Effect of Community-Based Prostate Screening on Prostate Abnormality Detection among Men in Nigeria

Abstract

Prostate abnormalities remain an important men's health concern in Nigeria, particularly among middle-aged and older men, and may include prostate enlargement, abnormal prostate-specific antigen (PSA) levels, urinary symptoms, and other prostate-related conditions that require further clinical evaluation. Benign prostatic enlargement, prostatitis, and prostate cancer are among conditions that may affect men's prostate health and can have significant consequences when not identified and managed appropriately. Some prostate conditions may develop gradually without obvious symptoms, while fear, stigma, limited awareness, financial constraints, geographical barriers, and inadequate access to screening services may discourage men from seeking routine prostate health assessments. Delayed detection may result in late presentation, complications, increased treatment costs, and poorer health outcomes, particularly when serious prostate conditions are involved. Community-based prostate screening provides an opportunity to bring prostate health assessment closer to men through community outreach programmes, primary healthcare centres, mobile health services, men's health campaigns, workplaces, religious organizations, and other community platforms. Early identification of prostate abnormalities may facilitate timely clinical assessment, laboratory investigations, imaging where indicated, specialist referral, treatment, and continued monitoring. However, inadequate screening coverage, limited diagnostic facilities, shortages of trained healthcare personnel, low awareness, fear of screening procedures, financial barriers, and weak referral and follow-up systems may limit the effectiveness of community-based prostate screening programmes in Nigeria. Against this background, this study investigates the effect of community-based prostate screening on prostate abnormality detection among men in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how men's perceptions of susceptibility to prostate abnormalities, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community-based prostate screening. Community Health Theory emphasizes the delivery of accessible health services through community structures and the involvement of communities in disease prevention, health promotion, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective prostate health screening and healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based prostate screening may influence prostate abnormality detection among men in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adult men aged 40 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible men. Community-based prostate screening will be measured using indicators such as availability of screening services, screening coverage, frequency of community screening activities, accessibility of screening locations, number of men screened, prostate health education, assessment of urinary symptoms and relevant medical history, PSA testing where appropriate, availability of trained healthcare personnel, availability of screening and diagnostic resources, and referral arrangements. Prostate abnormality detection will be assessed using indicators such as abnormal PSA results, abnormal prostate examination findings where clinically indicated, urinary abnormalities suggestive of prostate disease, suspected benign prostatic enlargement, suspected prostatitis, suspected prostate cancer, newly identified prostate abnormalities, referrals for further urological assessment, diagnostic confirmation where appropriate, and linkage to treatment or continued monitoring. Data will be collected using structured questionnaires, community screening registers, prostate screening records, laboratory reports, clinical assessment records, referral registers, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, prostate health findings, and patterns of prostate abnormality detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of community-based prostate screening on prostate abnormality detection. Where appropriate, detection rates before and after implementation of community-based prostate screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based prostate screening has a significant positive effect on prostate abnormality detection among men in Nigeria. Men who participate in community-based prostate screening are expected to have a higher likelihood of having previously undetected prostate abnormalities identified than men without access to such services. Community-based screening may facilitate early identification of abnormal PSA levels, urinary symptoms, and other findings requiring further clinical assessment and may provide opportunities for timely referral, diagnostic evaluation, treatment, and monitoring. Bringing prostate health services closer to men may reduce geographical, financial, and time-related barriers and encourage men who may otherwise avoid routine health assessments to participate in screening. Community-based health education may also increase awareness of prostate health and encourage men to seek care when symptoms occur. Conversely, inadequate screening coverage, limited diagnostic facilities, shortages of trained healthcare personnel, low awareness, fear of screening, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of community-based prostate screening. The study therefore expects accessible and well-organized community-based prostate screening to contribute significantly to improved detection and early management of prostate abnormalities among men in Nigeria. The study is expected to contribute to the literature on community-based prostate screening, prostate abnormality detection, men's health, prostate health, urological health, prostate cancer prevention, preventive healthcare, community health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, urologists, laboratory professionals, community health workers, men's health organizations, development partners, and policymakers regarding strategies for improving prostate health among men. The study will also provide evidence-based recommendations for expanding community-based prostate screening programmes, improving access to appropriate prostate health assessments and diagnostic services, strengthening men's health education, increasing availability of trained healthcare personnel and screening resources, improving referral and follow-up systems, reducing financial and geographical barriers to prostate care, and promoting early presentation and appropriate management of prostate abnormalities among men across Nigeria.
