Effect of Malaria Prevention Education on Insecticide-Treated Net Utilization among Households in Nigeria
Abstract
Malaria remains an important public health concern in Nigeria, particularly in communities where environmental, socioeconomic, and behavioural factors contribute to continued transmission. Insecticide-treated nets (ITNs) are an important malaria prevention intervention because they provide a physical barrier against mosquito bites while also reducing contact with malaria vectors. However, possession of an ITN does not necessarily result in consistent and correct utilization. Malaria prevention education provides households with information about malaria transmission, mosquito behaviour, the importance of sleeping under insecticide-treated nets, correct net installation and use, appropriate care and maintenance, replacement of damaged or old nets, and the importance of consistent nighttime protection. Insecticide-treated net utilization refers to the regular and appropriate use of ITNs by household members, particularly during nighttime sleeping periods. In Nigeria, inadequate utilization of ITNs may be associated with limited knowledge, misconceptions about malaria prevention, discomfort associated with sleeping under nets, inadequate installation, damaged nets, heat, household sleeping arrangements, and limited understanding of the importance of consistent use. Effective malaria prevention education may improve household knowledge, perceptions, confidence, and adherence to recommended ITN utilization practices. Against this background, this study investigates the effect of malaria prevention education on insecticide-treated net utilization among households in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding household malaria prevention decisions through perceived susceptibility to malaria, perceived severity, perceived benefits of ITN use, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, and environmental conditions in shaping preventive health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how malaria prevention education may improve household knowledge and attitudes and influence ITN utilization. Collectively, these theoretical perspectives provide a suitable framework for explaining how malaria prevention education may influence insecticide-treated net utilization among households in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from selected households in malaria-endemic communities in Nigeria using a structured interviewer-administered questionnaire, ITN utilization assessment, and, where feasible, direct observation of household ITN availability and installation. A multistage sampling technique will be employed to select states, local government areas, communities, households, and eligible household respondents. Where feasible, selected communities or household groups will be assigned to intervention and comparison groups. Households in the intervention group will receive structured malaria prevention education covering malaria transmission, mosquito behaviour, the importance of ITN use, correct installation and utilization, care and maintenance of nets, replacement of damaged nets, and the importance of consistent nighttime protection. ITN utilization will be assessed using indicators such as availability of an ITN in the household, proportion of household members sleeping under ITNs, frequency of nighttime use, correct installation, appropriate care and maintenance, and consistent use over a defined reference period. Where feasible, self-reported utilization will be complemented by direct observation and household inspection. Descriptive statistics will be used to summarize household sociodemographic characteristics, malaria prevention knowledge, ITN ownership, previous malaria education, and utilization practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic regression analysis, will be used to determine the effect of malaria prevention education on ITN utilization. Where appropriate, ITN utilization before and after the intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that malaria prevention education has a significant positive effect on insecticide-treated net utilization among households in Nigeria. Households exposed to structured malaria prevention education are expected to demonstrate improved knowledge of malaria transmission and increased awareness of the importance of consistent ITN use. Education may improve household members' ability to correctly install, use, maintain, and replace ITNs and may reduce misconceptions concerning discomfort, heat, or the perceived necessity of regular net use. Practical demonstrations and community-based education are expected to strengthen the translation of malaria prevention knowledge into consistent nighttime protection. However, education alone may not guarantee regular ITN utilization because households may continue to experience challenges such as inadequate access to functional ITNs, damaged nets, heat, sleeping arrangements, overcrowding, discomfort, and other environmental or socioeconomic factors. Consequently, malaria prevention education is expected to be most effective when supported by reliable access to quality ITNs, replacement mechanisms, community mobilization, environmental control measures, and sustained malaria prevention programmes. The study is expected to contribute to the literature on malaria prevention, insecticide-treated net utilization, household health practices, community health education, malaria control, health promotion, and public health in Nigeria by providing empirical evidence on the effect of malaria prevention education on ITN utilization among households. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Malaria Elimination Programme, state ministries of health, National Primary Health Care Development Agency, primary healthcare facilities, community health workers, malaria-control programmes, development partners, and policymakers regarding strategies for improving household-level malaria prevention. The study will also provide evidence-based recommendations for strengthening community-based malaria education, integrating ITN education into routine community health programmes, providing practical demonstrations on correct net use and maintenance, improving access to functional ITNs, strengthening replacement and distribution systems, addressing misconceptions about ITN utilization, and promoting consistent nighttime protection against malaria among households in Nigeria.
