Effect of Health Facility Accreditation on Patient Safety Practices in Nigerian Hospitals

Abstract

Patient safety is a fundamental component of healthcare quality and remains an important public health concern in Nigeria. Hospitals may face challenges such as inadequate adherence to clinical protocols, medication errors, healthcare-associated infections, poor documentation, insufficient incident reporting, and weaknesses in infection prevention and control systems. Health facility accreditation provides a structured mechanism for assessing healthcare facilities against established standards of quality, safety, infrastructure, clinical practice, and organizational performance. Accreditation may encourage hospitals to strengthen policies, standardize clinical procedures, improve monitoring, and promote a culture of patient safety. However, differences in accreditation standards, implementation, institutional capacity, staff compliance, resource availability, and monitoring may influence its effectiveness. Against this background, this study investigates the effect of health facility accreditation on patient safety practices in Nigerian hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, High Reliability Organization Theory, and the Systems Theory of Patient Safety. The Donabedian Model explains healthcare quality through structure, process, and outcome, with accreditation representing an important structural and organizational quality mechanism and patient safety practices representing measurable processes of care. High Reliability Organization Theory emphasizes continuous monitoring, standardized procedures, risk identification, learning from errors, and organizational commitment to preventing avoidable harm. Systems Theory of Patient Safety views adverse events as products of interactions among healthcare workers, technology, organizational processes, infrastructure, and the broader healthcare environment rather than isolated individual failures. Collectively, these theoretical perspectives provide a suitable framework for explaining how health facility accreditation may influence patient safety practices in Nigerian hospitals. The study will adopt a quantitative analytical cross-sectional or comparative research design. The study population will comprise selected accredited and non-accredited public and private hospitals and healthcare workers involved in patient care in Nigeria. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, units, and eligible healthcare workers. Health facility accreditation will be measured using indicators such as accreditation status, duration of accreditation, compliance with accreditation standards, frequency of accreditation assessments, corrective-action implementation, quality-improvement activities, accreditation renewal, availability of safety policies, and institutional monitoring of accreditation requirements. Patient safety practices will be assessed using indicators such as adherence to patient identification procedures, medication safety, infection prevention and control, hand hygiene, surgical safety procedures, clinical documentation, communication during patient transfer, incident reporting, adverse-event monitoring, patient fall prevention, safe use of medical equipment, and compliance with established clinical protocols. Data will be collected using structured questionnaires, standardized patient-safety assessment checklists, hospital accreditation records, quality-improvement reports, incident-reporting records, infection prevention records, and direct observation where appropriate. Descriptive statistics will be used to summarize hospital characteristics, accreditation status, healthcare workers' characteristics, and patient safety practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of health facility accreditation on patient safety practices. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that health facility accreditation has a significant positive effect on patient safety practices in Nigerian hospitals. Accredited hospitals are expected to demonstrate stronger adherence to patient safety standards, clinical protocols, infection prevention procedures, medication safety practices, patient identification requirements, documentation standards, and incident-reporting systems compared with non-accredited facilities. Accreditation processes may encourage hospital management and healthcare workers to identify safety gaps, implement corrective actions, monitor performance, and establish continuous quality-improvement activities. Regular assessment and feedback may also strengthen institutional accountability and promote a culture of patient safety. However, inadequate resources, staff shortages, workload pressures, weak enforcement of standards, limited management commitment, and challenges sustaining accreditation requirements may reduce its effectiveness. The study therefore expects well-implemented and continuously monitored health facility accreditation programmes to contribute significantly to improved patient safety practices in Nigerian hospitals. The study is expected to contribute to the literature on health facility accreditation, patient safety practices, healthcare quality, hospital quality improvement, clinical governance, infection prevention and control, medication safety, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Health Facilities Accreditation and Regulatory Agency, National Health Insurance Authority, state ministries of health, hospital management boards, healthcare facilities, professional regulatory bodies, quality-improvement teams, healthcare workers, development partners, and policymakers regarding strategies for strengthening patient safety. The study will also provide evidence-based recommendations for expanding accreditation coverage, strengthening compliance monitoring, improving hospital quality-improvement systems, providing adequate resources for patient safety, strengthening healthcare-worker training, improving incident-reporting mechanisms, and ensuring continuous compliance with accreditation standards across Nigerian hospitals.
