Impact of Pharmacy Counselling Services on Medication Adherence among Patients in Nigeria

Abstract

Medication adherence is an important component of effective healthcare because the benefits of prescribed medicines depend largely on patients taking them correctly and consistently. Poor medication adherence may result in treatment failure, disease progression, avoidable complications, increased healthcare utilization, and unnecessary healthcare costs. Pharmacy counselling services provide an important opportunity for pharmacists and other qualified pharmacy personnel to educate patients about their medicines, including dosage, frequency, duration, potential side effects, drug interactions, storage requirements, and the importance of following prescribed treatment regimens. Pharmacy counselling services refer to structured medication-related education, guidance, communication, and support provided by pharmacy professionals to patients to promote safe and appropriate medicine use. Medication adherence refers to the extent to which patients take their medicines according to the prescribed dose, frequency, timing, and duration. In Nigeria, medication adherence may be affected by inadequate knowledge, misconceptions about medicines, fear of adverse effects, complex treatment regimens, financial constraints, forgetfulness, limited access to healthcare professionals, and inadequate patient counselling. Strengthening pharmacy counselling services may improve patients' understanding of their medicines and encourage appropriate and consistent medication use. Against this background, this study investigates the impact of pharmacy counselling services on medication adherence among patients in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Andersen's Behavioral Model of Health Services Use. The Health Belief Model explains medication-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between individual knowledge, behavioural capability, social influences, and environmental factors in shaping health behaviours. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how access to pharmacy services may influence medication-related behaviour. Collectively, these theoretical perspectives provide a suitable framework for explaining how pharmacy counselling services may influence medication adherence among patients in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from adult patients receiving prescribed medication from selected hospitals, primary healthcare facilities, and community pharmacies across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, pharmacies, and eligible patients. Pharmacy counselling services will be assessed using indicators such as receipt of medication counselling, explanation of medication purpose, dosage and frequency instructions, information on treatment duration, counselling on possible side effects, drug interaction information, medication storage guidance, clarification of patient concerns, provision of written medication information, follow-up counselling, and opportunity for patients to ask questions. Medication adherence will be assessed using indicators such as regular medication use, adherence to prescribed dosage and frequency, timely medication refills, missed doses, treatment interruption, and discontinuation of prescribed medicines. Structured questionnaires will be used for data collection, while pharmacy or medication records may be reviewed where available to complement self-reported information. Descriptive statistics will be used to summarize participants' characteristics, exposure to pharmacy counselling services, and medication adherence patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of pharmacy counselling services on medication adherence. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that effective pharmacy counselling services have a significant positive impact on medication adherence among patients in Nigeria. Patients who receive comprehensive and understandable counselling are expected to demonstrate better adherence to prescribed medication regimens than patients who receive limited or no counselling. Appropriate counselling may improve patients' understanding of the purpose of their medicines, correct misconceptions, clarify dosage and timing requirements, prepare patients for possible side effects, and encourage them to seek professional advice rather than discontinue treatment independently. Opportunities for patients to ask questions and receive individualized guidance may further improve confidence in medication use. Conversely, inadequate counselling may contribute to incorrect medication use, missed doses, premature treatment discontinuation, and poor understanding of prescribed regimens. However, pharmacy counselling services alone may not fully determine medication adherence because medication cost, availability of medicines, treatment complexity, side effects, forgetfulness, disease severity, patient beliefs, family support, and accessibility of healthcare services may also influence adherence. The study is expected to contribute to the literature on pharmacy counselling, medication adherence, pharmaceutical care, patient education, medication safety, healthcare utilization, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Pharmacy Council of Nigeria, hospitals, primary healthcare facilities, community pharmacies, pharmacists, other healthcare professionals, pharmaceutical care programme managers, development partners, and policymakers regarding strategies for improving medication use. The study will also provide evidence-based recommendations for strengthening patient-centred pharmacy counselling, improving pharmacists' communication skills, ensuring adequate counselling time, developing standardized medication counselling procedures, providing written medication information, strengthening follow-up counselling, and integrating pharmaceutical care services into routine healthcare delivery to improve medication adherence among patients across Nigeria.
