that is consistent with recommended malaria treatment guidelines. Diagnostic confirmation may reduce unnecessary antimalarial use among individuals without malaria while supporting prompt treatment among individuals with confirmed malaria. Availability of appropriate medicines and treatment counselling may further improve adherence to recommended treatment regimens and reduce inappropriate medicine use. Regular supervision and adherence to treatment guidelines may also strengthen the quality of malaria case management at primary healthcare level. Conversely, shortages of diagnostic kits, essential medicines, trained personnel, poor adherence to treatment guidelines, limited supervision, and inadequate patient education may reduce the effectiveness of test-and-treat services. However, malaria test-and-treat services alone may not fully determine treatment practices because patients' knowledge of malaria, healthcare-seeking behaviour, previous treatment experiences, household income, accessibility of healthcare facilities, and availability of medicines may also influence appropriate treatment. The study is expected to contribute to the literature on malaria test-and-treat services, appropriate antimalarial treatment, malaria case management, rural health, infectious disease control, primary healthcare, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Malaria Elimination Programme, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare facilities, healthcare workers, community health programmes, development partners, and policymakers regarding strategies for improving malaria diagnosis and treatment. The study will also provide evidence-based recommendations for strengthening malaria diagnostic capacity, ensuring consistent availability of rapid diagnostic test kits and recommended antimalarial medicines, improving healthcare-worker training and supervision, strengthening adherence to malaria treatment guidelines, improving patient counselling, strengthening referral and follow-up systems, and integrating effective test-and-treat approaches into routine primary healthcare services across rural Nigeria.
