Effect of Hospital Complaint Management Systems on Patient Complaint Resolution in Nigeria

 

Abstract

Effective complaint management is an important component of patient-centred healthcare because it provides patients with an opportunity to report dissatisfaction, identify service deficiencies, and seek resolution of concerns arising during healthcare delivery. In Nigeria, patients may experience challenges relating to waiting times, staff communication, access to services, billing, medication availability, clinical care, privacy, cleanliness, and other aspects of hospital services. When complaints are not appropriately documented, investigated, communicated, and resolved, unresolved concerns may reduce patient trust, weaken accountability, and negatively affect perceptions of healthcare quality. Hospital complaint management systems provide structured mechanisms for receiving, recording, investigating, responding to, and monitoring patient complaints. Effective complaint management may enable hospitals to identify recurring service problems, provide timely resolutions, improve accountability, and strengthen patient-centred care. Against this background, this study investigates the effect of hospital complaint management systems on patient complaint resolution in Nigeria. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and the SERVQUAL Model. The Donabedian Model explains healthcare quality through structure, process, and outcome, with complaint management systems representing an important organizational process and complaint resolution representing a measurable outcome. Systems Theory views complaint management as an interconnected process involving patients, healthcare workers, hospital administrators, complaint officers, departments, communication channels, and quality-improvement structures. The SERVQUAL Model emphasizes service-quality dimensions such as responsiveness, reliability, assurance, empathy, and tangibles, which may influence how complaints are handled and resolved. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital complaint management systems may influence patient complaint resolution in Nigeria. The study will adopt a quantitative quasi-experimental or analytical comparative research design. The study population will comprise patients who have submitted complaints in selected public and private hospitals in Nigeria, hospital complaint-management officers, healthcare administrators, and other personnel involved in receiving and resolving patient complaints. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, complaint-management units, and eligible participants and complaint records. Hospital complaint management systems will be assessed using indicators such as availability of formal complaint policies, designated complaint officers or units, accessible complaint channels, complaint registration procedures, electronic complaint systems, complaint classification, acknowledgement procedures, investigation processes, referral and escalation mechanisms, response timelines, documentation, communication with complainants, staff training, monitoring, and follow-up procedures. Patient complaint resolution will be assessed using indicators such as proportion of complaints acknowledged, proportion investigated, complaints resolved within established timeframes, time taken to resolve complaints, proportion of complaints satisfactorily resolved, unresolved complaints, repeated complaints, communication of resolution outcomes, implementation of corrective actions, and complainants' satisfaction with the resolution process. Data will be collected using structured questionnaires, complaint registers, complaint-management databases, patient records, hospital quality-improvement reports, administrative records, and interviews or structured assessments where appropriate. Descriptive statistics will be used to summarize hospital characteristics, complaint patterns, complaint-management practices, and resolution outcomes. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of complaint management systems on patient complaint resolution. Where a quasi-experimental design is adopted, complaint-resolution outcomes before and after implementation of an improved complaint management system may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective hospital complaint management systems have a significant positive effect on patient complaint resolution in Nigeria. Hospitals with structured and accessible complaint management systems are expected to demonstrate higher proportions of complaints being acknowledged, investigated, resolved, and communicated to complainants within appropriate timeframes. Clear complaint procedures, designated complaint officers, systematic documentation, effective investigation, and escalation mechanisms may improve the efficiency and consistency of complaint resolution. Prompt communication with patients may also strengthen trust and demonstrate institutional responsiveness and accountability. Furthermore, analysis of complaint patterns may enable hospital management to identify recurring service problems and implement corrective measures. However, inadequate staffing, poor documentation, limited awareness of complaint procedures, fear of retaliation among patients, weak management commitment, delayed investigations, poor communication, and failure to implement corrective actions may reduce the effectiveness of complaint management systems. The study therefore expects accessible, transparent, responsive, and adequately monitored complaint management systems to contribute significantly to improved patient complaint resolution in Nigeria. The study is expected to contribute to the literature on hospital complaint management, patient complaint resolution, patient-centred care, healthcare quality, health service responsiveness, hospital accountability, patient experience, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public and private hospital administrators, patient liaison officers, healthcare workers, quality-improvement committees, regulatory bodies, development partners, and policymakers regarding strategies for strengthening complaint-management processes. The study will also provide evidence-based recommendations for establishing accessible complaint channels, standardizing complaint registration and investigation procedures, improving response and resolution timelines, strengthening staff training, integrating electronic complaint-management systems, improving communication with complainants, monitoring unresolved complaints, and using complaint data to guide continuous quality improvement across Nigerian hospitals.
