Impact of Postpartum Depression Awareness on Postpartum Depression Recognition among New Mothers in Nigeria
Abstract
Postpartum depression is an important maternal mental health concern that may affect women following childbirth and can have consequences for maternal wellbeing, mother–infant interaction, family functioning, and child development. New mothers in Nigeria may experience physical recovery from childbirth, sleep disruption, breastfeeding challenges, financial pressures, relationship difficulties, limited social support, and other postpartum stressors that may increase vulnerability to postpartum depression. Despite its potential consequences, postpartum depression may remain unrecognized because symptoms can be mistaken for normal postpartum experiences, while stigma, cultural beliefs, limited awareness, and inadequate access to mental health services may further delay recognition and appropriate support. Postpartum depression awareness provides an opportunity to improve mothers' understanding of the condition, including its symptoms, risk factors, warning signs, and the importance of seeking professional assistance. Against this background, this study investigates the impact of postpartum depression awareness on postpartum depression recognition among new mothers in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and the Theory of Planned Behavior. The Health Belief Model explains how new mothers' perceptions of susceptibility to postpartum depression, perceived severity, perceived benefits of awareness, perceived barriers, and cues to action may influence their ability to recognize symptoms. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, observational learning, social support, and environmental influences in developing mental health recognition skills. The Theory of Planned Behavior explains how attitudes, subjective norms, perceived behavioural control, and behavioural intentions may influence mothers' recognition of symptoms and willingness to seek appropriate assistance. Collectively, these theoretical perspectives provide a suitable framework for explaining how postpartum depression awareness may influence postpartum depression recognition among new mothers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise new mothers receiving postnatal, maternal, infant-care, or community-based maternal health services in selected public and private healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, postnatal clinics, and eligible mothers. Postpartum depression awareness will be measured using indicators such as exposure to awareness activities, frequency and duration of awareness sessions, knowledge of postpartum depression, understanding of symptoms, recognition of emotional and behavioural warning signs, awareness of risk factors, understanding of protective factors, knowledge of available treatment options, awareness of counselling and mental health services, stigma awareness, family-support information, and knowledge of appropriate referral pathways. Postpartum depression recognition will be assessed using indicators such as the ability to identify common symptoms, distinguish postpartum depression from expected emotional changes after childbirth, recognize persistent sadness, loss of interest, excessive worry, feelings of worthlessness, significant sleep or appetite changes beyond expected postpartum disruptions, difficulty functioning, and thoughts of self-harm or harm requiring urgent professional attention. Data will be collected using structured questionnaires, clinical case scenarios, and appropriately validated postpartum depression recognition instruments administered before and after the awareness intervention where a quasi-experimental design is used. Descriptive statistics will be used to summarize mothers' demographic, socioeconomic, obstetric, and postpartum characteristics, exposure to postpartum depression awareness, and recognition levels. Inferential statistical techniques, including paired or independent sample tests, chi-square tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of postpartum depression awareness on postpartum depression recognition. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that postpartum depression awareness has a significant positive impact on postpartum depression recognition among new mothers in Nigeria. Mothers exposed to structured postpartum depression awareness activities are expected to demonstrate greater ability to recognize symptoms, warning signs, risk factors, and situations requiring professional assessment compared with their baseline recognition or mothers without similar awareness exposure. Awareness activities delivered through postnatal clinics, healthcare workers, educational materials, community outreach, group discussions, and other maternal health platforms may help mothers distinguish clinically important symptoms from common postpartum emotional experiences. Improved recognition may encourage earlier communication with healthcare providers and facilitate timely assessment, treatment, counselling, and social support. However, stigma, cultural beliefs, misconceptions about motherhood and mental health, limited mental health services, financial barriers, inadequate postnatal follow-up, and insufficient family support may reduce the effectiveness of postpartum depression awareness activities. The study therefore expects accessible, culturally appropriate, practical, and sustained postpartum depression awareness to contribute significantly to improved recognition of postpartum depression among new mothers in Nigeria. The study is expected to contribute to the literature on postpartum depression awareness, postpartum depression recognition, maternal mental health, postnatal care, maternal wellbeing, mental health literacy, health education, maternal health promotion, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, primary healthcare facilities, postnatal clinics, obstetricians, midwives, nurses, psychologists, psychiatrists, counsellors, community health workers, maternal health programmes, development partners, and policymakers regarding strategies for improving early recognition of postpartum depression. The study will also provide evidence-based recommendations for integrating postpartum depression awareness into routine postnatal care, strengthening maternal mental health education, improving healthcare workers' capacity to recognize and communicate about postpartum depression, strengthening referral and follow-up systems, reducing stigma, increasing family and community support, and expanding access to appropriate mental health services for new mothers across Nigeria.
