Effect of Child Immunization Record Digitization on Immunization Record Completeness in Nigerian Primary Healthcare Centres

Abstract

Childhood immunization is an essential public health intervention for preventing vaccine-preventable diseases and reducing childhood morbidity and mortality. Accurate and complete immunization records are important for tracking children's vaccination histories, determining vaccines that are due or missed, supporting defaulter tracing, monitoring immunization coverage, and informing healthcare planning and decision-making. However, immunization recordkeeping in some Nigerian primary healthcare centres may rely heavily on paper-based systems, which can be affected by incomplete documentation, misplaced records, illegible entries, duplication, delayed updating, and difficulties retrieving previous vaccination information. Digitization of child immunization records provides an opportunity to improve the storage, accessibility, retrieval, and updating of vaccination information and may strengthen the completeness and reliability of immunization records. Against this background, this study investigates the effect of child immunization record digitization on immunization record completeness in Nigerian primary healthcare centres. The study will be anchored on the Donabedian Model of Healthcare Quality, Systems Theory, and the Technology Acceptance Model. The Donabedian Model explains healthcare quality through structure, process, and outcome, with digital immunization record systems representing an important structural and process component and record completeness serving as a measurable outcome. Systems Theory views immunization recordkeeping as an interconnected process involving children, caregivers, healthcare workers, primary healthcare centres, digital information systems, immunization programmes, and health authorities. The Technology Acceptance Model explains how perceived usefulness and perceived ease of use may influence healthcare workers' adoption and effective utilization of digital immunization record systems. Collectively, these theoretical perspectives provide a suitable framework for explaining how child immunization record digitization may influence immunization record completeness in Nigerian primary healthcare centres. The study will adopt a quantitative quasi-experimental or analytical comparative research design. The study population will comprise selected primary healthcare centres providing routine immunization services in Nigeria, healthcare workers responsible for immunization documentation, and immunization records of eligible children. A multistage sampling technique will be used to select states, local government areas, communities, primary healthcare centres, healthcare workers, and child immunization records. Child immunization record digitization will be assessed using indicators such as availability of electronic immunization records, proportion of records digitized, electronic registration of children, digital entry of vaccination events, unique child identification, automated updating of vaccination histories, accessibility and retrieval of records, data validation, synchronization across service points, system usability, staff training, technical support, system reliability, and frequency of system downtime. Immunization record completeness will be assessed using indicators such as completeness of demographic information, vaccination dates, vaccine types, doses received, batch or lot information where applicable, facility identification, caregiver information, missed-dose documentation, next vaccination dates, completion of vaccination schedules, and consistency between electronic records, paper records, and child-held vaccination cards. Data will be collected using structured questionnaires, electronic immunization databases, child immunization registers, vaccination cards, facility records, data-quality assessment checklists, and relevant programme documents. Descriptive statistics will be used to summarize healthcare worker characteristics, levels of record digitization, and patterns of immunization record completeness. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of immunization record digitization on record completeness. Where a quasi-experimental design is adopted, record completeness before and after digitization may be compared, or digitized facilities may be compared with similar facilities using predominantly paper-based records. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that child immunization record digitization has a significant positive effect on immunization record completeness in Nigerian primary healthcare centres. Primary healthcare centres using functional digital immunization record systems are expected to demonstrate more complete and consistently updated vaccination records than facilities relying predominantly on paper-based documentation. Digitization may reduce missing entries, improve the legibility and accessibility of records, facilitate timely updating of vaccination information, reduce duplication, and make it easier for healthcare workers to retrieve children's vaccination histories during subsequent visits. Digital systems may also support identification of missed vaccinations and improve the accuracy of immunization monitoring and reporting. However, inadequate electricity supply, poor internet connectivity, system downtime, insufficient staff training, limited technical support, weak data-entry practices, inadequate equipment, and resistance to technological change may reduce the effectiveness of record digitization. The study therefore expects reliable, user-friendly, adequately supported, and routinely monitored digital immunization record systems to contribute significantly to improved completeness and quality of child immunization records in Nigerian primary healthcare centres. The study is expected to contribute to the literature on immunization record digitization, digital health, immunization information systems, record completeness, routine immunization, primary healthcare, health information management, child health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, local government health authorities, primary healthcare centres, immunization programme managers, health information officers, healthcare workers, development partners, and policymakers regarding strategies for strengthening immunization information management. The study will also provide evidence-based recommendations for expanding digital immunization record systems, improving electricity and connectivity infrastructure, training healthcare workers in digital data management, strengthening technical support, implementing routine data-quality audits, improving interoperability with other health information systems, ensuring secure and accessible immunization records, and integrating digital recordkeeping into routine immunization services across Nigerian primary healthcare centres.
