Influence of Immunization Awareness on Childhood Vaccination Completion among Caregivers in Nigeria

 

Abstract

Childhood immunization is one of the most effective public health interventions for preventing vaccine-preventable diseases and reducing childhood morbidity and mortality. Despite the availability of routine childhood immunization services in Nigeria, incomplete vaccination remains a public health concern in some communities due to inadequate awareness, misconceptions about vaccines, concerns about adverse effects, accessibility challenges, missed appointments, and other social and health-system factors. Immunization awareness provides caregivers with information about the importance of childhood vaccination, recommended vaccination schedules, vaccine-preventable diseases, benefits and safety of vaccines, possible minor adverse reactions, vaccination service points, and the importance of completing all recommended doses. Childhood vaccination completion refers to a child receiving all age-appropriate vaccines and doses recommended under the applicable national childhood immunization schedule within the appropriate period. Caregivers play a central role in ensuring that children receive and complete routine vaccinations because they are often responsible for decisions regarding healthcare utilization and attendance at immunization sessions. In Nigeria, inadequate knowledge, misconceptions, misinformation, fear of vaccine-related adverse events, inconvenient service schedules, distance to health facilities, and missed vaccination appointments may contribute to incomplete vaccination. Effective immunization awareness may improve caregivers' knowledge and confidence and encourage timely completion of childhood vaccination. Against this background, this study investigates the influence of immunization awareness on childhood vaccination completion among caregivers in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains vaccination behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding caregivers' decisions to vaccinate their children. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from immunization awareness may influence childhood vaccination completion. Collectively, these theoretical perspectives provide a suitable framework for explaining how immunization awareness may influence childhood vaccination completion among caregivers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from caregivers of eligible children in selected communities in Nigeria using a structured interviewer-administered questionnaire and verification of children's vaccination status through available vaccination cards, facility records, or other appropriate immunization documentation where available. A multistage sampling technique will be employed to select states, local government areas, communities, households, and eligible caregivers. Where feasible, selected communities or participant groups will be assigned to intervention and comparison groups. Caregivers in the intervention group will receive structured immunization awareness education covering the importance of routine childhood vaccination, vaccine-preventable diseases, recommended vaccination schedules, benefits and safety of vaccines, management of common minor post-vaccination reactions, vaccination service points, appointment keeping, and the importance of completing all recommended doses. Childhood vaccination completion will be assessed based on documented receipt of age-appropriate vaccines and doses according to the applicable national immunization schedule. Descriptive statistics will be used to summarize caregivers' sociodemographic characteristics, immunization awareness, exposure to education, and children's vaccination completion status. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of immunization awareness on childhood vaccination completion. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that immunization awareness has a significant positive influence on childhood vaccination completion among caregivers in Nigeria. Caregivers exposed to structured immunization awareness education are expected to demonstrate improved knowledge of vaccine-preventable diseases, vaccination schedules, vaccine benefits and safety, and greater commitment to ensuring that their children receive all recommended doses. Education may reduce misconceptions and concerns about vaccination, improve appointment keeping, and encourage timely utilization of routine immunization services. However, awareness alone may not guarantee vaccination completion because caregivers may continue to face barriers such as distance to health facilities, transportation costs, inconvenient clinic schedules, vaccine stock-outs, competing household responsibilities, misinformation, and other service-delivery challenges. Consequently, immunization awareness is expected to be most effective when supported by reliable vaccine availability, accessible immunization services, effective reminder systems, trained healthcare workers, and community-based vaccination programmes. The study is expected to contribute to the literature on childhood immunization, health education, vaccination behaviour, maternal and child health, disease prevention, and public health in Nigeria by providing empirical evidence on the influence of immunization awareness on childhood vaccination completion among caregivers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), National Programme on Immunization, National Primary Health Care Development Agency immunization programmes, state ministries of health, local government health authorities, healthcare providers, community health workers, development partners, and policymakers regarding strategies for improving childhood vaccination completion. The study will also provide evidence-based recommendations for strengthening caregiver immunization education, improving vaccination reminders and follow-up systems, increasing community engagement, reducing vaccine-related misinformation, improving access to routine immunization services, and promoting completion of recommended childhood vaccinations across Nigeria.
