Effect of Hospital Occupational Health Services on Occupational Health Service Utilization among Healthcare Workers in Nigeria

Abstract

Occupational health services are essential for protecting healthcare workers from workplace hazards and promoting their physical, psychological, and social wellbeing within healthcare facilities. Healthcare workers may be exposed to biological, chemical, physical, ergonomic, and psychosocial hazards, including infectious diseases, needle-stick injuries, hazardous substances, radiation, musculoskeletal strain, workplace violence, and occupational stress. Despite these risks, utilization of occupational health services among healthcare workers in Nigeria may be limited by inadequate awareness, restricted service availability, concerns about confidentiality, cost, inconvenient service locations, workload, stigma, and weak institutional occupational health systems. Hospital occupational health services provide mechanisms for preventing and managing work-related health risks through occupational health assessments, vaccination, exposure management, health screening, injury reporting, counselling, workplace risk assessment, and health education. Effective and accessible occupational health services may therefore encourage healthcare workers to utilize available occupational health resources and improve workplace health protection. Against this background, this study investigates the effect of hospital occupational health services on occupational health service utilization among healthcare workers in Nigeria. The study will be anchored on the Health Belief Model, the Social Ecological Model, and the Donabedian Model of Healthcare Quality. The Health Belief Model explains how healthcare workers' perceptions of susceptibility to occupational hazards, perceived severity, perceived benefits of occupational health services, perceived barriers, and cues to action may influence service utilization. The Social Ecological Model emphasizes the interaction between individual, interpersonal, organizational, community, and policy-level factors in determining access to and utilization of occupational health services. The Donabedian Model explains healthcare quality through structure, process, and outcome, with hospital occupational health services representing important structural and process components and service utilization representing a measurable outcome. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital occupational health services may influence occupational health service utilization among healthcare workers in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise healthcare workers employed in selected public and private hospitals across Nigeria. Participants may include doctors, nurses, pharmacists, laboratory professionals, community health workers, healthcare assistants, radiographers, physiotherapists, and other hospital personnel exposed to occupational hazards. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, and eligible healthcare workers. Hospital occupational health services will be assessed using indicators such as availability of dedicated occupational health units, pre-employment health assessments, periodic medical examinations, vaccination services, occupational exposure management, post-exposure prophylaxis, workplace injury management, health education, occupational risk assessments, ergonomic assessments, psychological support, workplace health surveillance, occupational safety training, referral services, confidentiality procedures, accessibility of services, service hours, availability of trained occupational health personnel, and institutional policies supporting occupational health. Occupational health service utilization will be assessed using indicators such as attendance at occupational health clinics, participation in routine health screening, use of vaccination services, reporting of occupational injuries and exposures, utilization of post-exposure services, participation in occupational health education, use of counselling services, attendance for periodic health assessments, and timely seeking of occupational health services when workplace health problems occur. Data will be collected using structured questionnaires, occupational health clinic registers, vaccination records, workplace injury and exposure records, staff medical records, occupational health reports, and relevant hospital documents. Descriptive statistics will be used to summarize healthcare workers' demographic and professional characteristics, occupational exposures, availability of services, and patterns of service utilization. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of hospital occupational health services on occupational health service utilization. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that accessible, comprehensive, and adequately organized hospital occupational health services have a significant positive effect on occupational health service utilization among healthcare workers in Nigeria. Healthcare workers employed in hospitals with dedicated occupational health units, readily available vaccination services, effective exposure-management systems, routine health assessments, and accessible occupational health personnel are expected to demonstrate higher utilization of available services. Convenient service locations, flexible operating hours, confidentiality, health education, and supportive institutional policies may further encourage healthcare workers to seek occupational health services. Early utilization of these services may facilitate timely management of workplace injuries and exposures and strengthen prevention of occupational health problems. However, limited staffing, inadequate resources, poor awareness, concerns about confidentiality, workload, stigma, inconvenient service hours, and weak institutional enforcement may reduce service utilization. The study therefore expects accessible, confidential, comprehensive, and adequately resourced hospital occupational health services to contribute significantly to improved occupational health service utilization among healthcare workers in Nigeria. The study is expected to contribute to the literature on hospital occupational health services, occupational health service utilization, healthcare worker safety, workplace health, occupational hazards, employee health, infection prevention, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, occupational health departments, healthcare administrators, healthcare workers, professional regulatory bodies, occupational health specialists, development partners, and policymakers regarding strategies for strengthening occupational health protection in healthcare facilities. The study will also provide evidence-based recommendations for establishing and strengthening hospital occupational health units, increasing healthcare workers' awareness of available services, improving accessibility and confidentiality, expanding vaccination and occupational exposure-management programmes, strengthening routine health surveillance, improving occupational health staffing, and integrating occupational health services into broader hospital workforce and patient-safety programmes across Nigeria.
