Influence of Fall Prevention Education on Fall Prevention Practices among Healthcare Workers in Nigerian Hospitals

Abstract

Patient falls are an important patient safety concern in healthcare facilities and may result in fractures, head injuries, disability, prolonged hospitalization, increased healthcare costs, and other adverse outcomes. Healthcare workers play a central role in preventing patient falls through early identification of patients at risk, environmental safety measures, appropriate supervision, mobility assistance, medication-related risk recognition, patient and family education, and timely implementation of preventive interventions. Fall prevention education provides healthcare workers with knowledge and practical skills relating to patient fall risk factors, fall risk assessment, environmental hazards, safe patient mobility and transfer, appropriate use of assistive devices, supervision of high-risk patients, medication-related fall risks, patient and caregiver education, safe bed and room arrangements, appropriate documentation, and incident reporting. Fall prevention practices refer to the actions undertaken by healthcare workers to reduce patients' risk of falling, including fall risk assessment, environmental safety checks, appropriate supervision, mobility assistance, use of assistive devices, safe positioning, patient and caregiver education, and documentation of preventive interventions. In Nigerian hospitals, inadequate fall prevention education, high patient workloads, staff shortages, limited availability of mobility and safety equipment, environmental hazards, inadequate monitoring systems, and inconsistent implementation of fall prevention protocols may affect fall prevention practices among healthcare workers. Effective fall prevention education may improve healthcare workers' knowledge, safety awareness, clinical decision-making, and adherence to recommended preventive measures. Against this background, this study investigates the influence of fall prevention education on fall prevention practices among healthcare workers in Nigerian hospitals. The study is anchored on Social Cognitive Theory, the Knowledge-Attitude-Practice framework, and Reason's Swiss Cheese Model of Accident Causation. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, behavioural capabilities, social influences, and environmental conditions in shaping healthcare workers' safety practices. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from fall prevention education may influence preventive practices. Reason's Swiss Cheese Model explains how patient falls may result from the alignment of weaknesses within multiple layers of a healthcare system and emphasizes the importance of preventive barriers, environmental controls, monitoring, and reporting systems. Collectively, these theoretical perspectives provide a suitable framework for explaining how fall prevention education may influence fall prevention practices among healthcare workers in Nigerian hospitals. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from healthcare workers providing care to hospitalized patients in selected public and private hospitals in Nigeria using a structured self-administered questionnaire, fall prevention knowledge assessment, and an observational fall prevention practices checklist. A multistage sampling technique will be employed to select states, local government areas, hospitals, clinical departments, and eligible healthcare workers. Where feasible, selected hospitals or healthcare worker groups will be assigned to intervention and comparison groups. Healthcare workers in the intervention group will receive structured fall prevention education covering patient fall risk factors, fall risk assessment, environmental hazards, safe patient mobility and transfer, appropriate use of assistive devices, supervision of high-risk patients, medication-related fall risks, patient and caregiver education, safe bed and room arrangements, documentation, and incident reporting. Fall prevention practices will be assessed using indicators such as fall risk assessment, environmental safety checks, appropriate patient supervision, mobility assistance, use of assistive devices, safe positioning, patient and caregiver education, appropriate bed and room arrangements, and documentation of preventive measures. Direct observation and review of relevant healthcare records will be prioritized where feasible to reduce reliance on self-reported practices. Descriptive statistics will be used to summarize healthcare workers' sociodemographic and professional characteristics, previous fall prevention training, fall prevention knowledge, exposure to education, and fall prevention practice scores. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of fall prevention education on fall prevention practices. Where appropriate, pre-intervention and post-intervention practice scores will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that fall prevention education has a significant positive influence on fall prevention practices among healthcare workers in Nigerian hospitals. Healthcare workers exposed to structured fall prevention education are expected to demonstrate greater adherence to recommended preventive procedures, including fall risk assessment, environmental safety checks, appropriate supervision, mobility assistance, safe patient transfer, use of assistive devices, patient and caregiver education, and documentation. Education may improve healthcare workers' ability to identify patients at increased risk of falling and implement appropriate preventive measures before an incident occurs. Practical and case-based education may further strengthen healthcare workers' confidence and ability to apply fall prevention principles to different patient circumstances. However, education alone may not guarantee sustained fall prevention practices because healthcare workers may continue to face barriers such as inadequate staffing, high workloads, environmental hazards, insufficient mobility equipment, overcrowding, and weak institutional monitoring systems. Consequently, fall prevention education is expected to be most effective when supported by adequate staffing, safe hospital environments, appropriate assistive and safety equipment, standardized fall prevention protocols, regular risk assessments, supportive supervision, and continuous quality improvement. The study is expected to contribute to the literature on patient falls, fall prevention, patient safety, healthcare worker education, nursing practice, clinical risk management, hospital quality improvement, and public health in Nigeria by providing empirical evidence on the influence of fall prevention education on fall prevention practices among healthcare workers. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management authorities, Nursing and Midwifery Council of Nigeria, healthcare professional bodies, hospital quality improvement committees, healthcare administrators, clinical departments, development partners, and policymakers regarding strategies for reducing preventable patient falls in Nigerian hospitals. The study will also provide evidence-based recommendations for integrating fall prevention education into healthcare worker training and continuing professional development, strengthening routine fall risk assessment, improving hospital environmental safety, ensuring availability of appropriate mobility and safety equipment, enhancing patient and caregiver education, strengthening incident reporting and monitoring systems, and promoting consistent evidence-based fall prevention practices in Nigerian hospitals.
