Influence of Patient Safety Education on Patient Safety Practices among Healthcare Workers in Nigerian Hospitals

Abstract

Patient safety is an essential component of quality healthcare because preventable errors, unsafe clinical practices, healthcare-associated infections, medication errors, diagnostic mistakes, communication failures, falls, and other adverse events can result in patient harm, prolonged hospitalization, increased healthcare costs, disability, and death. Healthcare workers play a central role in promoting patient safety because their knowledge, skills, communication, clinical decision-making, and adherence to established safety procedures directly influence the quality and safety of patient care. Patient safety education provides healthcare workers with knowledge and practical skills relating to patient identification, effective communication, medication safety, infection prevention and control, safe clinical procedures, prevention of falls and other patient injuries, incident reporting, teamwork, clinical handover, risk identification, error prevention, and appropriate response to adverse events. Patient safety practices refer to the actions undertaken by healthcare workers to prevent avoidable harm and promote safe patient care, including correct patient identification, medication verification, infection prevention, effective communication and handover, safe clinical procedures, incident reporting, risk assessment, and adherence to established patient safety protocols. In Nigeria, challenges such as high patient workloads, staff shortages, inadequate resources, weak reporting systems, limited patient safety training, communication gaps, and inconsistent monitoring of clinical practices may affect patient safety practices in hospitals. Effective patient safety education may improve healthcare workers' knowledge, safety awareness, confidence, teamwork, and adherence to recommended patient safety procedures. Against this background, this study investigates the influence of patient safety education on patient safety practices among healthcare workers in Nigerian hospitals. The study is anchored on Social Cognitive Theory, the Knowledge-Attitude-Practice framework, and Reason's Swiss Cheese Model of Accident Causation. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, behavioural capabilities, social influences, and environmental conditions in shaping healthcare workers' safety behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how patient safety education may improve knowledge and attitudes and subsequently influence safety practices. Reason's Swiss Cheese Model explains how adverse events may arise from the alignment of weaknesses within multiple layers of a healthcare system and emphasizes the importance of safety barriers, reporting systems, and error prevention strategies. Collectively, these theoretical perspectives provide a suitable framework for explaining how patient safety education may influence patient safety practices among healthcare workers in Nigerian hospitals. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from healthcare workers in selected public and private hospitals in Nigeria using a structured self-administered questionnaire, patient safety knowledge assessment, and an observational patient safety practices checklist. A multistage sampling technique will be employed to select states, local government areas, hospitals, clinical departments, and eligible healthcare workers. Where feasible, selected hospitals or healthcare worker groups will be assigned to intervention and comparison groups. Healthcare workers in the intervention group will receive structured patient safety education covering correct patient identification, medication safety, infection prevention and control, safe clinical procedures, effective communication, clinical handover, teamwork, fall prevention, risk identification, incident and near-miss reporting, error prevention, and appropriate response to adverse events. Patient safety practices will be assessed using indicators such as correct patient identification, medication verification, hand hygiene and infection prevention compliance, safe clinical procedures, effective communication and handover, patient risk assessment, fall prevention, incident reporting, and adherence to hospital patient safety protocols. Direct observation will be prioritized where feasible to reduce reliance on self-reported practices. Descriptive statistics will be used to summarize healthcare workers' sociodemographic and professional characteristics, previous patient safety training, patient safety knowledge, exposure to education, and patient safety practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of patient safety education on patient safety practices. Where appropriate, pre-intervention and post-intervention practice scores will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that patient safety education has a significant positive influence on patient safety practices among healthcare workers in Nigerian hospitals. Healthcare workers exposed to structured patient safety education are expected to demonstrate improved knowledge and greater adherence to recommended patient safety procedures. Education may improve patient identification, medication safety, infection prevention, communication and clinical handover, safe clinical procedures, patient risk assessment, fall prevention, incident reporting, and teamwork. It may also improve healthcare workers' awareness of preventable risks and encourage timely reporting of errors and near misses as opportunities for learning and system improvement. However, education alone may not guarantee sustained patient safety practices because healthcare workers may continue to face barriers such as staff shortages, high workloads, inadequate equipment and supplies, weak incident-reporting systems, poor communication structures, limited supervision, and inadequate institutional support for patient safety. Consequently, patient safety education is expected to be most effective when supported by strong hospital safety policies, adequate staffing, functional incident-reporting systems, regular monitoring, supportive supervision, effective communication systems, and continuous quality improvement programmes. The study is expected to contribute to the literature on patient safety, healthcare quality, healthcare worker education, clinical risk management, hospital management, infection prevention and control, medication safety, adverse event prevention, and public health in Nigeria by providing empirical evidence on the influence of patient safety education on patient safety practices among healthcare workers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management authorities, healthcare professional regulatory bodies, hospital quality improvement committees, patient safety programmes, healthcare administrators, professional associations, development partners, and policymakers regarding strategies for strengthening patient safety in Nigerian hospitals. The study will also provide evidence-based recommendations for integrating patient safety education into healthcare worker training and continuing professional development, strengthening incident and near-miss reporting systems, improving clinical communication and handover procedures, enhancing supportive supervision, ensuring adequate staffing and resources, and promoting a strong culture of patient safety within Nigerian hospitals.
