Effect of Hand Hygiene Education on Handwashing Practices among Secondary School Students in Nigeria

Abstract

Hand hygiene is an important public health practice for preventing the transmission of infectious diseases in schools and communities. Secondary school students frequently come into contact with shared surfaces, food, water, and other students, making appropriate handwashing practices important for reducing the spread of communicable infections. Hand hygiene education provides students with information about the importance of handwashing, appropriate handwashing techniques, critical times for handwashing, appropriate use of soap and water, hand hygiene after toilet use, before eating or handling food, after coughing or sneezing, and after contact with potentially contaminated materials. Handwashing practices refer to the frequency, timing, technique, duration, and use of appropriate materials when students wash their hands. In Nigeria, inadequate handwashing practices among students may be associated with limited knowledge, misconceptions, inadequate access to water and soap, poorly maintained handwashing facilities, lack of supervision, and inconsistent school health education. Effective hand hygiene education may improve students' knowledge, skills, attitudes, and adherence to recommended handwashing practices. Against this background, this study investigates the effect of hand hygiene education on handwashing practices among secondary school students in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and the Knowledge-Attitude-Practice framework. The Health Belief Model provides a framework for understanding students' handwashing decisions through perceived susceptibility to infectious diseases, perceived severity, perceived benefits of hand hygiene, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, peer influence, school environment, and behavioural practices. The Knowledge-Attitude-Practice framework provides a basis for examining how hand hygiene education may improve students' knowledge and attitudes and influence handwashing behaviour. Collectively, these theoretical perspectives provide a suitable framework for explaining how hand hygiene education may influence handwashing practices among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from students in selected public and private secondary schools in Nigeria using a structured self-administered questionnaire, handwashing knowledge assessment, and handwashing-practice checklist. A multistage sampling technique will be employed to select states, local government areas, schools, classes, and eligible students. Where feasible, selected schools or student groups will be assigned to intervention and comparison groups. Students in the intervention group will receive structured hand hygiene education covering the importance of handwashing, correct handwashing techniques, critical times for handwashing, appropriate use of soap and water, hand hygiene after toilet use, before eating or handling food, after coughing or sneezing, and after contact with potentially contaminated materials. Handwashing practices will be assessed using indicators such as frequency of handwashing, handwashing after toilet use, handwashing before eating, handwashing before handling food, use of soap, correct handwashing technique, and appropriate duration of handwashing. Where feasible, self-reported practices will be complemented by structured observation of handwashing behaviour and assessment of available handwashing facilities. Descriptive statistics will be used to summarize students' sociodemographic characteristics, previous exposure to hygiene education, handwashing knowledge, and practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the effect of hand hygiene education on handwashing practices. Where appropriate, handwashing practices before and after the intervention will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that hand hygiene education has a significant positive effect on handwashing practices among secondary school students in Nigeria. Students exposed to structured hand hygiene education are expected to demonstrate improved knowledge of appropriate handwashing techniques, critical handwashing times, and the importance of using soap and water. Education may increase students' adherence to handwashing after toilet use, before eating, before handling food, and following exposure to potentially contaminated materials. Practical demonstrations, reminders, peer education, and repeated school-based health messages may further improve students' ability to translate knowledge into appropriate handwashing behaviour. However, education alone may not guarantee sustained handwashing practices because students may continue to experience barriers such as inadequate water supply, insufficient soap, poorly maintained handwashing stations, overcrowding, and inconsistent school hygiene supervision. Consequently, hand hygiene education is expected to be most effective when supported by functional handwashing facilities, reliable water and soap supplies, school hygiene policies, teacher supervision, and continuous health-promotion activities. The study is expected to contribute to the literature on hand hygiene, handwashing practices, school health education, infection prevention, communicable disease prevention, adolescent health, community health, and public health in Nigeria by providing empirical evidence on the effect of hand hygiene education on handwashing practices among secondary school students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, schools, local education authorities, school health programmes, teachers, school nurses, community health workers, development partners, and policymakers regarding strategies for strengthening hygiene practices among school-aged children. The study will also provide evidence-based recommendations for integrating practical hand hygiene education into school health programmes, improving access to water and soap, strengthening school handwashing infrastructure, training teachers to promote hygiene practices, implementing regular handwashing demonstrations and reminders, and establishing supportive school environments that encourage consistent handwashing practices among secondary school students in Nigeria.
