Impact of Medical Equipment Maintenance on Healthcare Service Quality in Nigerian Public Hospitals

Abstract

Medical equipment is essential for diagnosis, monitoring, treatment, surgery, emergency care, laboratory investigation, and other clinical services in modern hospitals. Effective maintenance of medical equipment is therefore important for ensuring that healthcare facilities have functional, safe, accurate, and reliable equipment available when required. Medical equipment maintenance refers to the systematic inspection, servicing, calibration, repair, preventive maintenance, corrective maintenance, and replacement of medical equipment to ensure optimal functionality and safety. Healthcare service quality refers to the extent to which healthcare services are safe, effective, timely, efficient, patient-centred, equitable, and responsive to patients' healthcare needs. In Nigerian public hospitals, medical equipment may experience frequent breakdowns, inadequate preventive maintenance, delayed repairs, limited availability of spare parts, insufficient technical personnel, and poor maintenance documentation. These challenges may result in equipment downtime, delayed diagnosis and treatment, unnecessary referrals, prolonged patient waiting times, increased operational costs, and reduced healthcare service quality. Conversely, effective equipment maintenance may improve equipment availability, reduce service interruptions, support accurate diagnosis and treatment, and enhance the efficiency and reliability of healthcare delivery. Against this background, this study investigates the impact of medical equipment maintenance on healthcare service quality in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, Total Quality Management Theory, and Systems Theory. Donabedian's Model identifies medical equipment and its maintenance as important structural components that influence healthcare processes and outcomes. Total Quality Management Theory emphasizes continuous improvement, preventive quality management, standardization, process monitoring, and organizational commitment to service quality. Systems Theory views the hospital as an interconnected system in which equipment, healthcare workers, infrastructure, maintenance systems, clinical processes, and patients interact to produce healthcare outcomes. Collectively, these theoretical perspectives provide a suitable framework for explaining how medical equipment maintenance may influence healthcare service quality in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected Nigerian public hospitals using structured hospital equipment assessment checklists, healthcare worker questionnaires, patient questionnaires, maintenance records, and healthcare service-quality assessment tools. A multistage sampling technique will be employed to select states, public hospitals, clinical departments, healthcare workers, patients, and relevant medical equipment. Medical equipment maintenance will be assessed using indicators such as availability of preventive maintenance schedules, frequency of routine servicing, calibration practices, corrective maintenance procedures, equipment downtime, response time to equipment faults, availability of spare parts, availability of qualified biomedical engineering personnel, maintenance documentation, maintenance expenditure, and equipment replacement practices. Healthcare service quality will be assessed using indicators such as equipment availability for clinical procedures, diagnostic accuracy, timeliness of care, patient safety, service continuity, waiting time, adherence to clinical standards, responsiveness, patient satisfaction, and occurrence of service interruptions caused by equipment failure. Where feasible, hospital maintenance logs, equipment inventories, service contracts, repair records, calibration certificates, downtime records, and quality-improvement reports will be reviewed to complement questionnaire data. Descriptive statistics will be used to summarize hospital characteristics, equipment maintenance practices, equipment functionality, and healthcare service-quality indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of medical equipment maintenance on healthcare service quality. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that effective medical equipment maintenance has a significant positive impact on healthcare service quality in Nigerian public hospitals. Hospitals with regular preventive maintenance, timely corrective repairs, effective calibration systems, adequate technical support, and proper maintenance documentation are expected to experience fewer equipment breakdowns and shorter periods of equipment downtime. Reliable equipment availability is expected to improve diagnostic and treatment processes, reduce delays, strengthen patient safety, support continuity of care, and improve healthcare providers' ability to deliver appropriate services. Conversely, inadequate maintenance may increase equipment failures, delay diagnosis and treatment, increase referrals, prolong patient waiting time, and reduce patients' confidence in healthcare services. Effective maintenance may also reduce the long-term cost of equipment ownership by preventing avoidable deterioration and extending equipment useful life. However, equipment maintenance alone may not fully determine healthcare service quality because quality is also influenced by healthcare worker availability and competence, medicine supply, infrastructure, hospital management, financing, infection prevention, and patient-related factors. Therefore, equipment maintenance should be integrated into broader hospital quality-improvement and health-system strengthening strategies. The study is expected to contribute to the literature on medical equipment maintenance, healthcare service quality, hospital infrastructure, healthcare technology management, biomedical engineering, patient safety, hospital administration, and public health in Nigeria by providing empirical evidence on the relationship between equipment maintenance and the quality of healthcare services. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, medical directors, hospital administrators, biomedical engineering departments, healthcare quality-improvement teams, healthcare professionals, development partners, and policymakers regarding strategies for strengthening medical equipment management. The study will also provide evidence-based recommendations for establishing preventive maintenance programmes, strengthening biomedical engineering capacity, improving maintenance documentation, ensuring timely procurement of spare parts, implementing equipment calibration and safety checks, monitoring equipment downtime, developing equipment replacement plans, strengthening maintenance budgets, and improving accountability for medical equipment management in Nigerian public hospitals.
