Effect of Continuing Professional Education on Clinical Competence among Healthcare Workers in Nigeria

 

Abstract

Continuing professional education (CPE) is an important component of healthcare workforce development because healthcare workers require regular opportunities to update their knowledge, skills, professional competencies, and clinical practices in response to advances in medical science, technology, treatment guidelines, and healthcare standards. Continuing professional education refers to structured learning activities undertaken by healthcare professionals after initial professional qualification to maintain, improve, and expand their professional knowledge and skills. Clinical competence refers to the ability of healthcare workers to apply appropriate knowledge, technical skills, clinical judgment, communication, ethical principles, and professional standards effectively and safely in the delivery of patient care. In Nigeria, changes in disease patterns, emerging health threats, technological developments, revised clinical guidelines, and evolving patient-care standards make continuous professional development increasingly important. However, challenges such as limited access to training programmes, inadequate funding, staff shortages, workload pressures, geographical barriers, and limited institutional support may restrict healthcare workers' participation in continuing professional education. Inadequate opportunities for professional development may contribute to knowledge gaps, outdated clinical practices, reduced confidence, and inconsistent application of evidence-based care. Conversely, regular and relevant continuing professional education may improve healthcare workers' knowledge, technical skills, clinical judgment, and adherence to professional standards. Against this background, this study investigates the effect of continuing professional education on clinical competence among healthcare workers in Nigeria. The study is anchored on Human Capital Theory, Knowles' Adult Learning Theory, and Benner's Novice-to-Expert Model. Human Capital Theory views education and training as investments that enhance individuals' knowledge, skills, productivity, and professional performance. Knowles' Adult Learning Theory emphasizes that adult professionals learn most effectively when education is relevant to their work, problem-centred, practical, and connected to their previous experiences. Benner's Novice-to-Expert Model explains the development of clinical competence through progressive acquisition of knowledge, experience, skills, and professional judgment. Collectively, these theoretical perspectives provide a suitable framework for explaining how continuing professional education may influence clinical competence among healthcare workers in Nigeria. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected healthcare facilities in Nigeria using structured healthcare worker questionnaires, continuing professional education records, training attendance records, professional development documentation, clinical competence assessment tools, and direct observation checklists where feasible. A multistage sampling technique will be employed to select states, healthcare facilities, professional groups, departments, and healthcare workers. Continuing professional education will be assessed using indicators such as participation frequency, number of training programmes attended, relevance of training content, duration of training, accessibility of educational opportunities, participation in workshops and seminars, refresher courses, clinical updates, online learning activities, professional conferences, and institutional support for continuing education. Clinical competence will be assessed using indicators such as clinical knowledge, technical skills, patient assessment, clinical decision-making, communication, medication administration, infection prevention practices, documentation, emergency response, adherence to clinical guidelines, ethical practice, and ability to perform relevant professional procedures. Where feasible, objective competence assessments, direct observation, clinical simulations, professional records, and supervisor assessments will be used to complement self-reported information. Descriptive statistics will be used to summarize continuing professional education participation and levels of clinical competence. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of continuing professional education on clinical competence. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that participation in continuing professional education has a significant positive effect on clinical competence among healthcare workers in Nigeria. Healthcare workers who regularly participate in relevant professional education and clinical update programmes are expected to demonstrate higher levels of clinical knowledge, technical proficiency, decision-making ability, adherence to clinical guidelines, and professional practice than healthcare workers with limited access to continuing education. Regular training is expected to help healthcare workers update their knowledge, acquire new skills, correct outdated practices, and respond more effectively to emerging clinical challenges. Practical and competency-based training may be particularly useful in improving healthcare workers' ability to perform clinical procedures safely and accurately. Conversely, limited access to professional education may contribute to knowledge gaps, reduced familiarity with updated clinical guidelines, and inconsistent clinical practices. However, continuing professional education alone may not fully determine clinical competence because competence is also influenced by professional experience, availability of equipment and medicines, supervision, workload, organizational support, and opportunities to apply acquired knowledge and skills in clinical practice. Therefore, professional education should be supported by appropriate clinical resources and supportive work environments. The study is expected to contribute to the literature on continuing professional education, clinical competence, healthcare workforce development, professional development, clinical practice, healthcare quality, patient safety, and public health in Nigeria by providing empirical evidence on the role of continuous learning in strengthening healthcare worker competence. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, healthcare regulatory councils, professional associations, hospital management boards, primary healthcare authorities, healthcare training institutions, healthcare facility administrators, development partners, and policymakers regarding strategies for strengthening continuing professional education. The study will also provide evidence-based recommendations for expanding access to relevant professional training, strengthening institutional support for continuing education, improving funding for professional development, incorporating competency-based learning methods, expanding digital and online professional education, establishing regular clinical competency assessments, and aligning continuing professional education programmes with emerging healthcare needs and national clinical practice standards in Nigeria.
