Influence of Antenatal Health Education on Antenatal Care Attendance among Pregnant Women in Nigeria

Abstract

Antenatal care (ANC) is an essential component of maternal and reproductive health services because it provides pregnant women with opportunities to receive preventive, promotive, and clinical care throughout pregnancy. Regular antenatal care attendance enables early identification and management of pregnancy-related complications, provision of health education, nutritional counselling, immunization, screening, preventive interventions, birth preparedness, and timely referral when necessary. Despite the importance of antenatal care, inadequate and irregular attendance remains a public health concern in Nigeria, particularly among women who experience financial, geographical, social, cultural, and informational barriers to accessing maternal healthcare services. Antenatal health education provides pregnant women with information about the importance and timing of antenatal visits, recommended antenatal services, maternal and fetal health, pregnancy danger signs, nutrition, preventive interventions, birth preparedness, and the consequences of inadequate antenatal care attendance. Antenatal care attendance refers to the extent to which pregnant women initiate and attend recommended antenatal care visits during pregnancy and comply with scheduled follow-up appointments. In Nigeria, inadequate knowledge, misconceptions, perceived low risk of pregnancy complications, financial constraints, distance to health facilities, transportation difficulties, long waiting times, and other barriers may influence antenatal care attendance. Effective antenatal health education may improve women's knowledge and perceived benefits of antenatal care and encourage regular attendance. Against this background, this study investigates the influence of antenatal health education on antenatal care attendance among pregnant women in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains healthcare utilization behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding pregnant women's decisions to attend antenatal care. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping maternal healthcare behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from antenatal health education may influence antenatal care attendance. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal health education may influence antenatal care attendance among pregnant women in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from pregnant women attending selected antenatal care facilities in Nigeria using a structured interviewer-administered questionnaire and, where feasible, verification of antenatal attendance through clinic records or appointment cards. A multistage sampling technique will be employed to select states, local government areas, healthcare facilities, antenatal clinics, and eligible pregnant women. Where feasible, selected participant groups or healthcare facilities will be assigned to intervention and comparison groups. Pregnant women in the intervention group will receive structured antenatal health education covering the importance of antenatal care, recommended timing and frequency of visits, maternal and fetal health monitoring, pregnancy danger signs, nutrition, preventive interventions, screening services, birth preparedness, and the importance of complying with scheduled antenatal appointments. Antenatal care attendance will be assessed using indicators such as timing of first antenatal visit, number of antenatal visits attended, regularity of attendance, compliance with scheduled appointments, and completion of recommended antenatal contacts according to applicable national and international guidance. Descriptive statistics will be used to summarize participants' sociodemographic and obstetric characteristics, antenatal health knowledge, exposure to education, and attendance patterns. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the influence of antenatal health education on antenatal care attendance. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that antenatal health education has a significant positive influence on antenatal care attendance among pregnant women in Nigeria. Pregnant women exposed to structured antenatal health education are expected to demonstrate improved understanding of the importance of antenatal care and greater willingness to initiate care early and attend appointments regularly. Education may improve awareness of pregnancy danger signs, increase perceived benefits of antenatal services, reduce misconceptions, and encourage compliance with scheduled visits. However, antenatal health education alone may not guarantee regular attendance because pregnant women may continue to face barriers such as transportation costs, long distances to health facilities, financial constraints, long waiting times, inadequate healthcare services, family responsibilities, and other socioeconomic or health-system challenges. Consequently, antenatal health education is expected to be most effective when supported by accessible and affordable maternal healthcare services, efficient appointment systems, reliable transportation, respectful maternity care, and community-based maternal health interventions. The study is expected to contribute to the literature on antenatal care, maternal health education, maternal healthcare utilization, reproductive health, maternal and newborn health, and public health in Nigeria by providing empirical evidence on the influence of antenatal health education on antenatal care attendance among pregnant women. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, local government health authorities, primary healthcare facilities, healthcare professionals, midwives, community health workers, development partners, and policymakers regarding strategies for improving antenatal care utilization. The study will also provide evidence-based recommendations for strengthening antenatal health education, improving maternal health communication, reducing barriers to antenatal attendance, strengthening appointment and follow-up systems, improving accessibility and quality of antenatal services, and promoting regular antenatal care attendance among pregnant women across Nigeria.
