Effect of Family Planning Education on Modern Contraceptive Use among Women of Reproductive Age in Nigeria

 

Abstract

Family planning is an important component of reproductive and public health because it enables individuals and couples to make informed decisions about the timing and spacing of pregnancies. Modern contraceptive methods can help prevent unintended pregnancies, reduce pregnancy-related health risks, promote healthy birth spacing, and improve maternal and child health outcomes. Despite the availability of various modern contraceptive methods in Nigeria, utilization remains suboptimal in some populations due to inadequate knowledge, misconceptions, fear of side effects, partner influence, cultural and religious beliefs, concerns about infertility, cost, limited access to services, and other social and health-system barriers. Family planning education provides women with information about fertility awareness, available modern contraceptive methods, effectiveness, benefits, possible side effects, eligibility, correct use, management of common side effects, and sources of reliable family planning services. Modern contraceptive use refers to the utilization of recognized modern contraceptive methods, including condoms, oral contraceptive pills, injectables, implants, intrauterine devices, emergency contraception, and other approved modern methods, for pregnancy prevention or birth spacing. Effective family planning education may improve women's knowledge, confidence, informed decision-making, and utilization of appropriate modern contraceptive methods. Against this background, this study investigates the effect of family planning education on modern contraceptive use among women of reproductive age in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains contraceptive behaviour through perceived susceptibility, perceived benefits, perceived barriers, perceived severity, and cues to action, providing a basis for understanding women's decisions regarding modern contraceptive use. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping reproductive health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from family planning education may influence modern contraceptive use. Collectively, these theoretical perspectives provide a suitable framework for explaining how family planning education may influence modern contraceptive use among women of reproductive age in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from women of reproductive age in selected communities and primary healthcare facilities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, communities, healthcare facilities, and eligible women. Where feasible, selected communities or participant groups will be assigned to intervention and comparison groups. Women in the intervention group will receive structured family planning education covering fertility and pregnancy prevention, available modern contraceptive methods, effectiveness, benefits, eligibility, correct use, possible side effects, management of common side effects, misconceptions, informed choice, dual protection, and available family planning services. Modern contraceptive use will be assessed using indicators such as current use of a modern contraceptive method, consistency of use, method type, correct use, continuation of the selected method, and utilization of appropriate family planning services. Descriptive statistics will be used to summarize participants' sociodemographic and reproductive characteristics, family planning knowledge, exposure to education, and modern contraceptive use. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the effect of family planning education on modern contraceptive use. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that family planning education has a significant positive effect on modern contraceptive use among women of reproductive age in Nigeria. Women exposed to structured family planning education are expected to demonstrate improved knowledge of modern contraceptive methods and greater utilization of appropriate methods for pregnancy prevention and birth spacing. Education may reduce misconceptions and fears regarding contraception, improve understanding of potential side effects and their management, increase confidence in contraceptive decision-making, and encourage utilization of professional family planning services. However, education alone may not guarantee modern contraceptive use because women may continue to face barriers such as cost, limited availability of preferred methods, distance to healthcare facilities, partner opposition, cultural expectations, fear of side effects, misinformation, and concerns about fertility. Consequently, family planning education is expected to be most effective when supported by accessible and affordable contraceptive services, trained healthcare providers, availability of a broad range of modern methods, respectful counselling, and community-based reproductive health programmes. The study is expected to contribute to the literature on family planning, reproductive health, maternal health, health education, contraceptive use, and public health in Nigeria by providing empirical evidence on the effect of family planning education on modern contraceptive use among women of reproductive age. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, local government health authorities, primary healthcare facilities, reproductive health professionals, community health workers, development partners, and policymakers regarding strategies for improving modern contraceptive utilization. The study will also provide evidence-based recommendations for strengthening family planning education, improving contraceptive counselling, increasing access to affordable modern contraceptive methods, addressing misconceptions and concerns about side effects, strengthening community engagement, and promoting informed and voluntary contraceptive use among women of reproductive age across Nigeria.
