Effect of Community Injury Prevention Education on Household Injury Prevention Practices in Rural Nigeria

 

Abstract

Household injuries are an important public health concern in rural Nigeria and may result from falls, burns, cuts, poisoning, drowning, animal bites, unsafe cooking practices, agricultural activities, electrical hazards, and other preventable risks within and around the home. Rural households may experience increased vulnerability to injuries because of limited access to health and emergency services, inadequate housing conditions, unsafe cooking and energy sources, poor environmental sanitation, limited safety equipment, and low awareness of injury prevention measures. Community injury prevention education provides an opportunity to increase household awareness of common injury hazards, promote safer behaviours, and strengthen the ability of families to prevent and respond appropriately to household injuries. Against this background, this study investigates the effect of community injury prevention education on household injury prevention practices in rural Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Social Ecological Model. The Health Belief Model explains how household members' perceptions of susceptibility to injuries, perceived severity of injury consequences, perceived benefits of preventive measures, perceived barriers, and cues to action may influence injury prevention behaviours. Social Cognitive Theory emphasizes observational learning, behavioural modelling, self-efficacy, reinforcement, and environmental influences in developing and maintaining safe household practices. The Social Ecological Model emphasizes the influence of individual, family, community, environmental, socioeconomic, and infrastructural factors on household injury risks and prevention. Collectively, these theoretical perspectives provide a suitable framework for explaining how community injury prevention education may influence household injury prevention practices in rural Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise households residing in selected rural communities in Nigeria, with eligible respondents being adults responsible for household safety and health-related decisions. A multistage sampling technique will be used to select geopolitical zones, states, rural local government areas, communities, villages, households, and eligible respondents. Community injury prevention education will be assessed using indicators such as exposure to injury prevention education sessions, frequency and duration of education, awareness of common household injury hazards, knowledge of fall prevention, burn prevention, poisoning prevention, electrical safety, drowning prevention, animal-bite prevention, safe storage of medicines and chemicals, safe cooking practices, agricultural injury prevention, and basic emergency-response measures. Household injury prevention practices will be assessed using indicators such as safe storage of medicines and chemicals, keeping dangerous objects away from children, safe cooking arrangements, proper handling of hot liquids and cooking equipment, maintaining clear walkways, safe storage of agricultural tools, appropriate electrical safety practices, supervision of children around water and fire, safe handling of animals, appropriate use of protective equipment during household or agricultural activities, and prompt response to injuries when they occur. Data will be collected using structured questionnaires, household injury-prevention assessment tools, environmental observation checklists, community health education records, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize household demographic characteristics, common injury hazards, previous injury experiences, exposure to community education, and prevention practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of community injury prevention education on household injury prevention practices. Where a quasi-experimental design is adopted, injury prevention practice scores before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community injury prevention education has a significant positive effect on household injury prevention practices in rural Nigeria. Households exposed to structured and practical injury prevention education are expected to demonstrate safer practices than households without comparable exposure. Education may improve household members' ability to identify injury hazards, safely store medicines and chemicals, prevent burns and falls, protect children from dangerous environments, improve electrical safety, prevent drowning and animal bites, and respond appropriately to injuries. Practical demonstrations and community-based safety activities may strengthen household members' confidence and ability to apply preventive measures within their homes. However, poor housing conditions, limited access to safety equipment, poverty, unsafe cooking and energy sources, inadequate water and sanitation infrastructure, low access to emergency services, and deeply established household practices may reduce the effectiveness of education alone. The study therefore expects accessible, practical, culturally appropriate, and continuous community injury prevention education, supported by improvements in household and community safety conditions, to contribute significantly to improved household injury prevention practices in rural Nigeria. The study is expected to contribute to the literature on household injury prevention, community health education, rural public health, injury epidemiology, health promotion, environmental health, child injury prevention, emergency preparedness, and community-based health interventions in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare centres, environmental health officers, community health workers, public health practitioners, traditional and community leaders, development partners, and policymakers regarding strategies for reducing preventable household injuries. The study will also provide evidence-based recommendations for strengthening community injury prevention education, improving household safety awareness, promoting safer cooking and energy practices, strengthening child injury prevention, improving safe storage of hazardous substances and equipment, enhancing community emergency preparedness, and developing sustainable injury prevention programmes for rural households across Nigeria.
