Impact of Maternal Malaria Prevention on Malaria Infection among Newborns in Nigeria

 

Abstract

Malaria remains a major public health problem in Nigeria and poses significant risks to pregnant women and their newborns, particularly in malaria-endemic communities. Maternal malaria during pregnancy may contribute to maternal anaemia, placental malaria, low birth weight, preterm birth, fetal growth restriction, and other adverse pregnancy outcomes. Newborns and young infants may also be vulnerable to malaria infection, particularly where maternal infection, environmental exposure, and inadequate preventive measures contribute to early-life transmission. Maternal malaria prevention through intermittent preventive treatment during pregnancy, use of insecticide-treated nets, appropriate diagnosis and treatment of malaria, health education, and other recommended preventive measures provides an important opportunity to reduce malaria-related risks during pregnancy and early infancy. However, inadequate antenatal care attendance, low uptake of preventive interventions, limited access to malaria services, poor adherence to preventive treatment, insecticide-treated net non-use, and socioeconomic and geographical barriers may reduce the effectiveness of maternal malaria prevention programmes. Against this background, this study investigates the impact of maternal malaria prevention on malaria infection among newborns in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how pregnant women's perceptions of susceptibility to malaria, perceived severity, perceived benefits of preventive measures, perceived barriers, and cues to action may influence participation in maternal malaria prevention. The Social Ecological Model emphasizes the influence of individual, household, community, healthcare, environmental, and policy factors on malaria prevention and newborn health. The Health Systems Framework emphasizes antenatal service delivery, health workforce, health information, availability of preventive commodities and medicines, financing, governance, and accessibility as important components of effective malaria prevention programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how maternal malaria prevention may influence malaria-related outcomes among newborns in Nigeria. The study will adopt a quantitative analytical cohort or quasi-experimental research design. The study population will comprise pregnant women receiving antenatal care in selected healthcare facilities in Nigeria and their newborns. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, antenatal clinics, pregnant women, and eligible mother-newborn pairs. Maternal malaria prevention will be assessed using indicators such as receipt and number of doses of intermittent preventive treatment during pregnancy, use of insecticide-treated nets, malaria testing during pregnancy, appropriate treatment of confirmed malaria infection, antenatal care attendance, malaria prevention education, and adherence to recommended preventive measures. Malaria infection among newborns will be assessed using indicators such as laboratory-confirmed malaria infection where appropriate diagnostic testing is available, malaria parasites detected through approved diagnostic methods, suspected malaria episodes, fever-related healthcare visits, malaria-related hospitalization, and other relevant clinical outcomes during the defined follow-up period. Maternal malaria status during pregnancy and relevant birth outcomes may also be assessed to provide additional epidemiological context. Data will be collected using structured questionnaires, antenatal care records, maternal malaria prevention records, laboratory reports, insecticide-treated net distribution records, delivery records, newborn medical records, hospital registers, and relevant malaria surveillance documents. Descriptive statistics will be used to summarize maternal and newborn characteristics, malaria prevention coverage, maternal malaria exposure, and newborn malaria outcomes. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of maternal malaria prevention on malaria infection among newborns. Where a cohort design is adopted, malaria-related outcomes among newborns of mothers with adequate malaria prevention during pregnancy may be compared with outcomes among newborns of mothers with inadequate or no documented preventive interventions, with relevant maternal, environmental, and socioeconomic factors appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective maternal malaria prevention has a significant positive impact on reducing malaria infection and malaria-related health risks among newborns in Nigeria. Newborns whose mothers received recommended malaria preventive interventions during pregnancy are expected to have a lower risk of malaria-related illness than newborns whose mothers received inadequate or no preventive interventions. Effective maternal prevention may reduce maternal malaria infection and placental malaria and thereby reduce malaria-related risks during pregnancy and early infancy. However, the relationship between maternal malaria prevention and malaria infection among newborns may also be influenced by environmental exposure, mosquito-vector density, household malaria prevention practices, maternal infection, and the timing of postnatal exposure. Inadequate antenatal care attendance, incomplete preventive treatment, poor insecticide-treated net use, limited access to diagnostic and treatment services, and persistent malaria transmission may reduce programme effectiveness. The study therefore expects integrated maternal malaria prevention, appropriate antenatal care, malaria diagnosis and treatment, household vector-control measures, and continued protection of infants after birth to contribute significantly to reducing malaria-related risks among newborns in Nigeria. The study is expected to contribute to the literature on maternal malaria prevention, newborn malaria, malaria in pregnancy, antenatal care, malaria prevention, maternal and child health, vector-borne diseases, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, National Malaria Elimination Programme, state ministries of health, healthcare facilities, maternal and child health programmes, community health workers, malaria control programme managers, development partners, and policymakers regarding strategies for strengthening malaria prevention during pregnancy. The study will also provide evidence-based recommendations for improving intermittent preventive treatment coverage, strengthening antenatal malaria services, increasing insecticide-treated net use, improving malaria testing and treatment during pregnancy, strengthening maternal malaria health education, improving monitoring of preventive interventions, and integrating maternal malaria prevention with broader maternal, newborn, and child health programmes across Nigeria.
