Abstract

Digital health services have emerged as an important approach for improving the accessibility, availability, and delivery of healthcare, particularly for populations living in underserved and geographically isolated communities. Digital health services include telemedicine, mobile health applications, electronic health information systems, remote consultations, digital appointment systems, and other technology-enabled healthcare services that facilitate communication between patients and healthcare providers. Healthcare access refers to the ability of individuals to obtain appropriate, affordable, timely, and acceptable healthcare services when needed. In Nigeria, rural communities continue to experience significant barriers to healthcare access, including inadequate healthcare facilities, shortages of healthcare professionals, long travel distances, poor transportation networks, high healthcare costs, limited availability of specialized services, and inadequate health information. These challenges may contribute to delayed healthcare seeking, untreated illnesses, preventable complications, and inequalities in health outcomes. The increasing adoption of digital health services provides opportunities to overcome some geographical and logistical barriers by connecting rural populations with healthcare providers, facilitating remote consultations, improving access to health information, supporting appointment scheduling, and enabling remote monitoring. However, challenges including poor internet connectivity, unreliable electricity supply, limited digital literacy, affordability of digital devices and data services, privacy concerns, and limited availability of digital health infrastructure may restrict their effectiveness in rural areas. Against this background, this study investigates the effect of digital health services on healthcare access among rural communities in Nigeria. The study is anchored on the Technology Acceptance Model (TAM), Diffusion of Innovations Theory, and Andersen's Behavioral Model of Health Services Use. The Technology Acceptance Model explains that individuals' adoption of digital health services is influenced by perceived usefulness and perceived ease of use, which subsequently affect their willingness to utilize technology-enabled healthcare services. Diffusion of Innovations Theory explains how new technologies are adopted within communities based on factors such as perceived advantages, compatibility, complexity, trialability, and observability. Andersen's Behavioral Model emphasizes that healthcare utilization is influenced by predisposing, enabling, and need-related factors, with digital health services representing an enabling mechanism that can facilitate access to healthcare. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how digital health services may influence healthcare access among rural communities in Nigeria. The study adopts a quantitative cross-sectional research design. Primary data will be collected from adult residents of selected rural communities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, rural communities, households, and eligible respondents. Digital health services will be measured using access to telemedicine, mobile health applications, remote medical consultations, digital appointment systems, electronic health information services, remote health monitoring, and mobile health communication platforms. Healthcare access will be measured using ability to obtain healthcare when needed, healthcare utilization, timeliness of receiving care, affordability of services, distance-related barriers, access to healthcare professionals, availability of health information, and continuity of care. Descriptive statistics will be used to summarize respondents' demographic and socioeconomic characteristics and patterns of digital health use and healthcare access. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be employed to determine the effect of digital health services on healthcare access. Appropriate diagnostic tests will also be conducted to assess multicollinearity, model specification, goodness of fit, and the robustness of the estimated relationships. The study anticipates that digital health services will have a significant positive effect on healthcare access among rural communities in Nigeria. Telemedicine and remote consultation services are expected to reduce geographical barriers by allowing rural residents to communicate with healthcare professionals without always travelling long distances to urban or secondary healthcare facilities. Mobile health platforms may improve access to health information, appointment scheduling, follow-up care, medication reminders, and health education, while digital communication between healthcare providers and patients may facilitate continuity of care. Digital health services may also reduce transportation-related costs and time associated with accessing healthcare. However, the effectiveness of digital health services may be constrained by poor network connectivity, unreliable electricity, limited digital literacy, lack of smartphones or other digital devices, affordability of internet data, concerns about privacy and confidentiality, and limited digital skills among both healthcare providers and community members. Consequently, the effect of digital health services on healthcare access is expected to depend partly on the availability of reliable digital infrastructure and users' ability to access and effectively utilize digital health technologies. This study is expected to make significant contributions to the literature on digital health, rural healthcare, health services utilization, and public health in Nigeria by providing empirical evidence on the relationship between digital health services and healthcare access among rural communities. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, local government health authorities, healthcare providers, telecommunications and technology stakeholders, development partners, and policymakers regarding the potential of digital health technologies to reduce rural healthcare inequalities. The study will also provide evidence-based recommendations for expanding telemedicine and mobile health services, improving digital infrastructure and internet connectivity in rural areas, strengthening digital literacy, reducing the cost of digital health access, improving electricity reliability, protecting patient data, and integrating digital health services into primary healthcare delivery to promote equitable access to quality healthcare services in Nigeria.
