Impact of Patient Complaint Management on Patient Satisfaction in Public Hospitals in Nigeria
Abstract
Patient complaints provide important information about patients' experiences of healthcare services and may identify problems relating to communication, waiting time, staff attitudes, service availability, billing, clinical care, facility conditions, and other aspects of hospital operations. Effective patient complaint management involves the systematic receipt, documentation, investigation, response, resolution, and monitoring of complaints raised by patients and their representatives. Patient satisfaction refers to the extent to which patients' expectations and perceptions of healthcare services are met, including satisfaction with the quality of care, communication, responsiveness, waiting time, staff behaviour, accessibility, and overall healthcare experience. In public hospitals in Nigeria, ineffective complaint-handling mechanisms may leave patients feeling ignored, frustrated, or dissatisfied with healthcare services, while timely and transparent responses to complaints may improve patients' perceptions of fairness, responsiveness, accountability, and service quality. Effective complaint management may therefore serve as an important mechanism for improving patient experience and identifying opportunities for continuous quality improvement. Against this background, this study investigates the impact of patient complaint management on patient satisfaction in public hospitals in Nigeria. The study is anchored on Donabedian's Structure-Process-Outcome Model, Expectancy-Disconfirmation Theory, and Total Quality Management Theory. Donabedian's Model provides a framework for understanding complaint management as part of the healthcare service process and examining how improvements in service processes may influence patient outcomes and experiences. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing patients' expectations with their actual healthcare experiences, including the way complaints are handled and resolved. Total Quality Management Theory emphasizes continuous improvement, customer or patient focus, staff involvement, service monitoring, and the use of feedback to improve organizational performance. Collectively, these theoretical perspectives provide a suitable framework for explaining how patient complaint management may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from patients attending selected public hospitals in Nigeria using structured patient questionnaires and a patient complaint-management assessment tool. A multistage sampling technique will be employed to select states, public hospitals, departments or service units, and eligible patients. Patient complaint management will be assessed using indicators such as availability of formal complaint channels, accessibility of complaint procedures, complaint documentation, acknowledgement of complaints, response time, investigation procedures, communication with complainants, resolution of complaints, feedback to patients, escalation procedures, and monitoring of recurring complaints. Where feasible, hospital complaint registers, complaint-management records, patient-relations reports, and quality-improvement documents will be reviewed to complement patient responses. Patient satisfaction will be assessed using indicators such as satisfaction with healthcare quality, staff attitude, communication, waiting time, responsiveness, accessibility, complaint resolution, confidence in the hospital, and overall satisfaction. Additional information will be collected on patients' sociodemographic characteristics, type of service received, frequency of hospital visits, previous complaint experience, waiting time, and healthcare costs. Descriptive statistics will be used to summarize patient characteristics, complaint-management practices, complaint experiences, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of patient complaint management on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that effective patient complaint management has a significant positive impact on patient satisfaction in public hospitals in Nigeria. Patients who perceive complaint procedures as accessible, responsive, transparent, and effective are expected to report higher levels of satisfaction than patients who experience ineffective or inaccessible complaint-handling systems. Prompt acknowledgement and investigation of complaints are expected to improve patients' perceptions of responsiveness and institutional accountability, while appropriate resolution and communication of outcomes may help restore trust following negative healthcare experiences. Complaint-management systems may also provide hospital administrators with valuable information for identifying recurring service deficiencies and implementing corrective measures. However, complaint management alone may not fully determine patient satisfaction because patients' experiences are also influenced by clinical outcomes, waiting time, healthcare worker attitudes, medicine availability, facility conditions, healthcare costs, and accessibility. Consequently, effective complaint management should form part of a broader patient-centred healthcare quality-improvement strategy. The study is expected to contribute to the literature on patient complaint management, patient satisfaction, healthcare quality, patient experience, hospital administration, healthcare accountability, service improvement, and public health in Nigeria by providing empirical evidence on how complaint-handling practices influence patients' perceptions of public hospital services. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospitals, patient-relations departments, healthcare administrators, healthcare workers, quality-improvement teams, regulatory bodies, development partners, and policymakers regarding strategies for strengthening patient-centred healthcare. The study will also provide evidence-based recommendations for establishing accessible complaint channels, improving complaint documentation, reducing response and resolution times, strengthening communication with complainants, training healthcare workers in complaint management, using complaint data for quality improvement, and promoting accountability and responsiveness within Nigerian public hospitals.
