Effect of Hospital Queue Management on Patient Waiting Time in Nigerian Outpatient Departments
Abstract
Efficient queue management is essential for improving patient flow, reducing congestion, and ensuring timely access to healthcare services in outpatient departments. Prolonged patient waiting time remains a common challenge in many Nigerian hospitals and may result from high patient volumes, limited healthcare personnel, inefficient registration procedures, poor appointment coordination, inadequate service points, delays in medical records retrieval, and ineffective management of patient queues. Excessive waiting time may reduce patient satisfaction, increase overcrowding, place additional pressure on healthcare workers, and affect the overall efficiency and quality of outpatient healthcare delivery. Hospital queue management involves organized procedures and systems for registering, prioritizing, directing, and monitoring patients as they move through different stages of outpatient care. Effective queue management may improve the allocation of available resources, regulate patient flow, and reduce unnecessary delays. Against this background, this study investigates the effect of hospital queue management on patient waiting time in Nigerian outpatient departments. The study will be anchored on the Donabedian Model of Healthcare Quality, Queuing Theory, and Systems Theory. The Donabedian Model explains healthcare quality through structure, process, and outcome, with queue management representing an important organizational process and patient waiting time serving as a measurable outcome. Queuing Theory provides a framework for understanding patient waiting lines and how arrival rates, service capacity, queue structure, and resource utilization influence waiting time. Systems Theory views the outpatient department as an interconnected system in which registration, medical records, consultation, laboratory, pharmacy, healthcare workers, patients, and administrative processes interact to influence patient flow. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital queue management may influence patient waiting time in Nigerian outpatient departments. The study will adopt a quantitative quasi-experimental or analytical comparative research design. The study population will comprise patients attending selected outpatient departments in public and private hospitals in Nigeria, as well as healthcare workers and administrative personnel involved in patient-flow and queue management. A multistage sampling technique will be used to select states, local government areas, hospitals, outpatient departments, clinical units, healthcare workers, and eligible patients. Hospital queue management will be assessed using indicators such as availability of formal queue-management procedures, queue registration systems, electronic queue systems, patient numbering, appointment coordination, patient prioritization, designated service points, queue monitoring, communication of waiting information, staff allocation, queue organization, service-point coordination, and staff training. Patient waiting time will be assessed using indicators such as time from arrival to registration, registration-to-medical-record retrieval time, time from registration to consultation, waiting time for laboratory services, waiting time at the pharmacy, total outpatient waiting time, number of service delays, frequency of prolonged waiting, and proportion of patients receiving services within expected timeframes. Data will be collected using structured questionnaires, patient-flow observation checklists, hospital queue records, electronic queue-management records where available, registration registers, medical records, laboratory records, pharmacy records, and time-tracking tools. Descriptive statistics will be used to summarize patients' demographic characteristics, outpatient attendance patterns, queue-management practices, and waiting-time distributions. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of hospital queue management on patient waiting time. Where a quasi-experimental design is adopted, patient waiting times before and after implementation of an improved queue-management system may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective hospital queue management has a significant negative effect on patient waiting time in Nigerian outpatient departments, indicating that improved queue management reduces delays. Outpatient departments with organized and adequately coordinated queue-management systems are expected to demonstrate shorter registration and consultation waiting times, reduced congestion, improved patient movement between service points, and shorter overall outpatient visit duration. Electronic queue systems, patient numbering, appropriate prioritization, real-time queue monitoring, and effective staff allocation may improve the distribution of patients across available service points. Better coordination between registration, medical records, consultation rooms, laboratories, and pharmacies may further reduce bottlenecks and unnecessary delays. However, high patient volumes, inadequate staffing, limited service capacity, poor infrastructure, unreliable information technology systems, delays in medical records retrieval, provider lateness, and weak implementation of queue-management procedures may reduce their effectiveness. The study therefore expects well-organized, patient-centred, technology-supported, and consistently implemented queue-management systems to contribute significantly to reducing patient waiting time in Nigerian outpatient departments. The study is expected to contribute to the literature on hospital queue management, patient waiting time, outpatient healthcare services, patient flow, hospital efficiency, healthcare operations, healthcare quality, digital health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, hospital administrators, outpatient department managers, health information managers, healthcare workers, information technology professionals, and policymakers regarding strategies for improving outpatient service efficiency. The study will also provide evidence-based recommendations for strengthening queue-management systems, expanding electronic queue technologies, improving patient numbering and prioritization procedures, optimizing staff allocation, strengthening coordination between outpatient service points, monitoring waiting-time indicators, improving medical records retrieval, and developing patient-centred queue-management strategies to reduce waiting times across Nigerian hospitals.
