Influence of Community-Based Vision Screening on Visual Impairment Detection among Market Traders in Nigeria
Abstract
Visual impairment remains an important public health concern among adults in Nigeria and may negatively affect productivity, safety, mobility, communication, and overall quality of life. Market traders may be particularly vulnerable to undetected visual problems because of prolonged working hours, limited access to routine eye-care services, financial constraints, occupational exposure to dust and other environmental hazards, and the need to prioritize daily economic activities over preventive healthcare. Common causes of visual impairment among adults include refractive errors, cataract, glaucoma, diabetic eye disease, age-related eye conditions, and other ocular disorders. Many of these conditions may develop gradually without obvious symptoms during their early stages, resulting in delayed diagnosis and treatment. Community-based vision screening provides an opportunity to bring basic eye assessment services closer to market traders through outreach programmes, mobile eye clinics, market-based health campaigns, primary healthcare centres, and other community platforms. Early detection may facilitate referral for comprehensive eye examination, corrective lenses, medical treatment, surgical intervention where appropriate, and other forms of eye care. However, limited screening coverage, inadequate eye-care equipment, shortages of trained eye-care personnel, low awareness, financial barriers, and weak referral systems may limit the effectiveness of community-based vision screening programmes in Nigeria. Against this background, this study investigates the influence of community-based vision screening on visual impairment detection among market traders in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how market traders' perceptions of susceptibility to visual impairment, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in community-based vision screening. Community Health Theory emphasizes the delivery of accessible health services through community structures and the involvement of communities in disease prevention, health promotion, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective eye-care and screening programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based vision screening may influence visual impairment detection among market traders in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise market traders aged 18 years and above working in selected markets across Nigeria. A multistage sampling technique will be used to select states, local government areas, markets, sections or trading units, and eligible market traders. Community-based vision screening will be measured using indicators such as availability of screening services, screening coverage, frequency of screening activities, accessibility of screening locations, number of traders screened, visual acuity assessment, availability of trained optometrists or other eye-care personnel, availability of screening equipment, eye health education, and referral arrangements. Visual impairment detection will be assessed using indicators such as abnormal visual acuity results, reduced vision, suspected refractive errors, cataract-related visual impairment, suspected glaucoma, diabetic eye abnormalities, other ocular abnormalities, number of traders referred for comprehensive eye examination, confirmed visual impairment cases, and provision or recommendation of appropriate corrective or medical interventions. Data will be collected using structured questionnaires, community screening registers, visual acuity records, optometric assessment records, referral registers, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, visual assessment findings, and patterns of visual impairment detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of community-based vision screening on visual impairment detection. Where appropriate, visual impairment detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based vision screening has a significant positive influence on visual impairment detection among market traders in Nigeria. Market traders who participate in community-based vision screening are expected to have a higher likelihood of having previously undetected visual impairment or eye abnormalities identified than traders without access to such services. Community-based screening may improve early identification of reduced visual acuity and other eye conditions and provide opportunities for timely referral, comprehensive eye examination, prescription of corrective spectacles, medical treatment, and surgical intervention where appropriate. Bringing vision screening services directly to markets may reduce geographical, financial, and time-related barriers that may prevent traders from attending conventional eye-care facilities. Early detection may improve traders' ability to work safely and effectively and may reduce the social and economic consequences of avoidable visual impairment. Conversely, inadequate screening coverage, limited availability of trained eye-care personnel, insufficient screening equipment, low awareness, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of community-based vision screening. The study therefore expects accessible and well-organized community-based vision screening to contribute significantly to improved detection of visual impairment among market traders in Nigeria. The study is expected to contribute to the literature on community-based vision screening, visual impairment detection, market traders' health, adult eye health, preventive eye care, occupational health, community health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, optometrists, ophthalmologists, eye-care organizations, market associations, community health workers, development partners, and policymakers regarding strategies for improving access to eye-care services among economically active adults. The study will also provide evidence-based recommendations for expanding community-based vision screening programmes, integrating eye screening into primary healthcare and community outreach services, improving access to affordable eye examinations and corrective spectacles, strengthening training of community health and eye-care personnel, increasing eye health awareness among market traders, strengthening referral and follow-up systems, and improving access to appropriate treatment for visual impairment across Nigeria.
