Influence of Healthcare Waste Management Practices on Environmental Health in Nigerian Healthcare Facilities
Abstract
Healthcare waste management is an important component of environmental health and healthcare safety because healthcare facilities generate different forms of waste that may pose risks to healthcare workers, patients, surrounding communities, and the environment when improperly handled. Healthcare waste includes infectious materials, sharps, pharmaceutical waste, chemical waste, pathological waste, general non-hazardous waste, and other materials generated during healthcare delivery. Effective healthcare waste management involves proper waste segregation, collection, storage, transportation, treatment, and final disposal in accordance with established environmental and public health standards. Environmental health refers to the protection of human health from environmental factors and the maintenance of conditions that support safe and healthy living environments. In Nigeria, healthcare facilities face several challenges in managing healthcare waste, including inadequate waste segregation, insufficient storage facilities, limited access to appropriate treatment technologies, poor disposal practices, inadequate protective equipment, insufficient staff training, and weak monitoring and enforcement mechanisms. Improper management of healthcare waste can result in environmental contamination, air pollution, water pollution, soil contamination, unpleasant odours, occupational injuries, exposure to infectious materials, and increased risks to communities living near healthcare facilities. Despite the existence of healthcare waste management guidelines and environmental regulations, variations in waste management practices persist across Nigerian healthcare facilities. Against this background, this study investigates the influence of healthcare waste management practices on environmental health in Nigerian healthcare facilities. The study is anchored on the Environmental Health Risk Theory, Waste Management Hierarchy, and Social Ecological Model. Environmental Health Risk Theory explains how exposure to hazardous environmental conditions can increase the likelihood of adverse health and environmental outcomes. The Waste Management Hierarchy emphasizes the prioritization of waste prevention, reduction, reuse, recycling, treatment, and safe disposal according to the level of environmental and health risk. The Social Ecological Model recognizes that effective healthcare waste management is influenced by interacting individual, organizational, community, environmental, and regulatory factors. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how healthcare waste management practices may influence environmental health within and around healthcare facilities. The study adopts a quantitative cross-sectional research design. Primary data will be collected from healthcare workers, waste handlers, environmental health officers, facility managers, and other relevant personnel in selected public and private healthcare facilities in Nigeria using structured questionnaires and facility observation checklists. A multistage sampling technique will be employed to select states, healthcare facilities, departments, and eligible respondents. Healthcare waste management practices will be measured using waste segregation at source, appropriate labeling and storage, waste collection frequency, internal transportation, availability and use of personal protective equipment, waste treatment, final disposal methods, staff training, record keeping, and compliance with healthcare waste management guidelines. Environmental health will be assessed using indicators such as cleanliness of healthcare environments, occurrence of environmental contamination, waste-related odour and pollution, evidence of improper waste disposal, occupational exposure risks, water and soil contamination indicators, and perceived environmental safety around healthcare facilities. Descriptive statistics will be used to summarize respondents' characteristics, facility conditions, waste management practices, and environmental health indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be employed to determine the influence of healthcare waste management practices on environmental health. Appropriate diagnostic tests will also be conducted to assess multicollinearity, model specification, goodness of fit, and the robustness of the estimated relationships. The study anticipates that effective healthcare waste management practices will have a significant positive influence on environmental health in Nigerian healthcare facilities. Proper segregation of waste at the point of generation is expected to reduce the mixing of hazardous and non-hazardous materials and consequently lower the risk of environmental contamination and accidental exposure. Appropriate collection, storage, transportation, treatment, and final disposal are also expected to reduce the release of infectious, chemical, and other hazardous materials into the surrounding environment. Adequate staff training and consistent use of personal protective equipment are expected to further reduce occupational and environmental risks associated with healthcare waste. Conversely, poor segregation, indiscriminate disposal, open burning, inadequate storage, and insufficient treatment may contribute to soil, water, and air pollution and increase environmental health risks. However, effective waste management may be constrained by inadequate funding, insufficient infrastructure, limited availability of treatment technologies, inadequate staffing, poor compliance, and weak regulatory monitoring. Consequently, improving environmental health in healthcare facilities requires not only appropriate waste management practices but also adequate institutional resources, staff capacity, effective supervision, and regulatory enforcement. This study is expected to make significant contributions to the literature on environmental health, healthcare safety, waste management, and public health in Nigeria by providing empirical evidence on the relationship between healthcare waste management practices and environmental health in healthcare facilities. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, Federal Ministry of Environment, National Primary Health Care Development Agency (NPHCDA), state ministries of health, healthcare facility managers, environmental health officers, waste management authorities, regulatory agencies, development partners, and policymakers regarding the environmental implications of healthcare waste management. The study will also provide evidence-based recommendations for strengthening waste segregation, improving healthcare waste treatment and disposal systems, providing adequate waste management infrastructure, enhancing staff training, ensuring availability of protective equipment, strengthening monitoring and compliance mechanisms, and promoting environmentally safe healthcare practices across Nigerian healthcare facilities.
