Influence of Patient Identification Practices on Medication Safety in Nigerian Hospitals

Abstract

Accurate patient identification is a fundamental component of safe healthcare delivery because correctly identifying patients before medication administration, diagnostic procedures, treatment, and other clinical interventions helps prevent errors that may cause avoidable harm. Patient identification practices refer to the procedures and systems used by healthcare workers to correctly establish and verify a patient's identity before providing healthcare services. These practices may include the use of patient identification bands, verification of patient names and identification numbers, confirmation of demographic information, use of multiple patient identifiers, electronic identification systems, and adherence to institutional patient-identification protocols. Medication safety refers to the prevention of medication-related errors and avoidable harm throughout the processes of prescribing, transcribing, dispensing, administering, monitoring, and documenting medicines. In Nigerian hospitals, challenges such as overcrowding, inadequate staffing, poor documentation, similar patient names, incomplete identification records, communication failures, and inconsistent adherence to patient-identification procedures may increase the risk of medication errors. Effective patient identification practices may reduce the likelihood of administering medicines to the wrong patient, administering incorrect medications or doses, and making documentation errors. Against this background, this study investigates the influence of patient identification practices on medication safety in Nigerian hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, Reason's Swiss Cheese Model of Accident Causation, and Systems Theory. Donabedian's Model identifies patient-identification systems, protocols, and resources as structural components that influence healthcare processes and patient outcomes. Reason's Swiss Cheese Model explains how medication errors may occur when weaknesses in multiple layers of defence align, with patient identification serving as an important barrier against wrong-patient medication errors. Systems Theory views medication safety as the outcome of interactions among healthcare workers, patients, identification systems, medication processes, documentation, technology, organizational policies, and hospital management. Collectively, these theoretical perspectives provide a suitable framework for explaining how patient identification practices may influence medication safety in Nigerian hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected Nigerian hospitals using structured healthcare worker questionnaires, patient identification practice assessment checklists, direct observation tools, medication-administration records, incident-reporting records, and relevant hospital documents. A multistage sampling technique will be employed to select states, hospitals, clinical departments, healthcare workers, and relevant patient-care units. Patient identification practices will be assessed using indicators such as use of patient identification bands, verification of patient name, confirmation of identification number, use of multiple identifiers, verification before medication administration, accurate patient records, electronic patient identification systems where available, compliance with identification protocols, and documentation of patient identity. Medication safety will be assessed using indicators such as correct-patient medication administration, medication administration errors, wrong-patient medication incidents, omitted or delayed medication, incorrect dosage, incorrect medication, documentation errors, medication reconciliation practices, and compliance with medication-administration safety procedures. Direct observation of medication-administration processes will be used where feasible to complement self-reported information, while medication incident reports and clinical records will provide additional evidence of medication-safety outcomes. Descriptive statistics will be used to summarize patient identification practices, medication-safety indicators, healthcare worker characteristics, and facility conditions. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the influence of patient identification practices on medication safety. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that effective patient identification practices have a significant positive influence on medication safety in Nigerian hospitals. Hospitals with consistent use of identification bands, multiple patient identifiers, appropriate verification procedures, accurate documentation, and adherence to patient-identification protocols are expected to demonstrate fewer wrong-patient medication incidents and stronger compliance with medication-safety procedures. Accurate patient identification is expected to strengthen the first stage of the medication-administration process by ensuring that medicines are administered to the intended patient. Consistent identification procedures may also reduce errors associated with patients who have similar names, incomplete records, emergency admissions, or changes in patient location within the hospital. Conversely, poor identification practices, missing identification bands, incomplete records, failure to verify patient identity, and reliance on a single identifier may increase the risk of medication errors and other patient-safety incidents. However, patient identification practices alone may not fully determine medication safety because medication safety is also influenced by prescribing practices, pharmacy systems, healthcare worker workload, availability of medicines, communication, electronic health systems, clinical protocols, and organizational safety culture. Therefore, accurate patient identification should form part of a comprehensive medication-safety system. The study is expected to contribute to the literature on patient identification, medication safety, patient safety, medication errors, healthcare quality, nursing, pharmacy practice, hospital management, and public health in Nigeria by providing empirical evidence on the importance of accurate patient identification in reducing medication-related risks. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare quality-improvement teams, nursing and pharmacy departments, medical directors, patient-safety committees, healthcare professional regulatory bodies, healthcare administrators, and policymakers regarding strategies for strengthening patient identification systems. The study will also provide evidence-based recommendations for improving patient identification protocols, ensuring consistent use of identification bands, strengthening multiple-identifier verification, improving patient documentation, introducing or expanding electronic identification technologies where feasible, training healthcare workers, conducting regular compliance audits, and integrating patient identification indicators into hospital medication-safety and patient-safety monitoring systems.
